Spleen transferring cold to the Liver. (Cold pathogen invades the organ that controls it.) Carbuncles and tendon spasms occur. (The Liver governs blood; cold causes stagnation leading to carbuncles. The Liver governs tendons; cold causes contraction.) Liver transferring cold to the Heart. (Transmission to the organ it generates.) Mania and chest obstruction occur. (Spirit is invaded by cold, water and fire agitate each other, hence mania. Cold knots in the middle, hence obstruction.) Heart transferring cold to the Lung. (Overcoming the organ it controls.) Lung Xiao occurs. Lung Xiao refers to drinking one unit of fluid and urinating two units. (This is Upper Xiao. Heart fire scorches Metal, the Lung cannot govern Qi, there is descent without ascent, hence drinking one and urinating two. If not yet severe, it is still treatable. Note: Carbuncles, mania, chest obstruction, and Lung Xiao are mostly heat syndromes, but the text says "transferring cold". If interpreted as heat, the following section on "transferring heat" would be redundant. Commentaries vary between heat and cold. I suggest "cold" refers to the initial stage of disease. For example, chest obstruction is mostly heat knotting, but cold obstruction exists; carbuncles are mostly heat, but cold ulcers exist. Similarly, mania, epilepsy, and Lung Xiao should be interpreted as cold transforming into heat after a long duration, which makes sense. In contrast, Kidney transferring cold causing Yong Water involves pure yin-cold throughout.) Lung transferring cold to the Kidney. (Transmission to the organ it generates.) Yong Water (Surging Water) occurs. Yong Water is characterized by a soft abdomen upon pressure, water qi lodging in the Large Intestine, making gurgling sounds when walking quickly, like a bag containing liquid. This is a water pathology. (Lung generates water, Large Intestine is its Fu organ. Kidney is the extreme Yin organ and the water reservoir. Kidney is the root, Lung is the branch, hence water accumulation.) Spleen transferring heat to the Liver. (Heat invades the organ that controls it.) Fright and epistaxis occur. (Liver stores blood and governs fright. Liver channel meets Du vessel at the vertex. Blood follows fire upward to the brain and exits the nose, causing epistaxis.) Liver transferring heat to the Heart. (Transmission to the organ it generates.) Death ensues. (Heart is the Sovereign, difficult to invade. Moreover, Liver Qi is dry and fierce, Wood and Fire burn together, hence death.) Heart transferring heat to the Lung. (Overcoming the organ it controls.) Ge Xiao (Diaphragm Xiao) develops. (Same meaning as "transferring cold" above, but Ge Xiao is Middle Xiao, less severe than drinking one and urinating two. Some say Ge syndrome and Xiao Zhong are two different diseases.) Lung transferring heat to the Kidney. (Transmission to the organ it generates.) Rou Jing (Soft Spasm) develops. (Qi and Bone are both hot, Marrow fails to generate, hence stiff bones causing spasm, and flaccid tendons causing relaxation.) Kidney transferring heat to the Spleen. (Heat invades the organ that controls it.) Deficiency and Chang Pi (Dysentery) develop, leading to death. (Water counter-controls Earth, Spleen and Kidney are both deficient, uncontrollable dysentery leads to death.) Bao Gong (Uterus) transferring heat to the Bladder. (Six Fu organs transfer among themselves.) Long (Difficulty in urination) and hematuria occur. (Bladder is the chamber of the Uterus. Heat causes urinary retention. Zheng Li Lun states: Heat in the Lower Jiao causes hematuria.) Bladder transferring heat to the Small Intestine. Obstruction of the diaphragm and intestines, leading to Kou Mi (Mouth Ulcers) upwards. (Bladder connects to Small Intestine above; Small Intestine channel passes through throat and diaphragm. Heat knots in diaphragm and intestines, preventing downward flow, rising upward to cause mouth sores.) Small Intestine transferring heat to the Large Intestine. Forms Fu Xia (Hidden Masses) and Chen (Deep-seated masses). (Jin-Ye and Blood knot to form masses; Chen implies depth. Some say "Chen" is a typo for "Shan/Hernia".) Large Intestine transferring heat to the Stomach. Polyphagia with emaciation, also called Shi Yi (Eating Without Gaining Weight). (Eating yet remaining thin, similar to Middle Xiao.) Stomach transferring heat to the Gallbladder, also called Shi Yi. Gallbladder transferring heat to the Brain. Causes Xin (Pungency at the nasal root), Bi Yuan (Nasal Sinusitis). Bi Yuan is continuous discharge of turbid mucus. (Jie Jing Wei Lun states: Brain seeps into mucus.) Further develops into Nu (Epistaxis), Mie (Hemohidrosis/Sweating Blood), and Ming Mu (Blurred Vision). (All caused by Qi Jue/Qi Reversal.) (From Qi Jue Lun)
[Ling Shu] Zhen Tou Tong (True Headache). Severe headache, pain throughout the brain, cold hands and feet extending to joints, untreatable. Half-head cold pain. First needle Hand Shaoyang and Hand Yangming, then Foot Shaoyang and Foot Yangming. (Acupuncture method. Hemiplegic headache belongs to Shaoyang, as channels run along the side of the head.) Jue Xin Tong (Jue Heartache). Pain pulling to the back, frequent sighing. Feeling as if touched from behind, stooping posture indicates Shen Xin Tong (Kidney Heartache). Abdominal distension, chest fullness, severe heart pain indicates Wei Xin Tong (Stomach Heartache). Pain like needle/prick, severe heart pain indicates Pi Xin Tong (Spleen Heartache). Pale/greenish face like a corpse, unable to take deep breaths all day indicates Gan Xin Tong (Liver Heartache). Pain relieves when lying/still, worsens with movement, unchanged complexion indicates Fei Xin Tong (Lung Heartache). Zhen Xin Tong (True Heartache), cold hands/feet (or cyanosis) to joints, severe heart pain, morning onset dies evening, evening onset dies morning. (Heart is Sovereign, hard to invade.) (From Jue Bing)
[Su Wen] Neck pulse throbs. (Renying pulse near Adam's apple.) Wheezing and coughing indicate Water Disease. (Water overflows Lung, neck pulse bulges, wheezing/coughing.) Eyelids slightly swollen, like silkworms, indicate Water Disease. (Ping Re Bing Lun: Water is Yin, under eyes is Yin. Abdomen is residence of Extreme Yin, so abdominal water causes under-eye swelling.) Yellow urine, lethargy indicates Huang Dan (Jaundice). Hunger after eating indicates Wei Dan (Stomach Jaundice/Grain Jaundice). Facial swelling indicates Wind. (Face is meeting of Yang channels, Wind is Yang, attacks upper body first, swelling not just water.) Leg/shin swelling indicates Water Disease. Yellow eyes indicate Jaundice. (Damp-heat steaming upward.) (From Ping Ren Qi Xiang Lun)
[Su Wen] Body feels cold not due to lack of clothes or internal cold, why? Is it Yin deficiency and Yang excess? No. Body feels cold internally without external cold, why? Due to Bi Qi (Obstructed Qi). Yang deficiency and Yin excess cause body cold like emerging from water. Limbs hot, feeling like burning when encountering wind/cold, why? Yin deficiency, Yang excess. Limbs are Yang, two Yangs combine, Yin is weak, little water cannot extinguish strong fire, Yang dominates alone. Alone Yang cannot grow, only dominates. Encountering wind/cold feels like burning, muscles will waste away. (Wind-fire scorch muscles.) Body cold, hot drinks/fire/clothes don't warm, but no shivering, why? Original Kidney Qi excess, excessive sexual activity. Taiyang (Bladder) Qi declines, Kidney essence dries up. One water cannot overcome two fires. Kidney is Water, generates bone. If Kidney fails, Marrow doesn't fill, cold reaches bone. No shivering because Liver is one Yang, Heart is two Yangs. Kidney is solitary organ, one water cannot overcome two fires, hence no shivering. Disease name: Gu Bi (Bone Bi). (Shivering is external cold, this is Bone Bi.) Tendons will contract. (Marrow dry, tendons shrink.) Muscle numbness/heaviness, even with cotton, remains numb. Ying Qi deficient, Wei Qi excess. (Excess means hyperactivity). Ying deficiency causes numbness. Wei deficiency causes immobility. Both deficient causes numbness and immobility. Body disconnected from Will is death. Qi reversal, inability to lie down, noisy breathing, is Yangming reversal. Foot Three Yang descend. Now reverses upward, hence noisy breathing. Yangming is Stomach channel, Sea of Six Fu, Qi descends. Reversal prevents lying down. Xia Jing says: Stomach disharmony leads to restless sleep. Resting normally but noisy breathing is Lung collateral reversal. (Lung governs Qi/breathing). Collaterals don't follow main channels, stay in meridians. Collateral disease is mild, hence normal rest but noisy breathing. Inability to lie down, panting when lying, is Water Qi invasion. Water flows with Jin-Ye. Kidney is Water organ, governs Jin-Ye, sleep, and panting. (Lung governs Qi, Kidney grasps Qi. Kidney channel enters Lung, governs panting. Night lying moves Qi to Yin, starts from Shaoyin, hence governs sleep.) (From Ni Diao Lun)
[Ling Shu] Eyes open, unable to sleep, what Qi causes this? Five grains enter Stomach, dregs, Jin-Ye, Zong Qi divide into three paths. (Dregs to Intestines is one path.) Zong Qi accumulates in Chest (Dan Zhong), exits throat, penetrates Heart channel, governs breathing. Ying Qi secretes Jin-Ye, injects into vessels, transforms to Blood, nourishes four extremities, enters Zang-Fu, corresponds to time. (Zong and Ying Qi move in vessels, one path, 100 ke). Wei Qi, fierce/fast Qi, moves first in extremities, muscle/skin,不休 (unceasing). (Wei moves outside vessels, one path). Day moves Yang, Night moves Yin, always starts from Foot Shaoyin division. Moves among Zang-Fu. Now Jue Qi (Evil Reversal) lodges in Zang-Fu, Wei Qi guards outside, moves Yang, cannot enter Yin. Yang moves, Yang Qi excess, Yang Qiao vessel sinks. Cannot enter Yin, Yin deficiency, eyes open. (Da Huo Lun: Yang full, Yang Qiao full, eyes closed due to Yang deficiency. Or: Wei stays Yin, Yin excess, Yin Qiao full, Yang deficiency, eyes close). Treatment: Ban Xia Tang (Pinellia Decoction). Yin-Yang connected, sleep immediate. (Use 1000-li flowing water, stirred 10,000 times, 5 sheng. Sorghum 1 sheng, Ban Xia 5 he. Boil to 1.5 sheng. Drink small cup, increase gradually. Sleep immediately after drinking, sweating cures it. Note: Ban Xia harmonizes Stomach, connects Yin-Yang. People fear it's drying, mistaken.) (From Xie Ke)
[Su Wen] Warm disease, sweat then fever returns, pulse rapid/restless, not reduced by sweat, mania, anorexia, named Yin Yang Jiao. (Wang Bing: Yin-Yang Qi not distinct. Zhang Jingyue: Sweat is Yin fluid, outward Yang. Yang heat doesn't resolve with sweat, re-enters Yin, named Yin Yang Jiao. Also Wu Yun Da Lun: Chi Cun reverse die, Yin Yang Jiao die, referring to pulse.) Jiao means death. Sweat comes from Grain, Grain produces Essence. Evil fights Essence in flesh/bone. Sweat means Evil retreats, Essence wins. Essence wins means appetite returns, no fever. Fever return means Evil wins. Sweat is Essence. Anorexia means Essence has no support. Disease lingers, life ends quickly. Re Bing Lun: Sweat but pulse still rapid/dead. Pulse quiet after sweat, live. No sweat, pulse rapid/dead. Pulse rapid gets sweat, quiet/live. Now pulse doesn't match sweat, can't defeat disease. (Evil excess, Zheng deficiency). Mania is loss of Zhi (Will/Kidney). Loss of Zhi is death. Three signs of death (sweat-fever, rapid pulse, mania/anorexia), no sign of life, will die even if temporarily better. Body heat, sweat, irritability not resolved by sweat, what disease? Sweat-heat is Wind. Sweat-irritability is Jue. Named Feng Jue. Ju Yang (Taiyang) governs Qi, attacked first. Shaoyin exterior-interior, receives heat, follows upward, becomes Jue. (Taiyang exterior attacked first. Yang evil enters Shaoyin interior, Shaoyin Qi reverses upward). Lao Feng (Labor Wind) located below Lung. (Kidney labor due to wind. Kidney channel enters Lung, wind evil below Lung). Neck stiffness, blurred vision. Saliva like phlegm. (Kidney saliva, Lung mucus. Kidney heat steams Lung). Aversion to wind, chills. (Yang internal attack, fails to defend exterior, internal heat, external chills). Cough green/yellow pus-like sputum, size of bullet. (Heat knot). Exits mouth/nose, if not, injures Lung, death. Shen Feng (Kidney Wind), facial swelling. Speech obstructed. (Kidney channel throat/tongue). Can needle? Say: Deficiency shouldn't needle. Needle wrongly, 5 days later Qi arrives, shortness of breath, intermittent fever, chest/back to head, sweat, hot hands, dry bitter mouth, yellow urine, under-eye swelling, bowel sounds, heavy body, amenorrhea, irritability, anorexia, inability to lie flat, cough when lying flat. Named Feng Shui (Wind Edema). Evil gathers where Qi is deficient. Yin deficiency, Yang invades. (Shaoyin Qi deficient, Taiyang heat invades). Hence shortness of breath, fever, sweat. Yellow urine, Lower Jiao heat. (Heat enters Bladder Fu). Can't lie flat, Stomach disharmony. (Stomach disharmony, restless sleep). Lying flat cough worsens, presses Lung. (Kidney water Qi presses Lung. Shui Re Xue Lun: Root Kidney, End Lung. Ci Cong Rong Lun: Cough/annoyance is Kidney Qi reversal). All water diseases, edema starts under eyes. Water is Yin, under eyes Yin. Abdomen Extreme Yin. Abdominal water causes under-eye swelling. True Qi reverses upward, bitter mouth, dry tongue, can't lie flat, cough clear water. All water patients can't lie down, lie down startles, startle coughs worse. Bowel sounds, root in Stomach. Presses Spleen, irritability, anorexia. Food stuck, Stomach obstruction. Heavy body, Stomach channel in foot. (Other channels in foot too, but Stomach moistens Zong Jin, binds bones, moves joints). Amenorrhea, Bao Mai (Uterus Vessel) closed. Bao Mai belongs to Heart, connects to Bao Zhong. Now Qi presses Lung. (Qi is Fire). Heart Qi can't descend. (Heart governs Blood). Hence amenorrhea. (From Ping Re Bing Lun)
[Su Wen] Heart/abdominal fullness, eat morning can't eat evening, named Gu Zhang (Drum Distension). Treat with Ji Shi Li (Chicken Dropping Wine). One dose knows effect. Two doses cured. (Use male chicken droppings, dried 8 he, fried fragrant, non-grain wine 3 bowls, boil to half, filter juice, drink hot at dawn, bowel sounds, black stool 2-3 times at Chen/Si hours, next day foot wrinkles appear, drink again, wrinkles to knees, cured. Wu Hegao: Morning wide evening tight, disease in Ying-Blood. Chicken dropping is filth, transforms Yin, enters Ying-Blood. Qi fierce kills parasites.
