[Taiping Wan]
Take Asparagi Radix (Tianmendong) and Ophiopogonis Radix (Mendongdong) (both with the hearts removed), Anemarrhenae Rhizoma (Zhimu), Fritillariae Cirrhosae Bulbus (Beimu), Periplocae Semen (Kuandonghua), and Armeniacae Semen Amarum (Xingren) (with skins and tips removed, each 2 liang); Danggui Radix (Danggui), Rehmanniae Radix Recens (Shengdihuang), Coptidis Rhizoma (Huanglian), and Rehmanniae Radix Preparata (Shudihuang); Ejiao (prepared by stir-frying with蛤粉 Ge Fen, each 1.5 liang); Puncteliae Cortex (Puhuang), Jingmo (Jingmo), Platycodonis Radix (Jiegeng), and Menthae Haplocalycis Herba (Bohe) (each 1 liang); and Moschus (Shexiang) a small amount (separately ground). The above herbs are processed into a fine powder, mixed with refined honey to form pills the size of bullet seeds. Take one pill each time, chew it thoroughly before going to sleep at night, and swallow with Mint Decoction (Bohe Tang). It treats consumptive disease (Laozheng), chronic cough, and lung abscess (Fei'ong). It is advisable to hold the pill in the mouth for gradual absorption to eradicate the root cause.
[Qie Wan / Er Shen Wan Base]
Treats spleen and stomach deficiency, characterized by diarrhea occurring in the early morning (Wugeng time), lack of appetite, or indigestion after eating.
Take Psoraleae Fructus (Pogu Zhi) (4 liang, stir-fried) and Myristicae Semen (Roudoukou) (2 liang, used raw).
Grind the above two herbs into powder. Use 49 large red dates (Hongzao) and 4 liang of fresh ginger (Shengjiang), chopped finely. Boil them in water until cooked, remove the ginger, take the date pulp, and mix it with the herbal powder to form pills the size of Sophora japonica seeds (Tongzi da).
Take 50 pills per dose.
My commentary: Academician Xu Shuwu records in *Ben Shi Fang* that Sun Simiao said, "Tonifying the kidney is inferior to tonifying the spleen." I argue, "Tonifying the spleen is inferior to tonifying the kidney." If kidney qi is weak, yang qi declines and fails to warm and steam the spleen and stomach. This causes coldness in the spleen and stomach, leading to chest and diaphragm fullness/stagnation, loss of appetite, and slow digestion. Symptoms may include empty distension in the abdomen and hypochondrium, vomiting of phlegm-saliva, or borborygmus and diarrhea. This is analogous to a pot without fire... [text implies inability to cook food/digest].
[Er Shen Wan]
This is Er Shen Wan plus Amomi Fructus Rotundus (Muxiang) (2 liang). Treatment method is described previously.
[Si Shen Wan]
This is Er Shen Wan plus Schisandrae Fructus (Wuweizi) (2 liang) and Evodiae Fructus (Wuzhuyu) (1 liang), processed into a fine powder. Use 8 liang of fresh ginger (Shengjiang) and 100 large red dates (Hongzao), boiled until cooked. Take the date pulp and mix with the powder to form pills the size of Sophora japonica seeds. Take 50-70 pills when empty-stomached or before meals, swallowed with plain warm water. It treats spleen and stomach deficiency, loose stools, poor appetite, dysentery, abdominal pain, and other syndromes.
[Variation of Si Shen Formula]
Use Er Shen Wan plus Amomi Fructus Rotundus (Muxiang) and Foeniculi Fructus (Huixiang) (stir-fried, each 1 liang). Another prescription uses Massae Medicatae Fermentatae (Shenqu) and Malto (Yamayi) (each 1 liang). Both are named "Si Shen" and treat spleen diarrhea and kidney diarrhea.
[Qie Wan (Alternative Prescription)]
Take Psoraleae Fructus (Pogu Zhi) (soaked in wine and stir-fried, 1 jin), Eucommiae Cortex (Duzhong) (skin removed, soaked in ginger juice and stir-fried, 1 jin), and Juglandis Semen (Hetao Rou) (skin removed, 20 pieces). Grind into powder. Mix with garlic paste (Suan Gao) formed by pounding garlic, and combine with the powder to form pills the size of Sophora japonica seeds. Take 20 pills when empty-stomached, swallowed with warm wine. Women should use light vinegar decoction. It treats kidney qi deficiency where wind-cold invades, or blood and qi stagnation causing lower back pain with difficulty in bending and stretching; or kidney injury due to overwork; or waist injury from damp-bi; or internal injury from falls; or pathogenic wind-cold attacking; or stagnant qi failing to disperse, all of which can cause lower back pain, a sensation of something dropping in the waist, or difficulty sitting and standing. Regular use strengthens sinews and bones, invigorates blood vessels, blackens beard and hair, and improves complexion.
