On the second day, the eruptive spots were faint and failed to emerge smoothly. Purple patches appeared on the left Taiyang region and the back. Although purgatives had been administered, the spirit was already exhausted, making it difficult to endure further drastic purgation. The signs of internal stagnation and depression were obvious; immediately administering attacking and dispersing medicines would pose the risk of "reversal of arms" (collapse of healthy qi).

For a hundred-day-old infant who has not yet consumed grain food, the stomach relies on the sweetness of breast milk, and the fu-organ qi is tender and thin. The vibration caused by the eruption of smallpox makes the interior true qi feel desolate and uneasy. With complex medicinal flavors and weak stomach qi, vomiting and diarrhea are inevitable; this means the pathogen is not attacked while the righteous qi is injured. Moreover, for such a thin constitution, harmonization is suitable, while pushing and荡涤 (drastic purgation) should be avoided; this is the supreme principle. In short, it relies on heavenly fortune and cannot withstand artificial intervention. It is proposed to resolve the muscle layer and penetrate the toxin, with the nursing mother taking the medicine on behalf of the infant, simultaneously promoting the flow of breast milk.

The form is emaciated and the constitution is weak. Erupting during the summer solstice when qi disorders occur, sweating leads to loose skin pores, and summer heat injures qi. For five days, the appearance has been withered and shriveled, lacking luster, roundness, and fullness. Yuan qi cannot transport toxins to transform them into pus. Within the limited timeframe, if qi is not filled externally to irrigate the lesions, external peeling or internal collapse into various orifices will inevitably occur. Generally, treating smallpox requires discussing the body constitution. This is a case of supporting deficiency and holding toxins, which is the most difficult to regulate, distinctly different from robust excess fire. It is proposed to administer Xiong Gui Tang (Chuanxiong and Danggui Decoction) to harmonize blood and internally support/toxin-resist, assisting in filling and growing.

Empty blisters appear all over, refusing to fill with pus. Secular practitioners often adhere to the notion that "excess qi causes blisters," using cooling medicines to clear qi-fire. How can a deficient body have excess righteous qi? This is mainly because blood fails to attach to qi, resulting in empty accumulation. Therefore, inside Baoyuan Tang (Preserve Origin Decoction), internally supporting and carrying pus is a fixed supreme principle. If this condition can be supported past the twelfth day, and cooling detoxification medicines are administered, conforming to the principle of tonifying first and clearing later, there will be good news.

Facial and bodily pus are at seven or eight parts; although described as thin and clear, if protected and nourished without damage, they can also thicken and scab. However, for those with damaged forms on the chest and back, gathering pus is difficult, leaving only empty shells with little attachment at the root halo. Furthermore, accompanied by vomiting and nausea, rebellious qi, mental restlessness, and internal symptoms failing to settle, deficiency mixed with toxin cannot be settled by mere tonification. In the process of benefiting yuan qi and blood, cooling agents must surely be added. Regarding Weng's discussion on smallpox, excessive tonification was originally criticized. Smallpox within ten days conforms to the autumn/winter command; summer eruption involves venting, where attacking medicines to accelerate putrefaction and ulceration are strictly taboo. Repeatedly urged!

Quiet during the day, restless at night: yang hyperactivity and yin deficiency. Because the young body belongs to yang, it lacks yin harmony. Cooling and detoxifying can only address seasonal epidemic fire-toxin. The congenitally stored toxin manifests at the beginning of form; how can it be thoroughly cleared in moments? Theoretically, one should strengthen water to control yang and benefit yin to relieve distress, imitating Zhang Zhongjing's proposal.
Liujun Wan (Six-Ingredient Rehmannia Pill) minus Shanzhuyu (Corni Fructus), plus Shaoyao (Paeoniae Radix).
At the critical juncture, dispersed and scattered yang toxin appears on the head vertex. Internal deficiency cannot tolerate the bitter cold of Huang Qin (Scutellariae Radix) and Huang Lian (Coptidis Rhizoma). Temporarily advance a formula to benefit yin zang organs and yang fu organs, preventing deep penetration into the interior. (This prescription is lost).
Winter-warmth remains unresolved, spirit and qi are not clear. Smallpox is a heat syndrome, but the body is soft and skin white, indicating original deficiency. Cold-clearing medicines should not be excessive. As long as the stomach is unharmed, no transformation symptoms will occur. Thin shells and many pustules require careful nursing.
