此眼初患之时。都无痒痛。还从一眼先患。后乃相牵俱损。皆因脑中热风冲入眼内。脑脂流下。凝结作翳。如银针

This text describes two types of eye diseases: Floating Cataract (Floating Opacity) and Sunken Cataract (Sunken Opacity).
**I. Floating Cataract (Floating Opacity)**
When this eye disease first occurs, there is no sensation of itching or pain. It typically begins in one eye and subsequently affects the other. This is caused by wind-heat pathogens from the brain rushing into the eyes, causing brain fluid to flow downward and coagulate into an opacity. The opacity resembles the color of a silver needle; although specific figures cannot be seen, the "Three Lights" (sun, moon, and stars) are still visible. It is suitable to use a gold needle for cataract surgery (needle extraction). Thereafter, it is advisable to administer Jueming San (Clear Bright Powder) or Zhuibi Wan (Opacity-Dropping Pill), which yield miraculous effects.
Poem:
Viewing floating opacity directly appears as if looking through an outer membrane; at first glance, its color is white like a silver lamp. The pupil expands and contracts with the balance of Yin and Yang; deep understanding reveals its true nature.
Once identified without error, using a gold needle to extract it brings it close to the black pupil (cornea). As long as treatment follows the prescribed methods with diligence, one can avoid disturbing the coagulated fluid and preserve clear vision.
**Jueming San (Clear Bright Powder)**
Ingredients: Shijueming (Haliotidis Concha), Renshen (Ginseng Radix et Rhizoma), Fuling (Poria), Dahuang (Rhei Radix et Rhizoma), Cheqianzi (Plantaginis Semen), Xixin (Asari Radix et Rhizoma) (each 1 liang); Fangfeng (Saposhnikoviae Radix) (2 liang); Chongweizi (Leonuri Fructus) (2 liang); Jiegeng (Platycodonis Radix) (1.5 liang).
Preparation and Administration: Grind the above ingredients into fine powder. After each meal, take 1 qian mixed with rice water.
**Zhuibi Wan (Opacity-Dropping Pill)**
Ingredients: Shijueming (Haliotidis Concha), Xixin (Asari Radix et Rhizoma) (each 1 liang); Zhimu (Anemarrhenae Rhizoma), Dihuang (Rehmanniae Radix), Fangfeng (Saposhnikoviae Radix) (each 1 liang); Tugan (Leporis Hepatis Praeparatum) (one liver, roasted); Wuweizi (Schisandrae Fructus), Renshen (Ginseng Radix et Rhizoma) (each 2.5 liang).
Preparation and Administration: Grind the above ingredients into fine powder, mix with refined honey, and form pills the size of paulownia seeds. Take 10 pills on an empty stomach with tea.
**II. Sunken Cataract (Sunken Opacity)**
When this eye disease first occurs, there is heat due to liver labor/exhaustion. It also begins in one eye initially. One may see black flowers dancing before the eyes, followed by involvement of the other eye. Heat qi from the brain flows downward, yet the patient can still distinguish the "Three Lights." It is advisable to have a practitioner perform needle extraction with a gold needle. Thereafter, taking Lingyangjiao Yinzi (Antelope Horn Decoction) or Kongqing Wan (Blue Azurite Pill) will lead to recovery.
Poem:
This category of four obstructions is also called Sunken Opacity; it hides its form, and the darkness is not yet deep. Only when looking carefully towards the sun can it be seen; it has been passed down since ancient times, not created today.
When extracting this opacity, the acupuncture point selected must be distant; observe that the physician does not use a short-headed needle. Manipulation should last more than fifty breaths (xi); seek firmness and do not let your mind wander.
**Lingyangjiao Yinzi (Antelope Horn Decoction)**
Ingredients: Lingyangjiao (Saigae Tataricae Cornu), Fangfeng (Saposhnikoviae Radix), Chongweizi (Leonuri Fructus) (each 2 liang); Cheqianzi (Plantaginis Semen), Heishen (Polygoni Multiflori Radix - specifically processed black root), Huangqin (Scutellariae Radix) (each 1 liang); Dahuang (Rhei Radix et Rhizoma) (0.5 liang).
Preparation and Administration: Grind the above ingredients into fine powder. Take 1 qian of the powder with one cup of water, decoct until reduced to half a cup, strain while warm, and take on an empty stomach.
**Kongqing Wan (Blue Azurite Pill)**
Ingredients: Kongqing (Azurite) (1 zhu); Wuweizi (Schisandrae Fructus), Cheqianzi (Plantaginis Semen), Xixin (Asari Radix et Rhizoma) (each 1 liang); Fangfeng (Saposhnikoviae Radix), Shengdihuang (Rehmanniae Radix), Zhimu (Anemarrhenae Rhizoma) (each 1 liang); Shijueming (Haliotidis Concha) (1 liang, separately ground finely).
Preparation and Administration: Grind the above ingredients into fine powder, mix with refined honey, and form pills the size of paulownia seeds. Take 10 pills on an empty stomach with tea.
