Sudden severe pain in the heart and abdomen, cyanosis of the face, cold sweating, and faintness of breath indicate a critical condition with imminent death.
Chao Yuanfang stated: Pregnancy-related epigastric and abdominal pain may be caused by pre-existing cold-phlegm (Cold Phlegm) retention within the abdomen or by recent exposure to wind-cold pathogens. These conditions arise due to deficiency of the Zang-Fu organs. The pathogenic factor and healthy Qi clash and merge with the Qi mechanism, moving up and down along with it. When rushing upward to the Heart, it causes Heart pain; when rushing downward to the abdomen, it causes abdominal pain, thus resulting in heart and abdominal pain. During pregnancy, pain occurs due to the internal clash between healthy Qi and pathogenic factors. If not resolved promptly, the pain will strike the Bao Luo (Uterine Collaterals), inevitably disturbing the fetus, and in severe cases, causing miscarriage or abortion. Chi Shi said: Sudden severe pain in the heart and abdomen during pregnancy is caused by blood depletion and fetal damage, leading to fetal movement. Cyanosis of the face and cold sweating indicate that the Heart and Spleen lack blood nourishment, leading to impending exhaustion of Qi.
I note: Xue Lizhai's *Furen Liang Fang* (Excellent Prescriptions for Women) contains Gouteng Tang (Uncaria Decoction), specifically treating pregnancy-related fetal disturbance and abdominal pain with cyanosis, cold sweating, and impending Qi exhaustion, which refers to this condition.
If vaginal bleeding persists, the fetal Qi rushes upward to the chest and abdomen along with the blood. If the heart and abdomen feel warm, the fetus has not yet died, and there may be hope for rescue. However, if palpation reveals no fetal movement, accompanied by coldness and stifling sensation in the heart and abdomen, the fetus has already died. Why would fetal stabilization be necessary?
According to Chao's *Bingyuan* (Source of Diseases): The sign of a dead fetus in the uterus during pregnancy may result from fright and falling, or from infection with epidemic toxins entering the Bao Zang (Uterus), causing fetal death. The sign is coldness at the site of the fetus, indicating fetal death.
Falling, stumbling, or lifting heavy objects can cause fetal death within the abdomen. If injury occurs but vaginal bleeding persists excessively, blood depletion leads to fetal death. As the fetus becomes increasingly dry and withered, it cannot be expelled, striking the heart and causing stifling pain, making the mother's life difficult to save.
According to Chao's *Bingyuan*: The sign of pregnancy collapse, fetal Qi rushing to the heart, and lower bleeding refers to falling while walking or falling from a height, damaging the Bao Luo (Uterine Collaterals). This causes bleeding that disturbs the fetus, and as blood is injured, Qi rebels. The fetal Qi follows the rebellious Qi upward to rush to the heart. Regarding signs of life and death for mother and child: If the mother's tongue is cyanotic, the fetus is dead and the mother lives. If the lips and mouth have no foam, the fetus lives. If the lips are cyanotic and foam is expelled, both mother and fetus die. If the lips and mouth are red and the tongue is cyanotic, the mother dies and the fetus lives. If vaginal bleeding does not stop, the Bao Gong (Uterus) becomes dry and the fetus withers, leading to fetal death.
