Postpartum edema of the hands and feet, characterized by shiny and lustrous skin, as well as symptoms caused by Spleen deficiency failing to control water and Kidney deficiency failing to transform water, must be treated primarily by strongly tonifying Qi and Blood. This should be supplemented with Cangzhu (Atractylodis Rhizoma), Fuling (Poria), and Baizhu (Atractylodis Macrocephalae Rhizoma) to strengthen the Spleen. If distension and fullness are present, add Banxia (Pinelliae Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Muxiang (Aucklandiae Radix), and Xiangfu (Cyperi Rhizoma) to regulate Qi. For patients with deficiency constitution, add Renshen (Ginseng Radix et Rhizoma) and Mutong (Akebiae Caulis). If heat signs are present, add Maidong (Ophiopogonis Tuber) and Huangqin (Scutellariae Radix) to clear Lung fire.
Jianpi Lishui Buzhong Yiqi Tang (Spleen-Strengthening Water-Draining Middle-Jiao-Qi-Supplementing Decoction): Indicated for conditions occurring more than seven days postpartum.
Ingredients: Renshen (2 qian), Baizhu (2 qian), Fuling (1 qian), Baishao (1 qian), Chenpi (0.2 qian), Mugua (0.8 qian), Mutong (0.4 qian), Zisu (0.4 qian), Cangzhu (0.4 qian), Houpo (0.4 qian), Dafupi (0.4 qian).
If constipation is present, add Rongzong (Cistanche Deserticola) 1 qian and Haren (Cannabis Seed) 1 qian. If the condition arises from Cold pathogen invasion and Dampness damaging the exterior, resulting in anhidrosis and edema, it is appropriate to use Shengjiangpi (Zingiberis Recens Cortex), Banxia, and Ziye (Perillae Folium), added to a formula that tonifies Qi and Blood to induce sweating and release the exterior.
Wupi Yin (Five Peel Decoction): Indicated for postpartum Dampness invading and damaging the Spleen meridian, causing Qi and Blood stagnation, leading to puffy edema of the face and eyes, swelling of the four limbs, and wheezing. Use Wujiaopi (Acanthopanacis Cortex), Dafupi (Arecae Pericarpium), Fulingpi (Poria Cum Sylpha), and Shengjiangpi (Zingiberis Recens Cortex), decocted in water and taken orally.
Postpartum worry, fright, fatigue, and excessive blood loss lead to inner restlessness and agitation, termed Zhenchong (palpitations with restlessness). If there is sudden fright, timidity, and a sensation as if someone is about to capture one, it is termed Jingji (palpitations with fright). The treatment for these two syndromes lies solely in harmonizing the Spleen and Stomach, tonifying Qi and Blood, stabilizing the will, calming the spirit, ensuring smooth Qi flow, and pacifying the Heart mind, after which the disease will resolve. If lochia clots have not yet dissipated after delivery, Shenghua Tang (Birth Recovery Decoction) should be taken to nourish Blood and dispel stasis; once Blood is abundant, Zhenchong and Jingji will naturally subside, without the need for additional sedative or Spirit-calming agents. If this syndrome persists after the clots have dissipated and pain has ceased, Jiajian Yangrong Tang (Modified Nourishing Vigor Decoction), which is Guipi Tang (Spleen-Restoring Decoction) modified by removing Chuanxiong (Ligustici Chuanxiong Rhizoma) and Maidong, and adding Muxiang, should be taken.
Jiajian Yangrong Tang Ingredients: Chuanxiong (1 qian), Danggui (2 qian), Fushen (Poria with Hostwood, 1 qian), Zaoren (Ziziphi Spinosae Semen, 1 qian), Renshen (1 qian), Maidong (1 qian), Baizhu (2 qian), Yuanzhi (Polygalae Radix, 2 qian, heart removed), Huangqi (Astragali Radix, 2 qian), Longyanrou (Longan Aril, 8 pieces), Chenpi (0.4 qian), Gancao (Licorice Root, 0.4 qian).

For empty heat and vexation, add Zhuru (Bambusae Caulis in Taenia, 1 pill). For Phlegm, add Zhuli (Bambusae Sap) and Jiangzhi (Zingiberis Juice).
Yangxin Tang (Heart-Nourishing Decoction): Indicated for postpartum Heart Blood unrest, causing fright and palpitations.
