Xu: Jaundice caused by essence depletion due to sexual excess, accompanied by dizziness, facial edema, weakness of the lower back and knees, burning sensation in the soles of the feet, and a pulse that is large and forceful but empty upon pressure. Prescribed Xue's Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) with Poria (Fuling, 茯苓) as the sovereign herb, removing Alisma (Zexie, 泽泻), adding Raw Rehmannia Root (Shengdihuang, 生地黄), Achyranthes Root (Niuxi, 牛膝 - steamed with wine), Lotus Seed (Lianzi, 莲子), and Coix Seed (Yiren, 薏苡仁). Decoction and pills were taken alternately; after meals, a tea brewed from Chrysanthemum (Ganju, 甘菊 - stir-fried), Gardenia (Shanzhi, 山栀 - blackened), Mulberry Leaf (Sangye, 桑叶 - tender), and Uncaria Hook (Gouteng, 钩藤) was consumed. Recovered after several months.
Gong: Qi depletion due to overwork leading to jaundice, reduced appetite, shortness of breath, yellow eyes, and dull complexion. Following Zhang Zhongjing's method, Huangqi Jianzhong Tang (Astragalus Decoction for Building the Middle) was used with Cinnamon Twig (Guizhi, 桂枝) and Ginseng (Renshen, 人参) removed, combined with Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder). Effective. Subsequently managed with Lotus Seed, Coix Seed, and Red Dates to regulate and tonify Spleen Yuan. This case specifically tonifies Spleen Yuan and does not follow the standard treatment for other types of jaundice; if Yinchen (Artemisia Capillaris) and Shanzhi (Gardenia) were uniformly used to drain damp-heat, it might have transformed into abdominal distension and fullness.
Shi: Yang jaundice transforms from heat; stasis-heat accumulates internally, steaming the gallbladder fluid, causing leakage and yellowing, bright like an orange peel. Currently, yellow eyes and bright complexion, yet dizzy, chest oppression without thirst, limb fatigue, heat in palms and soles, constipation, worsening with exertion. Pulse: right side large, left side deficient, soft, and soggy. Although dampness generates heat, a normal person with a large pulse indicates labor injury, and prolonged unresolved jaundice signals qi injury from physical labor. Used a method to tonify middle jiao and leach out dampness: Luduan Shen (Radix Codonopsis Pilosae), Poria, Coix Seed, Atractylodes Macrocephala Rhizome (Baizhu, 白术) (each 1.5 qian), Chicken Gizzard Lining (Jineijin, 鸡内金), Yinchen, Needle Sand (Zhensha, 针砂) (each 2 qian), Gardenia, Chrysanthemum, Moutan Cortex (Danpi, 丹皮) (each 1 qian), Honey-fried Licorice (Zhigancao, 炙甘草) (0.5 qian). After several doses, dizziness and chest oppression resolved, appetite increased. Removed Chrysanthemum and Gardenia, added Astragalus (Huangqi, 黄芪) and White Peony Root (Baishao, 白芍) (both stir-fried, 2 qian each), Lotus Seed (stir-fried, 10 grains). After more doses, jaundice gradually receded.
Certain Patient: In late summer, external steaming of summer-heat and dampness, internal accumulation of food and fluids, Spleen Yang failing to transport. Symptoms: dizziness, nausea, face yellow like smoked tobacco, abdominal distension after eating, afternoon irritability, dark urine. Pulse: soggy, left slightly larger. Initially treated with Qingli method (clearing and separating): Antelope Horn (Lingyangjiao, 羚羊角), Gardenia, Yinchen, Red Poria (Chifu, 赤茯), Coix Seed, Processed Pinellia (Zhibanxia, 制半夏), Amomum Villosum Shell (Sharenqiao, 砂仁壳), Talcum (Huashi, 滑石), Dendrobium (Shihu, 石斛), Plantago Seed (Cheqianzi, 车前子), Juncus Effusus (Dengxin, 灯心). After three doses, symptoms decreased. Changed to Magnolia Bark (N Houpo, 厚朴 - ginger processed), Bitter Orange (Zhike, 枳壳 - stir-fried), Tangerine Peel (Chenpi, 陈皮), Areca Peel (Dafupi, 大腹皮), Mint (Bohe, 薄荷), Yinchen. After two doses, distension resolved, but jaundice remained; patient sought quick results. Another doctor used Agarwood (Chenxiang, 沉香) and Stir-fried Atractylodes (Jiaozhu, 焦术), dry substances, suddenly causing malar jaundice. Then used Rhinoceros Horn (Xijiao, 犀角) and Coptis (Huanglian, 黄连), resulting in tidal fever before noon and afternoon. Then used Raw Rehmannia (Shengdi, 生地) and Anemarrhena (Zhimu, 知母), jaundice worsened, face became dull and black, alternating chills and fever with sweating on forehead, abdominal fullness, vomiting, diarrhea, greasy white tongue coating, sticky phlegm in diaphragm. Re-evaluated: determined this was excessive dampness. Dampness stagnation generates heat; treatment should leach dampness. Used Siling San (Four-Poria Powder) plus Pinellia Ferment (Banxiaqu, 半夏曲), Tangerine Pith (Jubai, 橘白), Coix Seed, Ginger (Wjiang, 煨姜). Before noon, diarrhea decreased, but vomiting of saliva persisted. Summer-heat and dampness steaming together, turbid saliva failed to descend, pulse soggy and rapid, indicating heat transforming from dampness. Changed to Weiling Tang (Poria and Magnolia Bark Decoction) removing Atractylodes, adding Processed Pinellia, Raw Coix Seed, Ginger. Brewed with Cangzhu (Cangshu, 苍术 - raw) pot crust soup. Vomiting stopped, diarrhea reduced. Only irritability and fever fluctuated with the sun's movement, severe at midday and early afternoon, indicating dampness gone but heat remains. Therefore used Scutellaria (Huangqin, 黄芩), Moutan Cortex, Gardenia, Red Poria, Groundcherry Root Bark (Digupi, 地骨皮), Trichosanthes Root Juice (Gualougenzhi, 栝楼根汁), plus Liuyi San (Six-to-One Powder, 1 qian) taken with water. Heat-draining symptoms stopped completely. Only mental fatigue, desire to sleep, dry mouth upon waking, fluids failing to ascend to Lung. Added Ginseng and Ophiopogon (Maidong, 麦冬) to Jiawei Xiaoyao San (Modified Free and Easy Wanderer Plus Ingredients Powder) to support Yuan and generate fluids, dispersing stagnant heat. Pulse normalized, only Left Guan pulse slightly large. Imitating "Shishi Milu", used Atractylodes Macrocephala (5 qian), Poria (3 qian), Coix Seed (1 liang), Gentian (Longdancao, 龙胆草), Gardenia, Yinchen (each 1 qian), Luduan Shen, Astragalus (each 2 qian) to dry Spleen dampness, cultivate True Yuan, assist in draining fire. Later, seeking quick results again, mistakenly took previous doctor's yin-nourishing agents, becoming incurable.
Xue: Spleen deficiency injuring dampness, disease manifested as Yin jaundice. For years, facial edema, foot swelling, dizziness, pale lips, hematochezia. Improved slightly with regulating and tonifying medicines. Recently, bleeding stopped, but stool is loose and unsatisfactory, urination short and frequent, abdomen large and hard, body heat, fatigue. Pulse: fine, small, soggy, rapid. Given Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) for lifting, assisted by mild diuretics. Leg and foot swelling subsided, pulse became spirited. Continued with Luduan Shen, Raw Atractylodes, Red Poria, Moutan Cortex, Blackened Gardenia, Yinchen, Oyster Shell (Muli, 牡蛎), Cimicifuga (Shengma, 升麻). Bowels cleared, heat lighter, occasional epigastric pain. Removed Moutan, Gardenia, Cimicifuga, Atractylodes, added Costus Root (Muxiang, 木香), Tangerine Peel, White Peony Root. Pain ceased. Changed to Shenqi Tang (Kidney Qi Decoction) removing Cornus (Shanzhuyu, 山萸), Alisma, Aconite (Fuzi, 附子), adding Roasted Ginger. Abdomen gradually softened. Later, due to dietary indiscretion, died of abdominal distension.
Geng: Long-standing jaundice, mental fatigue, dizziness, facial edema, intermittent fever and thirst. Pulse: deficient and large, Yang floating and unconsolidated. Should gather Yin and harmonize Yang. Prescribed Oyster Shell, White Peony Root, Schisandra (Wuweizi, 五味), American Ginseng (Yangshen, 洋参), Prepared Rehmannia (Shudi, 熟地), Ophiopogon, Dendrobium, Rubus Fruit Skin (Ludoupi, 橹豆皮). Several doses effective.
Qian: Jaundice complicated by food stagnation, abdominal distension, yellow urine. Used spleen-strengthening and separating/dispersing herbs, added Yinchen Tang. Abdominal distension unchanged. Diagnosed as Stomach Fu organ stasis-heat and depression steaming. Used Yuyuliang (Yuyuliyang, 禹余粮 - vinegar-quenched 7 times), Raw Rehmannia, Songluo Tea (Songluocha, 松萝茶) (each 4 liang), Green Vitriol (Lvfan, 绿矾 - calcined, 1 liang), Jujube Flesh (Zairou, 枣肉 - baked). Pulverized and kneaded into pills. Cured after taking.
Tang: Yellow face in childhood, good appetite, dizziness, intermittent fever. Caused by dampness generating heat, heat steaming transforming into yellow, stomach heat digesting grain. This is Grain Jaundice (Gu Dan, 谷疸). Suitable for Pig Stomach Pill (Zhudu Wan, 猪肚丸). In autumn, postprandial distension, dizziness, loose stool. Pulse: deficient and small. Heat and dampness battling, Taiyin failing to transport, Shaoyang transforming into wind. Mainly regulate Spleen Yang, assist with extinguishing wind. Raw Atractylodes, Luduan Shen, Tangerine Peel, Coix Seed, Chicken Gizzard Lining, Pinellia Ferment (all stir-fried), Yinchen, Red Poria, Chrysanthemum (stir-fried), Gastrodia (Tianma, 天麻 - baked). Cured after taking.
