Shigao (Gypsum Fibrosum), Mutong (Akebiae Caulis), Beimu (Fritillariae Bulbus, removed heart and pounded), Sangye (Mori Folium, raw use), Lianqiao (Forsythiae Fructus), Niubangzi (Arctii Fructus, raw use and pounded), Banlangen (Isatidis Radix), Yinhua (Lonicerae Japonicae Flos), Chantui (Cicadae Periostracum, removed feet and wings), Maidong (Ophiopogonis Radix, removed heart), Shengdi (Rehmanniae Radix), Shuwei Qingqin (Scutellariae Radix from mouse-tail variety).
The above twelve herbs are decocted in clear water. Take two doses, administered twice or three times a day. If the Taiyin root disease is severe while the Shaoyang marked symptoms are mild, remove Huangqin (Scutellariae Radix) and Niubangzi (Arctii Fructus), add Xiyangshen (Panax Ginseng C.A.Mey. var. major), and double the dosage of Shigao (Gypsum Fibrosum) and Maidong (Ophiopogonis Radix). If the marked symptoms are particularly severe, replace Huangqin (Scutellariae Radix) with Longdancao (Gentianae Radix). If phlegm and saliva are abundant and obstructing, continue to use Niubangzi (Arctii Fructus), or mix in fresh Laifu Zhi (Raphani Fructus Juice) and Niuhuang (Calculus Bovis), reduce Maidong (Ophiopogonis Radix) and Shengdi (Rehmanniae Radix), and add Xingren (Prunus Armeniaca Seed), Xuanfu (Inulae Flos), and Gualou Pi (Benincasae Exocarpium).
Fei Qingdai (Indigo Naturalis), Xi Niu Huang (Calculus Bovis), Lao Shi Da Bingpian (Borneolum Syntheticum, old type only, a small amount; new type must absolutely not be used), Xiguashuang (Prunus Armeniaca Kernel Frost), Xi Yuushi (Talcum), Lianzhu (Pearl).
The above drugs are ground into an extremely fine powder and blown into the throat using a bamboo tube. If the throat is dry, apply with high-quality white honey mixed as a paste. Alternatively, fresh Cebaiye (Platycladi Cacumen) can be crushed for juice and used for mixing. If dryness is severe, the throat-blowing method must not be used.
Shengdi (Rehmanniae Radix), Beimu (Fritillariae Bulbus), Maidong (Ophiopogonis Radix, removed heart and used), Shuwei Qingqin (Scutellariae Radix from mouse-tail variety), Lianqiao Jia (Forsythiae Fructus with calyx), Mutong (Akebiae Caulis), Yinhua (Lonicerae Japonicae Flos), Chantui (Cicadae Periostracum, removed wings and feet), Niubangzi (Arctii Fructus, raw use and pounded).
The above nine herbs are decocted in five cups of water until three cups remain. Divide into two doses and take warm, twice a day.
For Diphtheria (Baihou) symptoms as described previously: if at the onset there is mental restlessness (Xinfan), slight stiffness at the root of the tongue, and crimson tip, this indicates Taiyin Dryness-Fire complicated by Shaoyin Ministerial Fire. The primary formula is Zhubai Shuangqing San (Vermilion-White Double-Clearing Powder). For mild cases, Zhang's Shengong Pixie San (Divine Efficacy Evil-Expelling Powder) is used, removing Gegen (Puerariae Lobatae Radix), Jiangcan (Bombyx Batryticatus), Huangqin (Scutellariae Radix), Mabo (Belamcandae Rhizoma), Chantui (Cicadae Periostracum), and Qingguo (Canarii Fructus), and adding Zididing (Violae Herba), Zhuye (Bambusae Folium), Sheng Zhiren (Gardeniae Semen, raw), and Dengxincao (Junci Medulla).
This is a disease caused by Taiyin Dryness-Fire complicating Shaoyin Ministerial Fire. The Heart and Lung reside together in the Upper Jiao (Upper Burner) and are closest to the throat. When Lung Metal fails to descend and Dryness-Qi fails to circulate, it first invades the Heart. The Heart is a Fire organ, located close to the Lung position. Once Ministerial Fire boils, the Lung suffers first. Therefore, ordinary Warm-Thermal diseases transmit easily, especially when Dryness-Fire clashes and agitates each other. Mental restlessness indicates excessive internal heat. The Shaoyin meridian runs along the throat and connects to the root of the tongue. Intense Dryness-Fire scorches the Heart Ying (Nutrient Level), causing slight stiffness at the root of the tongue, and crimson tip in severe cases. Zhubai Shuangqing San focuses on purging the exclusive excess heat of Shaoyin, standing in contrast to the Shaoyang marked disease described in the previous section. These two conditions are actually the most common presentations in Diphtheria and require careful attention. The previous section discussed from the perspective of Shaoyang, thus using Huangqin (Scutellariae Radix) and Longdancao (Gentianae Radix), where bitter-draining methods are not contraindicated. This section emphasizes Shaoyin, thus using Huangbai (Phellodendri Cortex) and Mutong (Akebiae Caulis), relying specifically on bitter-descending methods. Lianqiao (Forsythiae Fructus) and Zhizi (Gardeniae Fructus) clear the accumulated heat within the Pericardium Luo (Collaterals); Sangye (Mori Folium) and Xingren (Prunus Armeniaca Seed) ventilate the consolidated heat of Lung Metal; Xuanshen (Scrophulariae Radix) purges Heart Fire to clear the Ying level; Zididing (Violae Herba) resolves the accumulated toxic heat. These two herbs are particularly important for clearing heat and cooling blood in Shaoyin. However, everywhere the treatment does not neglect the root disease of Dryness-Fire; therefore, both formulas use Shigao (Gypsum Fibrosum) as the Sovereign herb, without independently using Maidong (Ophiopogonis Radix) and Shengdi (Rehmanniae Radix). If the case is mild and the fire intensity has already diminished, moistening dryness and clearing heat addresses the key point, making it unnecessary to heavily use bitter-cold herbs that might aggravate dryness. In such cases, Shengong Pixie San (Divine Efficacy Evil-Expelling Powder) with modifications should be used.
