Twitching without cessation, and after cessation, twitching again. Startled crying accompanied by convulsions. Sweating with cold feet. Phlegm filling the chest and throat. Mouth open and eyes staring straight up.
In acute wind (Jing Feng) [convulsive disorder], if the eyes roll back, blood flows from the mouth, both legs jump, the abdomen twitches, or the spirit is sluggish while groping the body and searching for clothes, or the condition is severe with coma and shortness of breath, medication cannot be administered, opening-the-nose powder fails to induce sneezing, there is heat and pain in the heart region, or sudden loud screaming occurs, these are untreatable. In chronic wind (Man Jing), if the four limbs are cold, vomiting, diarrhea, and coughing occur, the complexion is dark and the spirit is desolate, the voice is hoarse like a crow's croak with stomach pain, qi moves in the two hypochondria, white sores appear in the mouth, hair stands erect with head shaking, the eyes are blue-black and unresponsive, phlegm rattles with wheezing and throat obstruction, the head is soft, incontinence of urine and stool occurs, or the hands and feet are pulled to one side, all these are untreatable. In slow spleen wind (Man Pi Feng), if the body is cold with sticky sweat, lying straight like a corpse with wheezing and coughing, the head is soft and the back stiff, the jaw is locked with head shaking, the sound of phlegm resembles sawing, the face lacks luster, the lips are retracted and breathing is coarse, this is untreatable.
When treating infant acute wind (Jing Feng) during convulsions, do not cause panic or disturbance; this is not something to fear. Although chronic wind (Man Jing) appears quiet, it is a critical illness. When acute wind (Jing Feng) is convulsing, simply support the child; do not forcibly restrain them, as the pathogenic wind-qi is vigorous and may flow into the sinews and vessels, leading to contracture of the hands and feet.
[Xue Ji] The human body has nine zang organs. The Heart stores the Shen (Spirit), and the Liver stores the Hun (Ethereal Soul). Both meridians govern Blood. When Blood is deficient, the Shen and Hun lose their tranquility, giving rise to fright palpitations (Jing Ji). The Classics state: "The color of the East is green, entering into communication with the Liver; its disease manifests as fright and terror." It also states: "Disease arising from Two Yangs and One Yin primarily presents as fright and terror." Fright (Jing) refers to the sudden beating of the Heart accompanied by terror; Palpitations (Ji) refer to the beating of the Heart accompanied by fluttering anxiety. Both conditions arise from Heart deficiency and Blood insufficiency, hence symptoms of forgetfulness follow. Use Si Wu Tang (Four Substances Decoction) combined with herbs that calm the Spirit. Danxi stated: There are also cases attributable to Phlegm, suitable for treating with Wendan Tang (Warm the Gallbladder Decoction) added with Chen Sha (Cinnabar) and Yuan Zhi (Polygala Root). If triggered by overthinking, it is a deficiency pattern, treat with Yang Xin Tang (Nourish the Heart Decoction). If intermittent, it is a Phlegm pattern, treat with Fu Ling Wan (Poria Pill). If easily startled by events, it is Heart and Gallbladder deficiency and timidity, treat with Wendan Tang (Warm the Gallbladder Decoction). If dreaming and prone to nightmares during sleep, it is Blood failing to return to its source, treat with Zhen Zhu Mu Wan (Pearl Shell Pill). If restless in dreams, it is the Hun of the Liver failing to maintain its post, treat with Ding Zhi Wan (Stabilize the Will Pill). If fearful and unable to stay alone, it is Gallbladder Qi deficiency and cold, treat with Fu Shen Tang (Poria Decoction). If restless in sleep, it is Gallbladder Qi excess and heat, treat with Suan Zao Ren Wan (Ziziphus Seed Pill). If dizzy with palpitations, it is internal production of Wind-Phlegm, treat with Chen Sha Yuan Zhi Wan (Cinnabar and Polygala Pill) from *Ben Shi Fang*. If thoughts are knotted due to depression, with Spleen deficiency and Qi stagnation, treat with Gui Pi Tang (Restore the Spleen Decoction). Although the aforementioned conditions are said to belong to the Heart and Liver, the governance of Blood actually lies with the Spleen. The emotion of the Spleen is Thought (Si). Excessive thought consumes Blood, preventing it from nourishing the Liver and Heart; this is caused by the Spleen. Internal movement of thought invariably exerts strain on the Heart. The Heart is the organ of Sovereign Fire and the master of the twelve officials. The virtue of a sovereign is authority without anger and governance through non-action. Therefore, it should be stabilized by tranquility and warned against reckless movement. Movement causes Ministerial Fire to converge and scorch Yin Essence. Once Essence and Blood are depleted, Fire flares up alone; thus arises the origin of fright palpitations (Jing Ji) and fluttering anxiety (Zheng Chong). The Fire of the Five Emotions is beyond the control of the Heart. Therefore, those who treat the Spleen must know how to nourish the Heart; those who nourish the Heart must know tranquility and reducing desires. Yet, who can be without thought? If thought is upright, there will be no desire for reckless movement. Zhu Xi said: "One must ensure that Dao Xin (The Way Mind) constantly serves as the master of the body, while Ren Xin (The Human Mind) obeys it." This is the method of nurturing the mind well.
Wendan Tang (Warm the Gallbladder Decoction) treats Heart and Gallbladder deficiency and timidity, being easily startled by events, or ominous dreams, leading to a startled heart and cowed gallbladder. Qi stagnation generates Phlegm; Phlegm and Qi contend, transforming into various syndromes. Or there is shortness of breath and fatigue, or recurrent spontaneous sweating. Gallbladder deficiency fails to control the Spleen, causing Spleen water-dampness to arise.
