治劳伤心肾，精血亏损，形体羸瘦，阴虚火旺，咳嗽潮热，失血等证
，脉来弦长紧实，滑数有力者，皆火郁内实，不受补者也，宜此主之。
当归（身，酒洗，一钱五分） 生地黄（忌铁，一钱五分，姜汁蒸） 白芍药（煨，一钱）
玄参（忌铁，一钱） 知母（去毛，忌铁，蜜炒，一钱） 黄柏（忌铁，蜜炒，褐色，一钱） 白
茯苓（去皮，一钱） 沙参（一钱五分） 北五味（二十粒） 黄 （蜜炒绵软者，一钱） 天门冬
（去心、皮，一钱五分） 甘草（炙八分） 麦门冬（去心，一钱）
上十二味，皆考《本草》，于前证所必用者，但如症而增减之，自可捷其验也。
若潮热甚者，加柴胡一钱，地骨皮一钱。若肺火盛而咽燥口腥者，加蜜炒枯芩一钱，天
花粉一钱。若肺热咳甚痰多者，加炒栝蒌仁一钱，贝母八分。若肺燥咳血，加炒阿胶珠一
钱，牡丹皮八分，藕节三个，童便一杯，生姜汁五匙，对服。若吐血加郁金三分藕节、
棕灰，减黄 。若肺火郁甚而嗽血者，倍片芩、生地，加桔梗、丹参、阿胶、藕汁、童便对
服，亦减黄 。若自汗甚，倍黄 ，加麻黄根五分，或加牡蛎粉。若盗汗，倍归、地，加
酸枣仁、茯神、牡蛎。若衄血，加紫参、磨犀角、片芩、炒黑栀子，倍生地黄。若梦遗，加
山药、山茱萸、茯神、莲须，或加牡蛎。若食少便溏，加山药、土炒白术，减知母。若咳而
声嘶，此火郁肺损，加诃子，倍天门冬，倍麦冬。若咳唾脓血喘嗽者，加紫菀茸。肺热燥
咳上气喘急，加百部。
To treat conditions resulting from overwork injuring the Heart and Kidney, with deficiency of essence and blood, emaciation of the body, Yin deficiency with Fire hyperactivity, cough with tidal fever, hemoptysis, etc. If the pulse is wiry, long, tight, and solid, slippery and rapid yet forceful, these all indicate Fire stagnation internally presenting as excess, which cannot tolerate tonification. This main formula should be used.
Danggui (Radix Angelicae Sinensis, use the root body, washed with wine, 1.5 qian) Shengdihuang (Rehmanniae Radix, avoid iron, 1.5 qian, steamed with ginger juice) Baishao (Radix Paeoniae Alba, roasted in hot ashes, 1 qian) Xuanshen (Scrophulariae Radix, avoid iron, 1 qian) Zhimu (Anemarrhenae Rhizoma, remove hairs, avoid iron, stir-fried with honey, 1 qian) Huangbai (Phellodendri Cortex, avoid iron, stir-fried with honey until brown, 1 qian) Bai Fuling (Poria, remove skin, 1 qian) Shashen (Glehniae Radix, 1.5 qian) Bei Wuwei (Schisandrae Fructus, 20 grains) Huangqi (Astragali Radix, select those stir-fried with honey until soft, 1 qian) Tianmendong (Asparagi Radix, remove heart and skin, 1.5 qian) Gancao (Glycyrrhizae Radix et Rhizoma, honey-roasted, 0.8 fen) Maidong (Ophiopogonis Radix, remove heart, 1 qian)
The above twelve herbs are all verified in *Bencao* (Compendium of Materia Medica) and are essential for the aforementioned symptoms. By adjusting the dosage according to the specific condition, efficacy can be achieved rapidly.
If tidal fever is severe, add Chaihu (Bupleuri Radix, 1 qian) and Digupi (Lycii Cortex, 1 qian). If Lung Fire is excessive causing dry throat and foul taste in the mouth, add Kuqin (Stir-fried Scutellariae Radix until charred yellow, 1 qian) and Tianhuafen (Trichosanthis Radix, 1 qian). If Lung Heat causes severe cough with much phlegm, add Gualouren (Stir-fried Trichosanthis Semen, 1 qian) and Beimu (Fritillariae Bulbus, 0.8 fen). If Lung Dryness causes hemoptysis, add Ejiaozhu (Stir-fried Donkey-hide Gelatin beads, 1 qian), Mudanpi (Moutan Cortex, 0.8 fen), Oujie (Nelumbinis Node, three pieces), Tongbian (Infant urine, one cup), and Jiangzhi (Ginger juice, five spoons), taken together. If hematemesis occurs, add Yujin (Curcumae Radix, 0.3 fen), Oujie (Nelumbinis Node), Zonghui (Livistona Fibre ash), and reduce Huangqi (Astragali Radix). If Lung Fire stagnation is severe causing coughing blood, double Pianqin (Scutellariae Radix) and Shengdihuang (Rehmanniae Radix), add Jiegeng (Platycodonis Radix), Danshen (Salviae Miltiorrhizae Radix), Ejiao (Donkey-hide Gelatin), Oujiang (Lotus node juice), and Tongbian (Infant urine), taken together; also reduce Huangqi (Astragali Radix). If spontaneous sweating is severe, double Huangqi (Astragali Radix), add Mahuanggen (Ephedrae Cauliculus, 0.5 fen), or add Muli Fen (Ostreae Concha powder). If night sweats occur, double Danggui (Radix Angelicae Sinensis) and Dihuang (Rehmanniae Radix), add Suanzaoren (Ziziphi Spinosae Semen), Fushen (Poria cum Pinus Radix), and Muli (Ostreae Concha). If epistaxis occurs, add Zishen (Periplocae Radix), Xijiao (Rhinocerotis Cornu, ground), Pianqin (Scutellariae Radix), and Heizhizi (Stir-fried black Gardeniae Fructus), double Shengdihuang (Rehmanniae Radix). If nocturnal emission occurs, add Shanyao (Dioscoreae Rhizoma), Shanzhuyu (Corni Fructus), Fushen (Poria cum Pinus Radix), Lianxu (Nelumbinis Stamen), or add Muli (Ostreae Concha). If appetite is poor and stool is loose, add Shanyao (Dioscoreae Rhizoma), Tuchao Baizhu (Earth-stir-fried Atractylodis Macrocephalae Rhizoma), and reduce Zhimu (Anemarrhenae Rhizoma). If coughing leads to hoarseness, this indicates Fire stagnation damaging the Lung; add Hezi (Terminalia Chebulae Fructus), double Tianmendong (Asparagi Radix), and double Maidong (Ophiopogonis Radix). If coughing up pus and blood with wheezing and cough occurs, add Ziwan Rong (Asteli Flos). If Lung Heat Dryness causes cough with shortness of breath and急促喘急 (rapid panting), add Baibu (Stemonae Radix).
