Xu Zhongguang treated a pediatric patient whose exanthema initially appeared on the right Taiyang (Bladder Meridian of Foot-Taiyang) region, presenting with alternating chills and fever. The lesions were pale in color with flat, sunken bases, and were small, fragmented, and dense. Initially, when releasing the exterior was indicated, Guizhi Tang (Cinnamon Twig Decoction) and Baishao (Radix Paeoniae Alba) were mistakenly used for their astringent properties.
To address the flat and non-elevated bases of the lesions, Zhenzhe Dan (Shocking Insect Pill), wine slurry, and silkworms were employed to promote eruption. On the fifth day, vomiting persisted; Huoxiang (Pogostemonis Herba) and Huanglian (Coptidis Rhizoma) stir-fried with ginger were administered to stabilize the stomach. By the sixth day, although vomiting ceased, the lesions remained dry and lacked moisture. Baizhi (Angelicae Dahuricae Radix), Huangqi (Astragali Radix), and Shan Jia (Pangolin Scales) were used to facilitate the fermentation and generation of pustular fluid. On the seventh day, since the pustular fluid had not yet filled out, the previous prescription was continued with the addition of 8 fen of Renshen (Ginseng Radix et Rhizoma). Additionally, water chestnut juice and wine slurry were given separately. After two days of this regimen, the facial surface finally became filled with pus, though the chest and limbs still showed unfilled lesions, accompanied by coughing and hoarseness. At this stage, the eighth day, the condition was critical. Fortunately, the child could eat and drink, and there was no diarrhea. A formula containing Renshen (Ginseng Radix et Rhizoma), Huangqi (Astragali Radix), Maidong (Ophiopogonis Radix), Chenpi (Citri Reticulatae Pericarpium), Gancao (Glycyrrhizae Radix et Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Honghua (Carthami Flos), Baishao (Radix Paeoniae Alba), Qianhu (Peucedani Radix), and Jiegeng (Platycodonis Radix) was prescribed. This was combined with Cured Human Excrement (Shao Ren Shi) and 3-4 fen of Cuizhe Dan (Eruptive Pill). After inducing prolonged vomiting, the pus spread throughout the body. At that time, the facial lesions had flattened and dried, but subsequently developed red halos around the roots, with accessory eruptions appearing nearby. The previously sunken lesions became sparse, and the dry ones became moist. Later, due to severe phlegm-cough and persistent hoarseness, the prescription was changed to Shandougen (Sophorae Tonkinensis Radix), Zhimu (Anemarrhenae Rhizoma), Beimu (Fritillariae Thunbergii Bulbus), Maidong (Ophiopogonis Radix), Huangqi (Astragali Radix), Niubangzi (Arctii Fructus), Lianqiao (Forsythiae Fructus), Chenpi (Citri Reticulatae Pericarpium), Danggui (Angelicae Sinensis Radix), Baishao (Radix Paeoniae Alba), and Gancao (Glycyrrhizae Radix et Rhizoma). Daily oral care involved using willow twigs to gently stir the mouth and remove white fungal-like deposits from the throat. Separately, a powder composed of Cured Human Excrement (Shao Ren Shi), Shandougen (Sophorae Tonkinensis Radix), Pengsha (Borax), Yaxiao (Natrii Nitras), Zhusha (Cinnabar), Bingpian (Borneolum Syntheticum), Danningxing (Arisaematis Cum Bile), and Qingdai (Indigo Naturalis) was administered. After three to four days, the cough stopped, the voice returned, and the patient recovered.
Wan Mizhai treated the son of the Wang family. As the variola (smallpox) lesions were about to crust over, they appeared grayish-white and ulcerated, accompanied by coma and unresponsiveness. Wan stated, "There is no danger; simply wait three days for the lesions to crust." When asked how recovery could occur without medication, Wan explained, "The whiteness of the lesions results from excessive heat turning them pale, resembling the rotting state of ripe fruit; this is not a deficiency syndrome. The coma is a sign that pathogenic factors have been exhausted and healthy qi is returning, indicating a transition from misfortune to good fortune, rather than true delirium. Wait another three days, and as healthy qi recovers, the patient will heal." This prediction proved correct.
