Postpartum hemorrhage (profuse vaginal bleeding) requires observation of the blood's color (red or purplish) and the patient's constitutional strength and Qi deficiency. If the color is predominantly purple, it indicates stasis (blood stasis) that should be expelled; it should not be treated as metrorrhagia. If the blood is bright red, it results from fright injuring the Heart, causing it to fail to govern the blood; anger injuring the Liver, causing it to fail to store the blood; or overwork injuring the Spleen, causing it to fail to control the blood. In such cases, it should be treated as metrorrhagia. The appropriate method is to first administer Shenghua Tang (Birth Recovery Decoction) for several doses. This allows for both moving blood and tonifying, so that blood generation will naturally flourish. For cases involving extreme physical collapse, rapid breathing, or incessant sweating, a modified Shenghua Tang with doubled Ren Shen (Ginseng Radix et Rhizoma) should be taken to tonify Qi. When Yang is generated, Yin grows, and blood production flourishes; this cannot be treated with hemostatic agents such as Zong Tan (Carbonized Palm Fibers). If more than a month has passed since delivery, Shengju Dabu Tang (Great Tonification and Lifting Decoction) is appropriate. This formula is also suitable for elderly patients with chronic weakness suffering from metrorrhagia.
Jia Shen Shenghua Tang (Modified Shenghua Tang with Ginseng)
Indications: Postpartum metrorrhagia, physical collapse, profuse sweating, and rapid breathing.
Chuan Xiong (Ligusticum Chuanxiong Rhizome, 3 qian), Dang Gui (Angelica Sinensis Radix, 4 qian), Jing Jie (Schizonepetae Herba, 4 fen), Tao Ren (Persicae Semen, 10 grains), Ren Shen (Ginseng Radix et Rhizoma, 3 qian), Rou Gui (Cinnamomi Cortex, 5 fen, remove after two doses), Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma, 5 fen), Da Zao (Ziziphus Jujuba Fructus, 2 pieces).
Decoct in water and take hot. For profuse sweating, add Huang Qi (Astragali Radix) and Ren Shen (Ginseng Radix et Rhizoma), each 3 qian. For thirst, add Mai Dong (Ophiopogonis Radix) and Wu Wei Zi (Schisandrae Chinensis Fructus). For diarrhea, add Fu Ling (Poria) and Lian Zi (Nelumbinis Semen). If the pulse is deep and hidden in both hands, or absent in the right hand, use Shengmai San (Generate Pulse Powder) with doubled Ren Shen (Ginseng Radix et Rhizoma). For phlegm, add Zhu Li (Bambusae Caulis in Taenia) and Jiang Zhi (Zingiberis Recens Juice), plus one cup of wine. For cough, add Xing Ren (Armeniacae Semen Amarum), Zhi Mu (Anemarrhenae Rhizoma), and Jie Geng (Platycodonis Radix), each 1 qian. For palpitations due to fright, add Suan Zao Ren (Ziziphi Spinosae Semen) and Bai Ren (Biota Orientalis Semen), each 2 qian. For prolonged anorexia and vomiting upon smelling medicine, use Du Shen Tang (Single Ginseng Decoction): 2-3 qian of Ren Shen (Ginseng Radix et Rhizoma) decocted in one cup of water until reduced to a scum-like consistency, taken to stimulate appetite. For continuous profuse bright red bleeding, add Sheng Ma (Cimicifugae Rhizoma) and Bai Zhi (Angelicae Dahuricae Radix), each 5 fen. If there are no symptoms of sweating or collapse, simply taking Shenghua Tang (Birth Recovery Decoction) with fried black Jing Jie (Schizonepetae Herba) ensures safety. It is a great error to claim that Chuan Xiong (Ligusticum Chuanxiong Rhizome) and Dang Gui (Angelica Sinensis Radix), which invigorate blood, cannot treat metrorrhagia.
Shengju Dabu Tang (Great Tonification and Lifting Decoction)
Indications: Prolonged postpartum metrorrhagia that does not cease, passage of large clots like chicken eggs, or shedding of blood in sheets. The treatment principle is to greatly tonify the Spleen and Stomach, lift Qi and Blood, and add a small amount of herbs that anchor and descend Heart Fire. This formula also treats metrorrhagia in patients of all ages.
Ren Shen (Ginseng Radix et Rhizoma), Bai Zhu (Atractylodis Macrocephalae Rhizoma), each 3 qian; Chuan Xiong (Ligusticum Chuanxiong Rhizome, 1 qian); Dang Gui (Angelica Sinensis Radix, 1.5 qian); Shu Di Huang (Rehmanniae Radix Praeparata, 2 qian); Huang Qi (Astragali Radix, 1 qian); Bai Zhi (Angelicae Dahuricae Radix, 4 fen); Jing Jie (Schizonepetae Herba, 4 fen); Chen Pi (Citri Reticulatae Pericarpium); Huang Lian (Coptidis Rhizoma); Huang Bo (Phellodendri Cortex, stir-fried; avoid if diarrhea is present); Qiang Huo (Notopterygii Rhizoma et Radix); Fang Feng (Saposhnikoviae Radix), each 4 fen; Sheng Ma (Cimicifugae Rhizoma); Gan Cao (Glycyrrhizae Radix et Rhizoma), each 5 fen.
For thirst, add Mai Dong (Ophiopogonis Radix) and Wu Wei Zi (Schisandrae Chinensis Fructus). For diarrhea, add Ze Xie (Alismatis Rhizoma) and Lian Zi (Nelumbinis Semen). For phlegm, add Ban Xia (Pinelliae Rhizoma). For concurrent leukorrhea, add Cang Zhu (Atractylodis Rhizoma) and Ban Xia (Pinelliae Rhizoma), each 1 qian.
