(Lips not swollen indicate Lung Heat; dryness causes lips to be dry; damp-heat leads to lip ulceration; excessive Lung Heat results in nodules on the lips; Huo or disease (a condition characterized by oral and genital ulcers) presents with purulent sores on the upper or lower lips; Wind causes lip tremors; deficiency manifests as pale, colorless lips; Cold causes blueish lips or lockjaw.)
The tongue is the sprout of the Heart. When the Heart is tranquil, the tongue is stable; when the Heart is disturbed, the tongue becomes chaotic. Furthermore, the collaterals of the Lung connect to the root of the tongue. If there is heat in the Heart and Lung, their external manifestation cannot remain tranquil, resulting in continuous twitching and protrusion of the tongue that fails to retract. The Spleen is the official responsible for granaries (digestion), and its luster manifests in the lips. Additionally, the Stomach meridian encircles the lips and mouth. Lip smacking, hardness, and swelling are signs of Spleen Fire. Applying softened broken lotus leaves or a mixture of Cinnabar and honey externally may help; however, if the lip tissue is already damaged, it is incurable. After the rash subsides, if these symptoms persist, heavy doses of Huanglian (Coptidis Rhizoma), Shigao (Gypsum Fibrosum), Lianxin (Nelumbinis Plumula), Zhizi (Gardeniae Fructus), and Huangbai (Phellodendri Cortex) should be used to clear the Heart's sprout. Since the tongue is the sprout of the Heart and the mouth is the orifice of the Spleen, reducing Fire will naturally dissipate Heat.
The syndrome of mania, though not always observed directly, generally arises from external constraint by Wind-Cold, closure of the interstices (Cou Li), dryness of the Large Intestine, fever without sweating, and the郁结 (stagnation) of toxic fire that cannot vent. In the initial stage, Mahuang San (Ephedra Powder) combined with Dahuang (Rhei Radix et Rhizoma) is used. After the rash recedes, treatment with Shigao (Gypsum Fibrosum, 4 liang), Zhimu (Anemarrhenae Rhizoma), and Huanglian (Coptidis Rhizoma, 1 qian each) can lead to recovery.
The Classics state: Irritability and restlessness result from the obstruction of toxic pathogens and disharmony of the interior and exterior. Scratching indicates pruritus associated with the rash. Restlessness indicates internal Heat. Moaning indicates pain. "Fan" (vexation) refers to dryness of Qi and Blood. The Classics state: Internal vexation with external agitation and urinary retention indicate accumulation of Heat in the Small Intestine. This symptom often occurs in measles patients, causing restlessness day and night.
Delirium, mania, and prolonged crying require treatment involving diaphoresis (sweating) and unblocking both the greater and lesser bowels. The Classics state: For mental confusion and restlessness, use Liangge San (Cooling the Diaphragm Powder) to relieve chest oppression, release the exterior, vent toxins, cool blood, resolve toxicity, and clear Fire to eliminate the root cause. It also states: Dryness of the Kidney causes vexation of the Heart, where Fire is constrained by Water and forced outward. The Classics state: For dryness and urgency of the Kidney, pungent foods should be consumed to moisten. The Lung governs vexation, and the Kidney governs dryness. In cases involving Phlegm, Fire, Toxins, Parasites, or Pruritus, herbs such as Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Maidong (Ophiopogonis Radix), Zhimu (Anemarrhenae Rhizoma), Beimu (Fritillariae Bulbus), Lianqiao (Forsythiae Fructus), Cheqianzi (Plantaginis Semen), Lingyangjiao (Saigae Tataricae Cornu), Xijiao (Rhinoceoris Cornu - *Note: Use substitutes due to conservation laws*), Fangfeng (Saposhnikoviae Radix), Dancao (Gentianae Radix), Genge (Puerariae Radix), Donghua (Adenophorae Radix), Fuling (Poria), Chaihu (Bupleuri Radix), Shanzhi (Gardeniae Fructus), Huaihua (Sophorae Flos), Shigao (Gypsum Fibrosum), and Gualouren (Trichosanthis Semen) may be used. Additionally, Xuanshen (Scrophulariae Radix), Sheng Gancao (Glycyrrhizae Radix et Rhizoma), Fuling (Poria), and Shanzhi (Gardeniae Fructus) should be added to clear Lung Fire, preferably in heavy doses, or formulas like Dahuang (Rhei Radix et Rhizoma) and Liuyi San (Six-to-One Powder) may be employed.
