The *Yellow Emperor's Inner Classic* states: "Apply sweating therapy, and the sores will heal." This refers to the method of dispersing pathogens from the Ying (Nutritive) and Wei (Defensive) levels, which is a fundamental principle. Therefore, in the initial stage of infection, if the pulse is floating and rapid, with swelling, redness, and pain, Jiajian Baidu San (Modified Decoction to Overcome Toxins) should be used to disperse the pathogen. If the pulse is deep and solid, accompanied by irritability, heat, restlessness, thirst, hard swelling with deep-seated pain, and constipation, indicating the pathogen is in the interior, purgative therapy should be applied; Huanglian Tang (Coptis Decoction) or similar formulas can be used to unblock, or Xianfang Huoming Yin (Immortal Formula for Preserving Life Decoction). If there is no external swelling and no internal constipation, the pathogen is in the meridians; Ying and Wei should be harmonized using Tuoli Yangrong Tang (Support Interior and Nourish Nutritive Qi Decoction) or Baizhi Shengma Tang (Angelica Dahurica and Cimicifuga Decoction). If the swelling is high and painful with a floating pulse, interior-supporting herbs should be used, such as Neituo Fujian San (Interior-Supporting Double-Boiled Powder). If there is severe pain or no pain, and the swelling does not suppurate, this indicates congealment of pathogenic factors; dispersing therapy is indicated, with oral administration of Xianfang Huoming Yin. If there is irritability, desire for cold drinks, painful swelling, and a rapid pulse, the pathogen is in the upper jiao; clearing and draining therapy is indicated, using Qingliang Yin (Cooling Drink) or Jinyinhua San (Honeysuckle Powder). If a malignant sore persists without ulceration for a long time, it indicates excess qi combined with cold pathogens; dissemination and tonification are indicated, using Shixuan San (Ten-Ventilation Powder). If fever, profuse sweating, extreme thirst, constipation, and delirium occur, this is a Jieyang Syndrome (Yang Knotting Syndrome); purgative therapy is indicated, such as Huanglian Neishu Tang (Coptis Interior-Purging Decoction) or Po Guan Dan (Sarcophagus-Breaking Pill). If suppuration does not occur and ulceration fails to happen, it is due to deficiency; strong tonification is required, using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) or Shiquan Dabu Tang (Perfect Ten Great Tonifying Decoction). If there is painful swelling, desire for cold drinks, fever, and somniloquy (sleep-talking), this is a Fire syndrome; clearing and descending fire is indicated, using Qingxin Tang (Clear Heart Decoction) [which is Fangfeng Tongsheng San plus Huanglian]. If there is no suppuration, failure to ulcerate, or failure of the wound to close, it is due to deficiency of Yang Qi; supporting interior and tonifying is indicated, using Tuoli Xiaodu San (Support Interior and Eliminate Toxins Powder). If there is slight swelling and slight pain without suppuration, or diffuse swelling without pain, or formed pus that does not ulcerate, strong tonification is appropriate for all; if the color is dark, Yang-augmenting herbs should be added, and tonification is also indicated. If a Yin syndrome occurs and cold medicines have been mistakenly administered, urgent use of hot medicines to restore Yang is required, such as Jiangfu Tang (Ginger and Aconite Decoction) or Sanjian Tang (Three Jian Decoction). First, Ruxiang (Frankincense) and Ludou Fen (Mung Bean Powder) should be taken to save the heart and protect the membranes. Vomiting at the onset of disease indicates toxic pathogens attacking the Pericardium collateral; the mind spirit must be rescued to achieve tranquility. Although severe illness may not be fatal, mild illness may sometimes prove fatal because the membranes are penetrated and Yuan Qi (Original Qi) leaks out; therefore, saving the heart and protecting the membranes is the primary principle. If pressing the area reveals hardness without heat or softness, indicating no pus has formed yet, urgent resolution is required; if pressing reveals softness, confirming pus formation but lack of ulceration, urgent incision is needed, otherwise internal ulceration will persist for a long time, making healing difficult. If pus has not yet formed, needle puncture must strictly be avoided.
