Wenkang cited Qian Yi (courtesy name Zhongyang) stating: In cases of slow convulsion following an illness, or due to stomach deficiency from vomiting and diarrhea, or qi weakness triggered by fright, the whites of the eyes appear like pale ink, and stools are greenish-yellow. This type of diarrhea requires warming and tonification, primarily treated with Zhi Sheng Bao Ming Dan (Supreme Holy Life-Preserving Pill) and Gouteng Yin (Uncaria Decoction). If milk passes down with the stool, Xiaoru Wan (Digest Milk Pill), Jinshi Wan (Advance Food Pill) should be used. If there is slight thirst, panting and irritability in the heart and spleen, along with fever and mania, this diarrhea is particularly difficult to treat; Chen Sha Wu Ling San (Cinnabar Five-Ingredient Poria Decoction) is indicated. For cold patterns, Dingming Yinzi (Life-Determining Infusion) is used, followed by Wenjing Zhu Jun San (Warming Fright Master's Powder) and Shuijing Taiyi Dan (Sleeping Fright Supreme One Pill) to calm the spirit and subdue fright.
Xue Ji stated: In pediatric fright-diarrhea, the Liver governs fright and belongs to Wood in the Five Elements. When Liver Qi is excessive, it inevitably overacts upon Spleen Earth. Once Spleen Earth is weakened, milk and food fail to transform, and water pathways become disordered, resulting in diarrhea with greenish stools, sometimes accompanied by twitching. Green is the color of the Liver, and twitching is a sign of Liver wind. There are also cases caused by the nursing mother suffering from Spleen deficiency and fright, or anger stirring up Liver fire. The Classic states: "Anger causes Qi to rebel; severely, it leads to hematemesis and undigested food diarrhea." The treatment method should focus on soothing the Liver and tonifying the Spleen. One must strictly avoid harsh purgatives, as this will further deplete Spleen Qi and exacerbate Liver pathogenic factors, potentially leading to convulsions and opisthotonos—these are adverse changes due to blazing Liver fire and depletion of the Middle Jiao. Whenever signs of fright are observed, one should immediately use formulas such as Sijunzi Tang (Four Gentlemen Decoction), Liujunzi Tang (Six Gentlemen Decoction), or Yigong San (Extraordinary Skill Powder), adding Baifuzi (Typhonii Rhizoma) to settle wind, and Chaihu (Bupleuri Radix) to soothe the Liver and guide the medicine, thereby preventing progression. This ensures no diarrhea or convulsions occur, and the child recovers. If diarrhea, vomiting, and fright-convulsions have already manifested, yet one fails to tonify the Spleen and soothe the Liver, instead using Baoming Dan, Baolong Wan, or Zhenjing Wan, the patient moves further away from life.
Zhi Sheng Bao Ming Dan: Treats fetal fright and internal drawing pain in infants, characterized by a tight and hard abdomen, restlessness during sleep, and frequent crying at night. It also treats acute and chronic convulsions with upward gazing of the eyes, limb jerking, and loss of consciousness. It is suitable for cold patterns.
Dingming Yinzi: Treats slow convulsions with severe vomiting and diarrhea, indicating potential transmission to slow spleen-wind. It is generally applicable.
Zhu Jun San: Treats the aforementioned symptoms after vomiting and diarrhea, especially when stools are green.
Shuijing Taiyi Dan: Regular use calms the spirit, subdues fright, stops night crying, and resolves greenish stools.
Sishen Wan: Treats Spleen deficiency and Stomach weakness, loose stools, lack of appetite, or dysentery with abdominal pain.
Ershen Wan: (Formula listed without specific indication in the provided text segment, typically associated with kidney-deficiency diarrhea).
