Belching is what the *Huangdi Neijing* (Yellow Emperor's Inner Canon) refers to as "yi" (burping). The classics state that spleen disease predisposes one to frequent belching. It also states that when cold qi invades the stomach, causing rebellious counterflow, it rises from below and disperses upward, exiting through the stomach, thus resulting in belching.
Later physicians attributed this to stagnation of spleen and stomach qi, originating in the middle jiao and emerging from the upper jiao. It is common after illness and in elderly individuals with weak spleen and stomach. However, there are cases where liver qi rebels and overacts upon the spleen/stomach, presenting with sour belching, a sensation of fullness, epigastric hardness and stuffiness, and persistent belching. Zhang Zhongjing stated that when stomach qi is deficient and pathogenic qi ascends, heavy and sinking agents must be used to suppress the rebellion; Xuanfudaihzhe Tang (Inula and Hematite Decoction) is indicated. There are also cases where lung qi fails to descend, causing belching; Suzi Jiangqi Tang (Perilla Seed Descending Qi Decoction) with Gui (Cinnamomi Ramulus) removed, plus Xingren (Pruni Armeniaci Semen) and Beimu (Fritillariae Bulbus), may be used.
For belching due to stomach deficiency and qi stagnation, Shiwei Baohe Tang (Ten-Ingredient Preserve Harmony Decoction) is used. For belching due to stomach cold and qi stagnation, Hewei Jian (Harmonize Stomach Decoction) is used. For belching accompanied by vomiting phlegm due to stomach deficiency, Hewei Erchen Jian (Harmonize Stomach Two-Aged Decoction) is used. For belching with false fullness due to stomach cold and indigestion, Yangzhong Jian (Nourish Middle Decoction) or Lizhong Wan (Regulate the Middle Pill) is used. If spleen and kidney yang deficiency with cold, leading to decline of Mingmen fire (Gate of Life fire) and failure of turbid yin to descend, resulting in stuffiness and belching, Liyin Jian (Nourish Yin Decoction) with modifications is used. For belching due to phlegm-fire in the stomach, Xingxia Zhizi Tang (Arisaema, Pinellia, and Gardenia Decoction) is used. When belching is specifically caused by spleen and stomach yang deficiency, with middle qi obstructed by yin pathogens, the treatment focuses on harmonizing the middle, using Jianpi San (Spleen-Strengthening Powder). If wood overacts on earth, the rebellious qi ascending from the liver cannot be restrained without heavy sedative and suppressing agents.
