The ear canal is very narrow, and swelling with pain is extremely distressing. If accompanied by Sha (a condition characterized by acute stagnation of Qi and blood, often manifesting as darkened subcutaneous veins), the pathology will inevitably intensify. Treatment must prioritize resolving Sha; in this way, the efficacy for treating ear pain can be verified.
Clinical Cases:
One patient, Zheng Weihe, had pus discharging from his left ear, with swelling and pain radiating to the left Taiyang region and shoulder/hypochondrium. His right pulse was deep and faint, while the left Guan position was fine and choppy. Sha vessels were observed, and over ten needles were inserted into the popliteal fossa of the left leg, after which the pain subsided. Internally, Rusheng San (Sage-like Powder) was administered, taken slightly cooled externally; topically, a single piece of sheep manure wrapped in cotton was inserted into the ear, resulting in immediate recovery.
Another patient, Weng Zouxi, suffered from swelling and pain in the right ear, restless day and night. His pulse was faint and tight. Twelve needles were used to release Sha from the popliteal fossa, yielding purple-black blood. Qingliang Zhibao Yin (Cooling Precious Treasure Drink), modified by removing Xixin (Asarum Root) and adding Niuxi (Achyranthes Root) and Shihu (Dendrobium Stem), was administered slightly cooled, leading to recovery.
When Sha qi attacks the Zang-Fu organs via the arms, it is termed Jin Sha (Tight Sha); when it does not attack the Zang-Fu, it is termed Man Sha (Slow Sha). For symptoms of Man Sha, only needling the arms is required; for symptoms of Jin Sha, Sha formulas should be taken concurrently.
Clinical Cases:
One patient experienced pain within the sinews of the right arm. Observing Sha vessels, three needles were inserted, releasing toxic blood, after which the arm pain resolved.
Gao Songyun suffered from left arm pain, which did not respond to medical treatment. Upon observing Sha vessels on the arm, four needles were inserted, releasing toxic blood, resulting in recovery without medication.
Swelling and toxicity that are Yang in nature are called Yong (Carbuncle/Erysipelas); those that are Yin in nature are called Ju (Deep Abscess/Gangrene). Often, these conditions arise with concurrent Sha. If Sha is not addressed first, attempting to eliminate the toxin alone will be ineffective.
Clinical Cases:
A patient surnamed Wang presented with a white swelling the size of a plate between the waist and kidneys, unable to turn or move in bed, suffering extreme pain for nearly two months. Palpating the pulse, it was wiry, tight, or hidden. I stated, "This is toxin complicated by Sha." Sha was released first, followed by treatment for the toxin. After half a month, a small amount of pus was discharged, and he recovered.
A patient had a thigh swollen and red like a melon. Sha was released first, followed by treatment, resulting in recovery.
A patient had isolated redness and swelling of the left scrotum. Sha was released first, followed by treatment, resulting in recovery.
A patient suffered extreme lower abdominal pain with the development of a toxic sore, flat and white. Sha was released first, resulting in recovery.
A patient developed a black Ding (malignant furuncle) on the right arm. Sha was released first, resulting in recovery.
A patient developed a Ju (deep abscess) on the right kidney area, large as a small plate, black in color, with dozens of openings, lacking sensation of pain or itch, accompanied by fever and anorexia. Observing Sha vessels on the left leg, Sha was released; body temperature normalized, appetite returned, and he recovered in four days.
A patient developed a Xuan Yong (suspicious carbuncle) and concurrently suffered from Heng Xuan (buboes/lymphadenitis). A practitioner skilled in releasing Sha was invited to insert six needles on the coccyx and seven needles in the popliteal fossae, resulting in recovery.
A patient suffered from E Zhang Feng (hand fungus/neurodermatitis). Sha was released, resulting in recovery.
A patient suffered from Da Ma Feng (Leprosy), with hands and feet curled into fists, a pitiful appearance. Observing Sha vessels, this case was recorded.
