妊娠胎动，有伤仆忤触动而不安者，人皆见证施治，故无差谬。若内伤而动，所因不同，治法亦异，

佛手散（见本卷诸痛门。）
Fetal movement and instability during pregnancy may result from falls or physical trauma; in such cases, treatment based on observed symptoms rarely leads to errors. However, when fetal instability arises from internal injury, the causes vary, and thus the treatment methods differ, often leading to mistakes by practitioners. Nevertheless, these conditions generally fall into four categories: deficiency (Xu), excess (Shi), cold (Han), and heat (Re). If one can discern the origin of the disease and regulate it accordingly, stability will naturally be achieved. Therefore, formulas for stabilizing the fetus should not be rigidly applied. Adhering strictly to specific months of gestation for medication is as inflexible as fixing the bridge of a zither before playing it. It is important to note that while medical texts often employ cooling herbs for fetal stabilization, there are instances where warming and tonifying methods are appropriate.
For cases of deficiency-induced instability, whether due to insufficiency of the Chong and Ren meridians resulting in weak conception, spleen-stomach Qi deficiency failing to secure the fetus, or damage from excessive sexual activity, fatigue, improper diet, or emotional disturbances, one must distinguish whether the pathology lies in Qi or Blood, and whether it involves deficient heat or cold, or false cold/heat patterns. Treatment should address the specific condition: tonify deficiency, and caution should be exercised. For excess-induced instability, caused by food stagnation, Qi stagnation, liver injury from anger, or spleen injury from depression affecting the blood vessels, one should identify the cause and promote free flow. For cold-induced instability, characterized by acid regurgitation, nausea, aversion to cold, lower jiao cold with diarrhea, habitual miscarriage, or slippery loss due to deficiency-cold unresponsive to cooling-tonifying methods, with a deep, fine, and weak pulse indicating Yang deficiency and cold syndrome, warming the middle Jiao to stabilize the fetus is indicated.
My wife suffered multiple miscarriages despite avoiding sexual activity, careful living habits, and consulting specialized gynecologists, all to no avail. Although I have some knowledge of medicine, my busy official duties prevented me from gaining the clinical experience and intuitive skill of a professional practitioner. Relying entirely on doctors, she experienced five consecutive miscarriages despite attempts at stabilization. Reviewing their prescriptions, they merely used cooling to nourish the fetus, tonified Qi and Blood, and astringed slipperiness. Upon her subsequent pregnancy, I diagnosed a weak pulse and administered strong tonification. At seventy days, bleeding occurred again, accompanied by soreness in the waist but no abdominal pain. This aligns with obstetric texts stating: both fetal movement (Tai Dong) and fetal leakage (Tai Lou) cause bleeding; fetal movement involves abdominal pain, whereas fetal leakage does not. Fetal movement requires regulating Qi, while fetal leakage requires clearing Heat. Since previous cooling treatments failed, I reasoned that repeated miscarriages implied Yin-Blood deficiency-cold causing slipperiness and Yang-Qi deficiency-cold failing to secure. Thus, I employed a method to warm the middle, adding Charred Ginger (Jiang Tan) and Charred Cinnamon Twig (Gui Tan) to Ginseng (Renshen), Atractylodes (Baizhu), Angelica (Danggui), Rehmannia (Dihuang), Dipsacus (Xuduan), Donkey-hide Gelatin (Ejiao), and Mugwort (Aiyi). Following the principle that "black stops red" (hemostasis), the bleeding stopped and the fetus stabilized after a few doses. This led me to realize that ancient masters cured morning sickness (E Zu) using Er Chen Tang, Liu Jun Zi Tang, Fresh Ginger (Sheng Jiang), and Pinellia (Ban Xia), confirming that warming the middle can also stabilize the fetus.
For heat-induced instability, marked by irritability, thirst, dryness, vaginal bleeding, or hematuria, or harm from inappropriate warming-tonifying drugs, clearing Heat and nourishing Blood is required. These illustrate the differences among deficiency, excess, cold, and heat. If the mother's illness causes fetal instability, treating the mother stabilizes the fetus. If fetal instability causes maternal illness, stabilizing the fetus cures the mother. For mild fetal movement with slight discomfort or spotting, use An Tai Yin promptly. For severe cases with abdominal pain, waist soreness, and bearing-down sensation suggesting imminent loss, use Fo Shou San. If the fetus is intact, it stabilizes; if damaged, it expels. Post-expulsion, treat according to symptoms. Physicians must examine carefully and apply flexible methods to save most cases. Blind adherence to partial views, worsening deficiency or excess, drives away what one wishes to keep, bringing shame.
According to *Bao Chan*, fetal instability in the second or third month often stems from long-term uterine deficiency and depleted Blood Sea, leading to miscarriage, a critical state unlike normal delivery. Those suffering this should take Du Zhong Wan prophylactically. I frequently follow Danxi’s An Tai Yin formulas for pregnant women with weakness, unstable Qi, poor appetite, frequent miscarriages, or abdominal/waist pain, finding them highly effective. I also used Hei Bai An Tai San for impoverished patients with remarkable results. Below are selected formulas with modifications for reference.
