(The rash resembles patterns on brocade, with empty spaces in between, resembling cloud shapes, is called "Sha" (fine rash); covering the whole body without gaps, varying in density, is called "Zhen" (measles-like rash).)
[Wan Quan] Zhen is also known as Mazu (smallpox/measles), caused by Jun Huo (Ministerial Fire/Heart Fire). (Some say it is caused by the Spleen.) The Classic says: When Shaoyin (Lesser Yin) qi arrives, it manifests as sores and rashes; in the human body, Heart Fire governs it. If Heart Fire becomes hyperactive and extreme, it controls what it overcomes (Kidney Water), burning and scorching Lung Metal. Since the Lung governs skin and hair, the Zhen toxin appears between the skin layers, resembling the marks of mosquito or flea bites. Smallpox lesions appearing in a uniform manner are favorable; if they erupt simultaneously with Zhen toxin, it is called Jia Zhen (Smallpox complicated with Measles/Rash). This condition is extremely malignant. Only when the smallpox base is sparse while complicated by rash can it potentially be treated. If the smallpox is originally dense and erupts together with rash, mixing indistinguishably and intricately, urgent treatment with acrid-cool herbs to induce sweating and resolve the exterior is required. If the Zhen toxin gradually subsides and the smallpox remains distinct and clear, it can also be treated. When Rash and Smallpox appear together, the toxin does not decrease; this is a critical and dangerous disease, who can predict survival? For smallpox complicated by rash, Jingfang Baidu San is the primary prescription; once the rash toxin dissipates, it can be cured. If rash reappears after the smallpox crusts have fallen off, this is a sign that residual toxins are dispersing, and no treatment is necessary.
[Xue Ji] Zhen is an external pathogenic factor of Wind Evil, while Smallpox is an internal eruption of fetal toxin. When both conditions occur together, the Zang-Fu organs are all affected. When the two mix, red halos appear, and the smallpox becomes more vigorous. Mistaking this for excessively dense smallpox often leads to untreatable cases. However, this is actually Smallpox complicated by Rash. It should be treated with Renshen Qianghuo San. Once the rash toxin resolves, the smallpox trend recedes. If Qi and Blood are deficient, preventing crusting, the urgent priority is to tonify the Spleen and Stomach. [Weng Zhongren] When Dan Zhen (red rash) erupts within smallpox, specific treatment is not needed; the focus should be on supporting the expulsion of smallpox toxins. Once the smallpox erupts, the rash will naturally subside.
