Commonly known as "locked belly" (Suodu), this condition is caused by heat accumulation in the womb, leading to an excess of heat-toxin that obstructs the anus, causing blockage and loss of lubrication. If stool is not passed by the third day after birth, a woman should immediately rinse her mouth with warm water and perform cupping-like suction on seven areas of the infant: the anterior chest, posterior back, below the umbilicus, and the palms and soles of both hands and feet, repeating this four or five times. Subsequently, dissolve half a qian (approx. 1.5–2 g) of Qinfen (Calomel, Calomelas) with a small amount of honey in warm water, and administer small amounts frequently until defecation occurs. If there is still no passage, it indicates imperforate anus (gangmen neihe); one must penetrate the area with an object to create an opening. A gold hairpin is preferred, followed by a jade hairpin. The pin should be inserted about two cun deep, and Suhexiang Wan (Styrax Pill) should then be inserted into the hole, with relief expected upon fecal discharge. If the abdomen becomes distended, the infant cannot nurse or eat, makes groaning sounds, and reaches the seventh day after birth, survival is unlikely. Tian’s treatment method involves first piercing the anus with a hard green onion stalk; if stool does not pass, follow up with Niuhuang San (Calculus Bovis Powder, see formula for wheezing) administered with Zhusha Wan (Cinnabar Pill), which should show results within one shichen (two hours).
When a newborn presents with severe abdominal distension resembling rupture and obstruction of both urination and defecation, apply the aforementioned suction method to the anterior chest, posterior back, palms, soles, and below the umbilicus—seven areas in total—until the skin turns red. Passage will naturally occur shortly thereafter.
The *Qianjin Fang* states: Six or seven days after birth, the infant’s qi and blood consolidate to form flesh, and the mouth, tongue, throat, and cheeks should be clear. If there are lesions in the throat or on the tongue resembling reed husks filled with water, or if the uvula (xuanyong) shows swelling, wrap silk around a long needle, leaving only a millet-grain-sized portion of the blade exposed, and puncture the lesion to drain the green, yellow, and red bloody fluid. After one puncture, stop and observe for one day. If unresolved, puncture again the next day. No more than three punctures are needed for complete resolution; minor residual issues will also resolve after three punctures. Lesions under the tongue are called Chongshe (Double Tongue); those in the cheeks and upper palate are referred to as Chongyin (Double Gums/Palate); those on the gingiva are also called Chongyin. All require puncturing to drain the bloody fluid. After puncturing, wash and wipe with salt water solution, and immediately apply Rusheng San or Yizi San to the affected area.
Rusheng San
Qianshuang (Lead Frost, Plumbum Carbonas)
Zhen Niuhuang (Real Calculus Bovis, Calculus Bovis)
Taiyin Xuanjingshi (Taiyin Crystal Stone, Selenitum)
Zhusha (Cinnabaris, processed by water levigation and dried)
Grind these four ingredients separately to an extremely fine powder. Weigh exactly two qian five fen (approx. 8–9 g) of each, mix uniformly, add Wulongnao (Borneolum Syntheticum, 0.5 qian), and grind again until uniform. For each use, take an amount equivalent to one zi (approx. 0.3–0.5 g) to half a qian, sprinkle into the infant’s mouth, and use occasionally.
Yizi San
Zhusha (Cinnabaris)
Pengsha (Borax, Boracum) — each half a qian
Longnao (Borneolum Syntheticum)
Puxiao (Natrii Sulfas, Mirabilite) — each one zi (approx. 0.3–0.5 g)
Grind all ingredients into an extremely fine powder. Mix a small amount with honey, dip a goose feather quill into the mixture, and brush it inside the mouth. Swallowing is harmless.
