There is a type of nebula (corneal opacity), round like the moon, appearing generally the same in both yin and yang parts [of the eye], standing prominently on the cornea, clear and bright as seen in water. This is due to blood deficiency causing the spirit (Shen) to scatter, and essence (Jing) deficiency leading to incomplete qi (vital energy). To remove it from its root, one must obtain an immortal prescription.
This condition presents with a slight nebula obscuring the pupil. Although the patient may not be aware of it, they find it hard to endure the dimness; it is named "Empty Pool Reflecting the Moon" (Xu Tan Cheng Yue). This describes its smooth and deep appearance, seeming non-existent yet actually present. Whenever all kinds of dangerous external obstructions cause blindness, this will inevitably remain after recovery. Since it cannot be cured, there is no need to investigate its origin and progression.
Secular texts tend to use analogy, creating many names and categories. However, the treatment principles are the same. One could say they are truly unafraid of trouble. Fortunately, this is a medical book; if it were written as history, the value of the ink and brush would rival cities, let alone the price of paper.
Xiahou Dun died holding a grudge over his eaten eye; Ruan Ji's white eyes made him poor [in social standing]; furthermore, someone startles the secular world, cooking a stone snail to replace the pupil spirit [metaphor for drastic intervention].
This condition arises because the divine pearl [pupil/cornea] is steamed by head wind-phlegm-fire, becoming dead-colored and solid, resembling a cooked field snail, with convexity and concavity similar to it, hence the name. In the past, I have seen people suffering from this; initially disregarding it, but once the condition has formed, their eagerness to seek medical help is such that some would travel thousands of li willing to serve as a groom. It makes one sigh.
Autumn water is clear, dew is abundant; starlight is faint, sword light is cold. Where does the pupil spirit hide tonight? Sweep away the evil aura and look carefully.
This condition manifests as a scar caused by physical impact. The color is white or slightly scorched yellow, thicker in the center and thinner on the sides, lying horizontally across the black eye [cornea], thus called "Sword Crossing Autumn Water" (Jian Heng Qiu Shui). Severity varies: in severe cases, although the upper and lower eyelids [wind wheels] are exposed, the pupil spirit is covered, resulting in total loss of vision; in mild cases, it remains covered, and vision is also ambiguous. Even with miraculous pills, only slight improvement can be expected. If it persists for a long time, becoming sunk and slippery, with depressed contours, or adding fine filamentous tissue, it will never heal in this life. There is also the case of flickering heat obstruction, or excessive application of topical or oral cooling medicines, causing a line of rigid wind to attack upwards, scraping the corneal wheel. Initially unnoticed, after the disease recedes, a white nebula tightly adheres to the center, like a needle fixing a compass; this name is also similar. Treatment is even more difficult than the previous ones. As the saying goes, "A long sword leaning against the sky outside," requiring mountain-uprooting strength to move it.
