Wan Mizhai's eldest grandson was born with sores and scabies all over his body. Therefore, he formulated a prescription using *Zaocys* (Ushao She), soaked in wine, skin and bones removed, with the clean meat dried by roasting to one qian; *Sophorae Flavescentis Radix* (Kushen) soaked in wine, sliced and sun-dried, ground into powder taking one and a half qians; *Tribuli Fructus* (Bajili) stir-fried to remove spines, taking one and a half qians. These three ingredients were ground into powder, mixed with wine paste to make pills the size of millet seeds. Each time, fifteen pills were taken, washed down with decoction of bamboo leaves. The parasitic scabies would disappear and not recur.
A child had parasitic scabies on his body. A physician applied medicinal ointment, causing the sores to completely retract, but the child developed abdominal distension and wheezing. He sought treatment from Wan Mizhai. Wan said, "Fortunately, convulsions have not occurred; it is still treatable." He prescribed *Xionghuang Jiedu Wan* (Realgar Detoxification Pill), administered with decoction of bamboo leaves and *Juncus* (Dengxincao). After the child discharged yellow phlegm, the sores reappeared, and the condition stabilized. Someone asked, "Can parasitic scabies not be treated with topical applications?" Wan replied, "Parasitic scabies is a type of fetal toxin. It is appropriate to use detoxifying medicines to allow the toxin to disperse externally. One must not recklessly use topical drugs that force the toxin back inward. Topical remedies for sores often contain realgar (*Pishuang*) and mercury to kill parasites; the toxic qi of these drugs can enter the interior through deficiency, endangering the child's life. Caution is advised. (Wu Qian notes: Adults also present with this syndrome when suffering from sores and scabies.)"
A one-month-old infant first developed sores in the mouth, which then spread to the entire body, lasting for over a year without healing. *Smilax Glabrae Rhizoma* (Tufuling) was ground into powder and administered mixed with breast milk. The infant recovered after more than a month.
Another infant, one month old, developed parasitic scabies all over the body, with weeping, wet rotting, and skin shedding like peeling. The child cried day and night. Suddenly, one day, all the sores disappeared, followed by convulsions and death.
Feng Chuzhan treated his grandson. The mother had long suffered from yin deficiency with night fevers. Over a hundred days after birth, the infant developed widespread leprosy-like sores. (This may well be due to the mother taking *Bawei Dihuang Wan* (Eight-Ingredient Rehmannia Pill) for a long time during pregnancy. See the section on Dystocia.) Feng used *Rehmanniae Radix Preparata* (Shengdi), *Moutan Cortex* (Danpi), *Angelicae Sinensis Radix* (Danggui), *Paeoniae Radix Rubra* (Chishao), *Dioscoreae Hypoglaucae Rhizoma* (Bixie), *Polygoni Multiflori Radix* (Shouwu), *Lonicerae Japonicae Flos* (Yinhua), *Forsythiae Fructus* (Lianqiao), *Fritillariae Thunbergii Bulbus* (Tubei), *Glycyrrhizae Radix et Rhizoma* (Gancao), *Cicadae Periostracum* (Bieshi - likely a typo or local term, kept as is), *Sesami Fructus* (Huma), *Smilacis Glabrae Rhizoma* (Tufuling), and *Akebiae Caulis* (Mutong). Large doses were administered to the nursing mother day and night for dozens of prescriptions. Later, damp-heat moved downward, causing ulceration of both feet with dripping clear fluid and loss of fingernails. Those close to the nursing mother were infected. This indicated that the congenital heat-toxin qi had been fully expelled outward. Subsequently, a large toxic swelling formed behind the ear, caused by the aggregation of rootless fire due to yin deficiency. Feng used *Bawei Dihuang Wan* (Eight-Ingredient Rehmannia Pill) added with *Achyranthis Bidentatae Radix* (Niuxi) and *Schisandrae Fructus* (Wuweizi) in decoction. After several doses, the swelling suppurated and healed. Afterwards, body fluids became depleted, and the scabs dried up, with conditions alternating between healing and recurring. To prevent internal invasion of toxins, four liang of mutton were boiled in water, to which four qians of raw *Astragali Radix* (Huangqi), two qians of *Angelicae Sinensis Radix* (Danggui), three qians of *Lonicerae Japonicae Flos* (Yinhua), four fen of stir-fried * Cimicifugae Rhizoma* (Shengma), three slices of ginger, and two jujubes were added. The decoction was given to the nursing mother. In less than ten doses, the foot sores completely healed, and the toxicity rose to the head and neck. Upon further administration, the head sores also healed. (Wu Qian notes: The subsequent use of *Bawei Dihuang Wan* led to dryness of fluids after ulceration.)
Liu Yuxi's *Chuanxin Fang* records: Previously, while giving birth in Wuling, I encountered hot sores immediately postpartum. Various topical medications were ineffective, and the condition worsened daily, spreading to half the body. The infant cried loudly, refused milk, and could not sleep. Five eggs were boiled thoroughly, the whites discarded, and only the yolks retained. They were decocted with tangled hair until liquid exuded. This liquid was collected in a bowl until exhausted. It was applied to the sores, and dusted with powder of *Sophorae Flavescentis Radix* (Kushen). It proved miraculously effective. Cited in *Bencao Gangmu*.
Sun Wen Yuan treated a child who had a toxic swelling behind the ear, painful and swollen, accompanied by large blister-like sores all over the body, aversion to cold, and fever. He prescribed *Lonicerae Japonicae Flos* (Yinhua), *Angelicae Sinensis Radix* (Dangguiwei), *Glycyrrhizae Radix et Rhizoma* (Gancao), *Paeoniae Radix Rubra* (Chishao), *Forsythiae Fructus* (Lianqiao), *Bombyx Batryticatus* (Jiangcan), *Arctii Fructus* (Niubangzi), and *Scrophulariae Radix* (Yuanshen). After two doses, the symptoms subsided.
Xue Lizai treated a child whose eyes were red and swollen since birth, sometimes itching, sometimes developing nebulae. This was liver fire from the fetal stage. He cured him with *Aloe Pill* (Luhui Wan) and *Liudihuang Wan* (Six-Ingredient Rehmannia Pill).
Wan Mizhai treated a five-year-old child who developed pustular sores all over the body every spring. This was fetal toxin. He warned against using topical applications, fearing that the toxicity of powdered realgar might enter the abdomen through deficiency. The child recovered after covering the sores with sesame.
Li Sishou of Wanzhou had a son aged five who developed wet scabies all over the body, which suddenly dried up. Wan said, "Once the sores appear and the convulsions stop, there is no cause for worry." Several physicians had treated the case unsuccessfully before.
Xue Lizai treated the grandson of Grand Secretary Liang. At just one year old, the child had fetal toxin on the neck. Xue waited until suppuration occurred, then punctured it. About one bowl of pus drained out, and the child's feeding remained normal. He administered tonifying and promoting pus-discharge medicines, and the child recovered after more than a month. Grand Secretary Jin's son suffered from the same condition. He waited for the pus to discharge naturally and nearly died. My hometown scholar Xu Neihan's son suffered from smallpox toxin. Timely puncturing prevented internal invasion of the toxin, and he recovered in a few days. Children have weak blood and qi. If pus forms but is not punctured, death is rare to avoid.
