(The symptoms of eclampsia are roughly similar to those of spasm.)
Xue Lizhai treated a woman who usually had a bitter taste in her mouth, irregular menstruation, and occasional alternating chills and fever. At five months of pregnancy, her arms were sometimes spasmodic and tight, sometimes flaccid and relaxed. This was caused by Liver fire and blood deficiency. She was treated with Siwu Tang (Four Substances Decoction) added with Chaihu (Bupleuri Radix), Shanzhi (Gardeniae Fructus), Danpi (Moutan Cortex), and Gouteng (Uncariae Ramulus cum Uncis), and she recovered. (Xu Lingtai's note: "Flame rises and makes things bitter"; a bitter taste in the mouth is due to Gallbladder fire flaming upward.)
Another pregnant woman, due to anger, experienced chills and fever, shaking of the head and neck, and convulsions of the four limbs. This was due to Liver fire, blood deficiency, and wind-heat. She was treated with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) added with Gouteng (Uncariae Ramulus cum Uncis), and recovered after several doses.
Another pregnant woman had a stiff neck and pain in the lower back. This was caused by wind-pathogen invading the Bladder meridian. She was cured with one dose of Bacui Qianghuo Tang (Extraction Notopterygium Decoction). Subsequently, Duhuo Jisheng Tang (Angelica Pubescens Decoction for Preserving the Living) and Bazhen Tang (Eight Treasures Decoction) were used to dispel pathogenic factors and consolidate the root, leading to recovery.
Another pregnant woman could not extend her four limbs. After taking drugs that dispel wind and dry blood, she developed urinary incontinence, excessive phlegm, and limb convulsions. This was due to Liver fire and blood dryness. The main treatment was Bazhen Tang (Eight Treasures Decoction) added with stir-fried black Huangqin (Scutellariae Radix), assisted by Gouteng Tang (Uncariae Ramulus Decoction), which stabilized the condition. Later, due to anger, the previous symptoms recurred with hematuria, chills and fever, insomnia, and poor appetite. Gouteng San (Uncariae Ramulus Powder) added with Shanzhi (Gardeniae Fructus) and Chaihu (Bupleuri Radix) stopped the bleeding. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) resolved the chills and fever and restored sleep. Liujunzi Tang (Six Gentlemen Decoction) added with Baishao (Paeoniae Radix Alba) and Gouteng (Uncariae Ramulus cum Uncis) improved appetite and gradually stabilized the patient.
Wan Mizhai treated a maid near term who suffered from facial deviation, opisthotonos, flexion of hands and feet, and unconsciousness. Huanglian Jiedu Tang (Coptis Decoction for Detoxification) added with Zhusha (Cinnabar) was pried open and administered orally, slightly stabilizing her. A male infant was born that night. Postpartum, she remained comatose. Seven Zhen Tang (Seven Treasures Decoction) was given, and she immediately calmed down. (According to Wan, this is eclampsia.)
Xue Lizhai treated a pregnant woman who sweated, had trismus, opisthotonos, and intermittent episodes. This was due to anger stirring Liver fire. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) gradually improved the condition; Gouteng San (Uncariae Ramulus Powder) stopped the spasms; and Sijunzi Tang (Four Gentlemen Decoction) added with Gouteng (Uncariae Ramulus cum Uncis), Shanzhi (Gardeniae Fructus), and Chaihu (Bupleuri Radix) stabilized her.
Another pregnant woman fell to the ground due to anger, revived after a long time, vomited phlegm, had convulsions, trismus, and neck stiffness. Lingyangjiao San (Antelope Horn Powder) gradually improved the condition; Gouteng San (Uncariae Ramulus Powder) led to recovery; and Guipi Tang (Restore the Spleen Decoction) stabilized her.
Sun Wen Yuan treated Mrs. Huang, who was young and in her first pregnancy, nearing full term. Suddenly, in the middle of the night, she muttered, looked upward, exhibited opisthotonos, exposed herself without shame, had facial deviation, was confused and unconscious, and could not speak when asked. This was eclampsia formed by wind-phlegm stirred by anger. It should be treated according to Yun Qizi's Gegen Tang (Kudzu Root Decoction) with Dafupi (Arecae Pericarpium) added. One or two doses would cure it. Gegen (Puerariae Lobatae Radix), Beimu (Fritillariae Bulbus), Danpi (Moutan Cortex), Fangfeng (Saposhnikoviae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Fuling (Poria), Guixin (Cinnamomi Cortex), Zexie (Alismatis Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma) each 2 qian, Duhuo (Angelicae Pubescentis Radix) and Renshen (Ginseng Radix) each 4 qian were decocted in water and administered, restoring consciousness. (Original note: Beimu (Fritillariae Bulbus) facilitates childbirth; for those not yet at term, Shengma (Cimicifugae Rhizoma) should be substituted.)
Lu Xiaoyu treated the daughter of Prefect Xie. Both she and her husband were young. At nearly seven months of pregnancy, she became furious after scolding a maid, then quarreled with her husband, crying loudly for half a day. That night, she could not sleep. By midnight, she spoke deliriously, looked upward, and emerged naked from the bed. Her husband held her tightly, but she became confused, unconscious, and unresponsive. Doctors did not recognize the disease and attributed it to demonic possession. Mr. Xie dressed in red robes and wielded a sword to suppress it, but she screamed, laughed, and cursed endlessly. Lu Xiaoyu was summoned. After examining the symptoms and having several women hold her pulse, he found all six pulses surging, rapid, and forceful. He said, "This is an eclampsia syndrome, not demonic possession." Such syndromes are seen occasionally, but this case is particularly severe. Zhenzheng Xiatic Qu (True Cloud Sky Fermented Dough), Beimu (Fritillariae Bulbus), Huanglian (Coptidis Rhizoma), Shanzhi (Gardeniae Fructus), Tianma (Gastrodiae Rhizoma), Qingpi (Citri Reticulatae Pericarpium Viride), Baishao (Paeoniae Radix Alba), Longdancao (Gentianae Radix), and Qingdai (Indigo Naturalis) were used, added with Dengxincao (Junci Medulla) and Zhuli (Bambusae Caulis in Taenia), resulting in awakening after one dose, halving of symptoms after two doses, and complete recovery after four doses. She had no memory of the illness.
Wu Qiao treated the wife of Cheng Jun, who was nearly four months pregnant. While wearing clogs on a bridge plank, she slipped and fell. Upon being helped up, she looked upward, closed her eyes, and spoke as if possessed. Wu Qiao observed that the Cun Kou (inch position) pulse was moving but faint, while the Chi position (cubit position) pulse was uninterrupted upon pressure, with the right side slightly stronger. He said, "This is not a serious illness; it will be easily resolved. The fetus is safe; a boy will be born." Listeners were astonished. Large doses of Renshen (Ginseng Radix) and other calming-spirit and stabilizing-mind medicines were administered. After taking only half, she sighed, opened her eyes slightly, and recounted encounters with the deceased before closing her eyes again. The same medicine was continued, she woke up, gradually stabilized, and recovered in seven days. Someone asked why a previous Cheng family woman from Yucun died in seven days despite similar symptoms. Wu Qiao replied, "I was not present then and dare not speculate. However, this patient has deficient Middle Qi. Pregnancy consumes maternal Qi almost entirely. The mother loses nourishment and is shocked unexpectedly. Qi sinks downward while Fire flares upward, Phlegm obstructs the Heart collaterals, causing confusion and a desire to die, but it is not true death." Regarding seeing ghosts, he cited the classics: "Those who lose Yang see ghosts," which is not strange. (Xu Lingtai's note: This must involve Phlegm. If it were true Yang collapse, it would be a critical failure; calming the spirit and stabilizing the mind could not lead to immediate recovery.)
Shen Yaofeng stated: There are three main origins for diseases in pregnant women: First, Yin deficiency. Human essence and blood are limited and gathered to nourish the fetus, inevitably depleting Yin. Second, Qi stagnation. An obstacle in the abdomen causes stagnation of ascending and descending Qi. Third, Phlegm-fluid retention. Organs and viscera are adjacent; the addition of a mass in the abdomen renders their mechanisms ineffective, causing body fluids to accumulate into Phlegm-fluid. Knowing these three points prevents confusion by false theories. Sudden collapse and silence in pregnant women, or facial deviation, or limb tremors, are all named Wind Stroke. Opisthotonos with intermittent consciousness is named Wind Spasm, also known as Eclampsia. Historically, it was treated as Wind, but sudden collapse and silence are actually Jue (Syncope/Reversal Cold), defined as Yin deficiency below and solitary Yang reversing upward. Facial deviation and limb tremors may be due to Phlegm obstructing collaterals or Yin deficiency failing to absorb Liver Yang, stirring internal wind. As for opisthotonos, terminal signs include fixed upward gaze (Daiyan). Why? The Foot-Taiyang Bladder meridian originates at the inner canthus, crosses the forehead, meets at the vertex, follows the shoulder-blade interior, runs along the spine, and reaches the waist. Foot-Taiyang governs body fluids. When deficient, meridians contract intermittently, causing opisthotonos. The classics state: "High pupils indicate Taiyang insufficiency," meaning insufficient Taiyang fluids. Constricted and urgent meridians cause high pupils, and severely, Daiyan. Treatment should focus on nourishing fluids using equal parts Dihuang (Rehmanniae Radix) and Maidong (Ophiopogonis Tuber). Yang deficiency in prenatal diseases is extremely rare; do not mistakenly use Xiao Xuming Tang (Minor Xu Ming Decoction).
Shen Yaofeng treated the wife of Qian Huyun. Her diet and起居 (daily life) were normal, but she experienced dozens of syncope episodes in one night. During attacks, her eyes rolled upward, and she resembled a corpse. The next day, dozens more episodes occurred. By evening, one episode did not resolve. Burning charcoal was thrown into vinegar and fumed near her nose, but she felt nothing. Her pulses in all three positions responded, neither rapid nor slow, showing no abnormalities. After diagnosis, her father asked if it was treatable. Shen said, "Use one jin of Qingqian (Galena) melted, poured into basin water, retrieved, remelted, and repeated three times. Use the water to decoct Shengdi (Rehmanniae Radix Preparata) 1 liang, Tiandong (Asparagi Tuber) 2 qian, Shihu (Dendrobii Caulis), Gancao (Glycyrrhizae Radix et Rhizoma) 1 qian, and Shichangpu (Acori Tatarinowii Rhizoma) 1 qian. Take orally." That night, syncope reduced to six times and was mild. Taking the prescription again stopped the syncope completely. She later gave birth to a son. Calculating the time, this was the first month of conception. Transferred to treating middle-aged women, it is also very effective for non-pregnant patients.
