The patient's physical appearance and pulse are both deficient, with no sense of hunger and an inability to eat. In individuals debilitated by chronic overexertion, when cool air from late autumn invades from the exterior, it stirs up latent pathogens within, causing internal steaming that leads to irritability and thirst. This is no longer a syndrome treatable with Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), or Banxia (Pinelliae Rhizoma). It is urgent to rescue the body fluids and clear the latent pathogens.
Prescription: Zhuye (Bambusae Folium), Shengdi (Rehmanniae Radix), Lizhi (Pyrus Juice), Lianqiao (Forsythiae Fructus), Maidong (Ophiopogonis Radix), Zhezhijie (Sacchari Canne Juice).
If hemoptysis is not treated, but only the damp pathogen of the season is targeted, the illness may subside. Whenever one of the Six Qi prevails excessively, there must be a counter-reaction; once dampness is removed, dryness arrives. The violent heat of early autumn scorches the body fluids; therefore, one should first nourish the Stomach Yin, and after the White Dew solar term, a conditioning prescription can be established.
Prescription: Maidong (Ophiopogonis Radix), Renshen (Ginseng Radix et Rhizoma), Dazao (Jujubae Fructus), Banxia (Pinelliae Rhizoma), Shengcao (Glycyrrhizae Radix et Rhizoma), Jingsmi (Oryzae Semen).
The pulse is deficient and rapid, the body feels cold, there is irritability in the heart, and qi rebels upward causing coughing and choking after the fifth watch (3-5 AM). At the Autumn Equinox solar term, Dry Metal governs. After the release of great heat, all things tend to transform into dryness. The Lung is a delicate organ, situated highest in the Upper Jiao, and is the first to receive the impact of dryness. It is appropriate to moisten dryness to nourish the source of transformation.
Prescription: Dong Sangye (Mori Folium), Nan Huafen (Trichosanthis Radix), Sheng Yiren (Coicis Semen), Da Shashen (Adenophorae Radix), Yuzhu (Polygonati Odorati Rhizoma), Mi Zhi Juhong (Citri Reticulatae Pericarpium Flavum).
Usage: Take three he of white glutinous rice, wash thoroughly, soak in running water, extract the extremely clear liquid, and use this liquid instead of water to decoct the medicine.
Coughing has slightly decreased at night, allowing for brief sleep with eyes closed. Warm pathogens and internal heat have stolen the body fluids, leading to a state of dryness transformation. Autumn weather descends and changes color; the Lung is located highest, so the Upper Jiao is affected first. Generally, dampness rises from the earth, while dryness descends from heaven; this is a fixed principle. Now, the skin is rough and scaly like fish scales, and muscles are wasted; nothing happens faster than this. Here, we discuss the treatment of the Qi aspect, as the Upper Jiao governs Qi. We propose using Yu Shi's formula, removing blood-invigorating herbs, to specifically address dryness in the Upper Jiao.
Prescription: Jing Shuang Sangye (Mori Folium exposed to frost), Yuzhu (Polygonati Odorati Rhizoma), Tian Xingren (Armeniacae Semen Amarum), Pipaye (Eriobotryae Folium), Tian Lipi (Pyrus Pericarpium), Huafen (Trichosanthis Radix) [add Er Qian of filtered Shigao powder (Gypsum Fibrosum)].
In early autumn, sore throat and choking occur because summer heat and dry qi enter through respiration during the interaction of Qi. The Lung is located highest, and its clearing function is impaired; only light and clear herbs can resolve this. If medicines are too bitter, acrid, and cold, they damage the Stomach, reduce appetite, and leave the Upper Jiao obstructed. The ancients said, "When metal is empty, there is sound"; hoarseness and a rapid pulse suggest concerns about Fei Wei (Lung Withering).
Prescription: Tian Shui Li (Sweet Water Pear), Douling (Stemonae Radix or Aristolochiae Fructus - retaining original name), Beishashen (Glehniae Radix), Chuan Beimu (Fritillariae Cirrhosae Bulbus), Hezipi (Terminaliae Chebulae Pericarpium), Zhejiang (Sacchari Canne Juice), Tian Xingren (Armeniacae Semen Amarum).
Preparation: Decoct into a paste.
