Breathing and movement are calm, and food and drink have flavor; this is the first "Good" (Shan). Bowel movements and urination are regulated; this is the second "Good". Pus discharges, swelling subsides, and the fluid is fresh without foul odor; this is the third "Good". Spirit is bright and clear, voice is crisp and resonant; this is the fourth "Good". Body qi is harmonious and peaceful; these are collectively known as the Five Goods.
Intense thirst with fever, diarrhea or urinary retention; this is the first "Bad" (E) (Treatment: Huang Zhu Ye Tang [Yellow Bamboo Leaf Decoction]. If due to deficiency of both Qi and Blood, use Bazhen Tang [Eight Treasure Decoction] added with Huangqi [Astragali Radix], Maidong [Ophiopogonis Radix], Wuweizi [Schisandrae Fructus], and Shanyao [Dioscoreae Rhizoma]. Alternatively, use Bawei Wan [Eight-Ingredient Pill], crushed and decocted for administration.) After pus and blood have discharged, yet pain and swelling are even more severe, and the pus color is败坏 (decayed) and foul-smelling; this is the second "Bad" (Treatment: Renshen Huangqi Tang [Ginseng and Astragalus Decoction], or Shiquan Dabu Tang [Ten Complete Tonifying Decoction] added with Maidong and Wuweizi.) Eyes gaze abnormally, the black pupil is tight and small, the white sclera is greenish-red, and the pupil looks upward; this is the third "Bad" (Treatment: Liuwei Wan [Six-Ingredient Pill] added with Shanzhi [Gardeniae Fructus], Maidong, and Wuweizi. If ineffective, use Bazhen Tang added with Shanzhi, Maidong, and Wuweizi.) Rapid and coarse breathing with shortness of breath, mental confusion and somnolence; this is the fourth "Bad" (Treatment: Liujunzi Tang [Six Gentlemen Decoction] added with Shengjiang [Zingiberis Rhizoma Recens] and Dazao [Jujubae Fructus]. If ineffective, use Buzhong Yiqi Tang [Tonify the Middle and Augment the Qi Decoction] added with Shanyao, Shanyuzhu [Corni Fructus], Wuweizi, and Chifuling [Poria cum Ligno]. Or use Shiquan Dabu Tang added with Shanyao, Shanyuzhu, and Wuweizi.) Inconvenience in shoulders and back, heaviness in the four limbs; this is the fifth "Bad" (Treatment: Buzhong Yiqi Tang added with Shanyuzhu, Shanyao, and Wuweizi. If ineffective, use Shiquan Dabu Tang added with Shanyuzhu, Shanyao, and Wuweizi.) Inability to take medicine, inability to swallow food, tongue loses sense of taste; this is the sixth "Bad" (Treatment: Liujunzi Tang added with Muxiang [Aucklandiae Radix] and Sharen [Amomi Fructus Rotundus]. If ineffective, add Fuzi [Aconiti Lateralis Radix Praeparata]). Hoarse voice and deteriorated complexion, lips and nose appear greenish-red, edema of face, eyes, and four limbs; this is the seventh "Bad" (Treatment: Buzhong Yiqi Tang added with Shengjiang and Dazao. If ineffective, use Liujunzi Tang added with Paojiang [Zingiberis Rhizoma Preparatum]. For severe cases, add Fuzi, or use Shiquan Dabu Tang added with Fuzi and Paojiang.)
If abdominal pain and diarrhea occur, along with coughing, counterflow of Qi, coma, and ulceration, use Jiuli Wenzhong Tang [Decoction to Rescue the Interior and Warm the Middle]. Subsequently, use Liujunzi Tang added with Fuzi, Shengjiang, and Guizhi [Cinnamomi Ramulus]. If after ulceration there are alternating chills and fever, thirst, palpitations, fright, restlessness in sleep and wakefulness, locked jaw, spontaneous sweating, and a flooding pulse that feels empty upon pressure; or if there is body heat with aversion to clothing, desire to plunge into water, or feeling cold despite thick clothing, cold extremities, opisthotonos, and stiff neck, urgently rescue with Renshen [Ginseng Radix et Rhizoma], Fuling [Poria], Danggui [Angelicae Sinensis Radix], Baizhu [Atractylodis Macrocephalae Rhizoma], and Fuzi. If three of the Five Goods are present, it indicates auspiciousness; if four of the Seven Bads are present, it indicates critical danger. Excess patterns presenting with bad prognostic signs are treatable; deficiency patterns presenting with bad prognostic signs are untreatable.
