Stagnant blood rushes and obstructs the Heart, then transfers to the Lung. Qi and Blood combine to form a mass; entering the throat, it produces sound, resembling the chirping of a cicada. This condition is fatal in ten out of ten cases.
Stagnant blood, taking advantage of deficiency, flows into the meridians. Over time, it transforms into water-dampness, causing yellowish complexion and edema of the four limbs. If treated as Water-Qi disease (Shuiqi Bing), it will lead to premature death. It is appropriate to use Xiao Tiaojing Tang (Regulating Menses Decoction Minor) or Da Tiaojing Tang (Regulating Menses Decoction Major). When blood transforms into water, swelling worsens in the afternoon, and the swollen areas must show blood marks, or there is distending pain and stabbing pain in the hypochondrium.
Cough with difficult urination indicates Cold-Water insulting Spleen-Earth. It is appropriate to tonify the Spleen and Lung. Use Buzhong Yiqi Tang (Decoction for Supplementing the Middle and Augmenting Qi). For Qi deficiency, use Liujunzi Tang (Six Gentlemen Decoction). Remove Gancao (Licorice Root) from Sijunzi Tang (Four Gentlemen Decoction), add Yiren (Coix Seed) and Chao Chenbuzi (Fried Radish Seed).
If the six pulses are deep, fine, and powerless, accompanied by cough or incessant diarrhea, this is due to Spleen-Earth deficiency-cold and decline of Mingmen Fire (Gate of Life Fire). Use Shenqi Tang (Kidney Qi Decoction) from *Jinkui Yaolue* (Essential Prescriptions from the Golden Cabinet).
Thirst with aversion to cold, and a deep, powerless pulse indicate body deficiency with accumulation of damp-heat. There must be fullness and oppression in the Upper Jiao. It is appropriate to tonify the Middle and move Qi, combined with diuretic agents such as Baizhu (Atractylodes Macrocephala Rhizome), Fuling (Poria), Chenpi (Tangerine Peel), and Mutong (Akebia Stem).
Exogenous Wind-Cold causes stagnation of Qi and Blood in the meridians, or cramping/stabbing pain in the hypochondrium and abdomen. Vomiting and nausea require Jiawei Wuzhuyu Tang (Modified Evodia Decoction).
Qi stagnation causing distension is treated with Zhishi (Immature Bitter Orange) and Chao Baizhu (Fried Atractylodes Macrocephala Rhizome).
Postpartum fever may result from overexertion, Blood deficiency, retained stasis, milk engorgement, Wind-Cold invasion, or excessive eating. The etiologies vary, and symptoms resemble Shanghan (Cold Damage). Do not induce sweating, lest it lead to muscle twitching, Yang collapse with delirium, Yu Mao coma, and other conditions.
For Blood deficiency, use Chuanxiong (Szechwan Lovage Rhizome), Danggui (Angelica Sinensis Root), Renshen (Ginseng Root), and Jiangtan (Charred Ginger).
For Qi deficiency, use Buzhong Yiqi Tang (Decoction for Supplementing the Middle and Augmenting Qi) with fried Shengjiang (Fresh Ginger).
Acid regurgitation and belching of rotten food, with fullness and anorexia, indicate food stagnation. Use Sijunzi Tang (Four Gentlemen Decoction) with Sharen (Amomum Fruit), Shanzha (Hawthorn Fruit), and Shenqu (Medicated Leaven).
Liver deficiency with dry Blood and internal flare-up of Yin fire requires Xiaoyao San (Free and Easy Wanderer Powder) to clear Liver fire. Using heavy tonification will exacerbate heat and dryness.
Yin deficiency leaves Yang without foundation, causing it to float and scatter externally, manifesting as fever. Use Siwu Tang (Four Substances Decoction) with Zhi Ganjiang (Prepared Dried Ginger). Adding Fuling (Poria) allows Heavenly Qi to descend and generate Yin, which is also excellent.
Profuse blood loss with sweating, inability to lie down, palpitations, fright, delirious speech, and visual hallucinations require Jiawei Guipi Tang (Modified Restore the Spleen Decoction).
Blood deficiency with Wind invasion requires Shenghua Tang (Generate the New Decoction) with Jingjie Sui (Schizonepeta Spike).
Erroneous use of cold-cooling agents causes Cold Qi to block Yang on the exterior. Use Sijunzi Tang (Four Gentlemen Decoction) with Ganjiang (Dried Ginger) and Rougui (Cinnamon Bark). If ineffective, add Fuzi (Prepared Aconite Root).
Skin fever, red face, and extreme thirst indicate Hair-Blood depletion and dryness. Use Danggui Buxue Tang (Angelica Tonifying Blood Decoction).
Stagnant blood attacking the Heart, causing manic speech and running about, is treated with Gan Heye (Dried Lotus Leaf), Shengdi (Rehmannia Root), Danpi (Moutan Cortex), and Sheng Puhuang (Fresh Pollen Typhae).
Distending pain in the chest and hypochondrium, epigastric fullness, hematemesis, night sweats, or incessant hematochezia indicate deficiency and damage of various Zang organs, presenting as false heat due to deficiency. Use Shiquan Dabu Tang (Perfect Ten Great Tonifying Decoction).
Postpartum tidal fever with sweating and constipation should not be exclusively diagnosed as Yangming syndrome; headache with aversion to cold should not be exclusively diagnosed as Taiyang syndrome; headache with hypochondriac pain should not be exclusively diagnosed as Shaoyang syndrome. All stem from dual deficiency of Qi and Blood, and disharmony of Yin and Yang, mimicking exogenous diseases. Use Jiawei Shenghua Tang (Modified Generate the New Decoction) to harmonize Ying and Wei (Nutritive and Defensive levels); all symptoms will recede.
Dry mouth and throat with thirst should not be exclusively diagnosed as Shaoyin syndrome; abdominal fullness, dry throat, and solid stool should not be exclusively diagnosed as Taiyin syndrome; irritability, fullness, and cold limbs should not be exclusively diagnosed as Jueyin syndrome; delirium with constipation should not be exclusively diagnosed as dry feces in the Stomach. These often result from fatigue damaging the Spleen, delayed transformation and transportation, dry Blood, and dry intestines—deficiency patterns mimicking excess diseases. Use Jiawei Tongyou San (Modified Unblock You Powder).
