(The so-called epidemic toxic syndrome in common parlance specifically refers to this heat syndrome.)
In the initial stage, white membranes appear at the exterior gate (Waiguan). They may be thin or small, pale red with slight swelling, and slightly painful. The voice is resonant, while there is slight obstruction in the jaw and swallowing. Symptoms include dry mouth, head heaviness, eye distension, slightly yellow tongue coating, and slightly yellow urine; these constitute the clinical presentation. At this stage, the pathogenic heat remains in the exterior. Treatment should prioritize Ren Shen Bai Du San (Ginseng Detoxification Powder), Sheng Yang San Huo Tang (Ascending Yang and Dispersing Fire Decoction), or Lian Qiao Yin (Forsythia Drink) with modifications.

If the white membrane appears both inside and outside the gate, the color will necessarily be dry and scorched, possibly yellow and protruding, dense and abundant. There is lockjaw, severe redness and swelling of the entire throat, excruciating pain, copious phlegm and saliva, difficulty in swallowing food, slurred speech, deep yellow or even scorched black tongue coating with prickles, thirst, foul breath, constipation, dysuria, red eyes, irritability, a sensation of lightness in the body, and aversion to heat; these constitute the clinical presentation. At this stage, the pathogenic heat has entered the interior. Treatment should prioritize Da Yuan Yin (Reach the Origin Drink), Pu Ji Xiao Du Yin (Universal Benefit Toxin-Resolving Drink), or Qing Yan Li Ge Tang (Clear Throat and Benefit Diaphragm Decoction) with modifications.
