[Xue Ji] "Yuan" (convulsion, referring to the contraction of tendons and vessels) denotes the emergency and shrinking of tendon vessels, while "Che" (convulsion, referring to the relaxation and extension of tendon vessels) denotes the slackness and stretching of tendon vessels. One contraction and one extension cause the hands and feet to pull and twitch incessantly. Generally, this condition is similar to convulsions in infants, hence it is called Yuan-Che (choreic convulsions). This syndrome mostly belongs to Wind pathogen, as Wind governs trembling and movement. Luo Longji said: The Heart governs the vessels, the Liver governs the tendons; the Heart belongs to Fire, the Liver belongs to Wood; Fire governs Heat, Wood governs Wind. When Wind and Fire blaze together, Yuan-Che occurs. Treatment methods: If caused by Wind-Heat, use Gouteng Tang (Uncaria Decoction) added with Chai Hu (Bupleurum), Shan Zhi (Gardenia), Huang Qin (Scutellaria), and Bai Zhu (Atractylodes) to calm the Liver Wood, descend Heart Fire, and nourish Qi and Blood. If Wind-Phlegm surges upward, add Zhu Li (Bamboo Sap), Nan Xing (Arisaema), and Ban Xia (Pinellia). If acute twitching is caused by Wind pathogen, add Quan Xie (Scorpion) and Jiang Can (Bombyx Batryticatus). If due to deficiency of Qi and Blood, use Bazhen Tang (Eight-Treasure Decoction) added with Gouteng (Uncaria) and Shan Zhi (Gardenia) as the main formula. If there is weak twitching, eyes rolling upward (Dai Yan), body arching backward (Fan Zhe), and sweating like pearls, this indicates the exhaustion of the Liver, and all such cases are incurable. A pregnant woman had limbs that could not extend; after taking agents for dispelling Wind and drying Blood, she suffered from fecal incontinence and excessive phlegm, with limb convulsions. I considered it Liver Fire and dry Blood, so I used Bazhen Tang (Eight-Treasure Decoction) added with stir-fried black Huang Qin (Scutellaria) as the main formula, assisted by Gouteng Tang (Uncaria Decoction), and she recovered. Later, due to anger, the previous symptoms recurred with hematuria, alternating chills and fever, insomnia, and poor appetite. I used Gouteng San (Uncaria Powder) added with Shan Zhi (Gardenia) and Chai Hu (Bupleurum), and the bleeding stopped. I used Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder), and the chills/fever subsided and sleep returned. I used Liu Jun Zi Tang (Six-Gentlemen Decoction) added with Shao Yao (Peony) and Gouteng Gou (Uncaria Hook), and her diet improved, leading to gradual recovery.
