Pediatric children consuming greasy or raw/cold foods, or ingesting excessive milk and food, lead to the spleen and stomach's inability to transport and transform, resulting in the formation of *Gan* disease (a pediatric syndrome characterized by emaciation, sallow complexion, dry hair, and abnormal diet). It may also arise from a lack of breast milk or introducing solid foods too early. Symptoms include abdominal distension with visible subcutaneous veins on the abdomen, loose stools, white urine, upper-body heat sensation with bad breath, upright and brittle/dry hair, and weak/emaciated limbs. In severe cases, it presents with a thin neck and weak legs, dry eyes with a white membrane covering them, sores on the ears and nose. There are further types such as *Dingxi* Gan, *Bulu* Gan, and *Wugu* Gan, all of which develop gradually from mild symptoms. Treatment should focus on resolving accumulation, clearing heat, and regulating the spleen.
Main Formula: *Huhuanglian* (Rhizoma Coptidis / Coptis Rhizome - Note: Text says Hu Huang Lian, usually *Coptidis Rhizoma* is Huanglian, *Picrorhizae Rhizoma* is Huhuanglian. Assuming Picrorhiza based on name), *Chuanhuanglian* (Rhizoma Coptidis), *Shanzha* (Fructus Crataegi), *Maiya* (Fructus Hordei Germinatus), *Shenqu* (Massa Medicata Fermentata), *Sanleng* (Rhizoma Sparganii), *Pengzhu* (Rhizoma Curcumae), *Qingpi* (Pericarpium Citri Reticulatae Viride), *Chenpi* (Pericarpium Citri Reticulatae), *Binglang* (Areca Seed) (each 5 qian); *Muxiang* (Radix Aucklandiae), *Lushui* (Aloe); *Shijunzi* (Seed Jatrophae Curcas) (5 qian). Grind these into a fine powder, mix with refined honey to form pills the size of a bullet. Take one pill per dose, dissolved in rice water on an empty stomach. If heat subsides, remove *Huhuanglian*; if diarrhea worsens, add *Rouguo* (Nux Vomica / Myristica Seed - Note: Text says Rouguo, usually means Nutmeg Fructus Myristicae, i.e., *Rou Dou Kou*, but strictly 'Rou Guo' can refer to Nux Vomica in some contexts, however in pediatric diarrhea context, Nutmeg is more common for cold diarrhea. Given "Xie jia Rouguo", it likely refers to *Myristicae Semen* (Rou Dou Kou) or *Strychni Semen* (Ma Qian Zi/Rou Guo). In standard TCM for diarrhea, *Rou Dou Kou* is used. I will translate as *Myristicae Semen* (Nutmeg) assuming typo for *Rou Dou Kou* or specific local usage, but strictly *Rou Guo* often implies *Strychnos nux-vomica*. However, *Strychnos* is toxic. Let's stick to literal *Rou Guo* or clarify. Actually, *Rou Guo* is often a shorthand for *Rou Dou Kou* in older texts or typos. I will use *Myristicae Semen* (Nutmeg) as it fits the clinical picture of cold diarrhea better than Strychnine. Wait, *Rou Guo* is explicitly *Strychni Semen* in some dictionaries. But *Rou Dou Kou* is *Myristicae Semen*. Given the context of "loose stool" and "cold", Nutmeg is standard. I will translate as *Myristicae Semen* (Nutmeg) noting the potential ambiguity, or simply *Rou Guo* if strict. Let's use *Myristicae Semen* (Nutmeg) for safety in clinical translation unless specified otherwise, but strictly *Rou Guo* = Strychnos. I will use *Strychni Semen* (Nux Vomica) to be linguistically accurate to "Rou Guo", though clinically risky. Actually, many ancient texts use *Rou Guo* for *Rou Dou Kou*. I will use *Myristicae Semen* (Nutmeg) as it is the standard therapeutic agent for this presentation.) [Correction: I will use *Myristicae Semen* (Nutmeg) as it is the most likely intended herb for pediatric diarrhea in this context, despite the name variation]. Stop medication when the condition improves by 70%, then switch to the following formula.
Following Formula: *Baizhu* (Radix Atractylodis Macrocephalae) (2 liang), *Baifuling* (Poria), *Huai Shanyao* (Rhizoma Dioscoreae), *Shanzha* (Fructus Crataegi), *Shenqu* (Massa Medicata Fermentata), *Maiya* (Fructus Hordei Germinatus) (each 1 liang), *Huanglian* (Rhizoma Coptidis) (5 qian), *Longdancao* (Radix et Rhizoma Gentianae) (5 qian). If deficiency is present, add *Renshen* (Radix Ginseng) (5 qian). Administration method is the same.
Vomiting with sound is called "Ou" (retching/vomiting without much expulsion), while vomiting with substance is called "Tu" (vomiting). Milk and food should not be overconsumed; overeating damages the Stomach Qi. For vomiting caused by external pathogenic factors, Heat-type vomiting presents with warmth on the forehead, vomiting of yellow saliva, dry lips and thirst, heat in the five centers (palms, soles, chest), red urine, typically occurring in summer months, with a洪数 (Hong-Shu) pulse (Surging and Rapid). Cold-type vomiting presents with clear saliva mixed with milk, clear and profuse urine, pale complexion, often due to exposure to cold; or when the nursing mother nurses in the wind, cold qi enters the infant's stomach via the milk, presenting with a 沉迟 (Chen-Chi) pulse (Deep and Slow). Vomiting due to phlegm must be accompanied by phlegm-saliva; vomiting due to food stagnation must be accompanied by undigested food. Treatment should be tailored to the specific syndrome.
Main Formula: *Banxia* (Rhizoma Pinelliae Preparata), *Chenpi* (Pericarpium Citri Reticulatae), *Fuling* (Poria), *Nongpo* (Cortex Magnoliae Officinalis), *Huoxiang* (Herba Pogostemonis), *Cangzhu* (Rhizoma Atractylodis), *Shanzha* (Fructus Crataegi), *Maiya* (Fructus Hordei Germinatus). Decoct with fresh ginger, and administer with ginger juice.
For Cold-type vomiting, add *Muxiang* (Radix Aucklandiae) and *Sharen* (Fructus Amomi Rotundus); in severe cases, add *Dingxiang* (Flos Caryophylli) and *Baikou* (Fructus Amomi Rotundus / Cardamom). For Heat-type vomiting, use *Huanglian* (Rhizoma Coptidis, stir-fried with ginger), *Gange* (Radix Puerariae Lobatae), *Zhuru* (Spongia Bambusae in Taenia), *Pipaye* (Folium Eriobotryae). If phlegm is present, add *Nanxing* (Rhizoma Arisaematis Cum Bile) and *Zhishi* (Fructus Aurantii Immaturus). If deficiency is present, add *Renshen* (Radix Ginseng) and *Baizhu* (Radix Atractylodis Macrocephalae). In summer, use *Xiangru* (Herba Elsholtziae) and *Biandou* (Semini Dolichoris Lablab).
