Wan Shi (Master Wan) believes: When smallpox first appears, there is a spot of blood at the root, forming only a small blood blister; during the eruption stage, it gradually enlarges, and the blood transforms into water, becoming a vesicle; when it turns to pustulation, it becomes a pustule, which signifies true maturity. At the time of maturity, it is required that every pimple suppurates, being plump, lustrous, full, with a red and vibrant base showing a pale wax color. In such cases, one can accurately calculate the days and predict the time of scabbing.
When smallpox first appears, if the center of the pimple is depressed, or the top is flat, or the root is white, as long as the patient can eat, or the treatment has not deviated from the correct method, then after eruption, the depressed ones will all rise, the flat ones will become pointed, and the white/pale ones will turn red and vibrant. The blood and water in the follicles have transformed into pus. Thus, the rising of depressions and pointing of flats indicates that eruption has been thorough; redness, vibrancy, and fullness indicate sufficient Qi and Blood; the transformation of water into pus indicates that the toxin has been resolved; the absence of itching and pain on the exterior, and vomiting and diarrhea on the interior, indicate that neither the exterior nor the interior is diseased. In such situations, one should simply wait for scabbing and must not recklessly administer decoctions.
For mild cases, pimples usually erupt in three or four batches, varying in size and timing; eruption also occurs in three or four batches, with earlier eruptions erupting first and later ones later, maintaining their initial sizes. During the nourishing-pus stage, those that matured first produce pus first, and those later matured produce pus later, similarly varying in size. They are distinct and do not adhere to each other, which is the best outcome. For dense cases, although they grow plump until pus formation, they inevitably interconnect. As long as depressions rise thoroughly, penetration is complete, and they transform into pus, this is considered secondary. Once pus forms, the toxic qi is resolved and no longer lingers. People often say that once smallpox reaches the pus-formation stage, the toxin is resolved, implying safety; however, one must not know that pus also has auspicious and inauspicious signs. If the skin is thick, pus is turbid, and the structure is intact without any damage, this is truly auspicious. If the skin is soft and thin, pus is clear and dilute, leaking and moist, and easily damaged, this remains inauspicious. Only in cases where the sore has matured late, the time for scabbing has passed, yet scabbing does not occur, causing pus to revert to water and skin to break easily, should one not apply the above theory.
At the initial stage of eruption, one should avoid wind and cold, stay away from others, regulate diet, and observe taboos. At the nourishing-pus stage, caution is even more critical than during eruption. Previously, the disease was recent, Qi and Blood were strong, sufficient to resist aberrant changes. Now, Qi is depleted, Blood is deficient, and spirit is diminished; slight aberrations cannot be borne. Moreover, since the sores are newly formed, they are easily offended by external pathogens, requiring utmost caution.
If the weather is extremely hot, remove clothing and bedding to keep cool, but guard doors, windows, and curtains to prevent evil wind from entering. If the weather is extremely cold, add thick covers to keep warm, and have family members hug from both sides. The room should be bright, with lights never extinguished. Frequently burn "Bi Hui Xiang" (Dispel Filth Incense) with frankincense to fill the air with fragrance. Day and night, one person should watch over the patient, rotating shifts to avoid fatigue. Fear that the child may itch; gently massage to prevent accidental scratching, which could cause suppuration and rotting, leading to unsightly scarring.
**Bi Hui Xiang (Dispel Filth Incense)**
* Cang Zhu (Rhizoma Atractylodis Macrocephalae, 1 jin)
* Da Huang (Radix et Rhizoma Rhei, 0.5 jin)
Crush finely. Mold into balls and burn in a stove continuously without interruption.
**[Failure to Form Pus]** When smallpox is about to finish erupting and wants to penetrate, reaching the nourishing-pus stage, it must form pus. Fullness indicates pus has formed. Turbidity is the shape of pus. Yellow-white is the color of pus. If at the time of supposed pus formation, it remains an empty shell, this means Qi carries the toxin, but Blood does not adhere to Qi. Toxin resides in Blood; since Blood does not arrive, toxin remains trapped in Blood and fails to emerge. Use **Si Wu Tang** (Four Substances Decoction [for blood loss]) combined with **Zi Cao Yin** (Arnebiae Decoction [for manifestation stage]) plus Chan Tui (Cicada Slough) as the main formula. If it has already turned to water, clear and grayish-white, unable to form pus, this indicates deficiency of both Qi and Blood. The remaining water is merely the initial drop of blood, dispersed by Qi into water, not water rising from within. Use **Shi Quan Da Bu Tang** (Comprehensive Great Tonifying Decoction [for sweating]) as the main formula. These two conditions regarding itching and carbuncle-toxin must be understood. After initial eruption, if pus should form but does not, discuss this under four syndromes similar to failure to erupt and black depression. If affected by wind-cold, use warming dispersion: **Gui Zhi Ge Gen Tang** (Cinnamon Twig and Pueraria Decoction [for initial fever]) plus Huang Qi (Astragali Radix), Bai Zhi (Angelicae Dahuricae Radix), Fang Feng (Saposhnikoviae Radix). If toxic qi is abundant, use interior-supporting, detoxifying, and diuretic methods: alternate **Zi Cao Yin Zi** (Arnebiae Decoction [for eruption]) and Lian Qiao Tang (Forsythiae Decoction [for abdominal fullness]). For constipation, use Xuan Feng San (Wind-Releasing Powder [for fever]). For internal deficiency, use warming interior methods: **Shi Quan Da Bu Tang** or Chen Shi Mu Xiang San (Chen's Costus Powder [for diarrhea]). For external offense, use fumigation and resolution: internally take Zi Cao Yin, externally use Yin Chen Fumigation Method (Artemisiae Scopariae Fumigation [for eruption]). For restlessness and drowsiness, use Long Nao Gao (Borneol Ointment [for black depression]). Master Xue says: If grayish-white, or itchy with non-penetrating pus, use Zi Cao (Arnebiae Radix), Si Jun Zi Tang (Four Gentlemen Decoction), and Mu Xiang (Aucklandiae Radix). If red, or itchy with non-penetrating pus, use Zi Cao Mu Tong Tang (Arnebiae and Cystander Decoction [for slow eruption]). If penetrating but clear/thin, use Shen Nei Tuo San (Ginseng Interior-Supporting Powder [for itching]). If ineffective, add Fu Zi (Aconiti Lateralis Radix Praeparata); delay leads to death. *Mi Yao* states: Depressed top without pus is adverse; urgently use Bao Yuan Tang (Protect the Origin Decoction) plus Chuan Xiong (Ligustici Chuanxiong Rhizoma), Rou Gui (Cinnamomi Cortex), Dang Gui (Angelicae Sinensis Radix), Shao Yao (Paeoniae Radix), Mu Xiang, Nu Mi (Oryza Sativa Germ), boil well, and take warm with Ren Ru (Human Placenta) and good wine. White like crystal, without internal pus, use Bao Yuan Tang plus Nu Mi, taken with human placenta and good wine.
Note: Patients with inherent weak Yuan Qi. Or taking too many detoxifying herbs due to density during eruption. Or experiencing vomiting/diarrhea during eruption. All damage Yuan Qi. Although using Ginseng and Astragalus to assist pus formation, fear remains of weak Yuan Qi resulting only in clear pus, lack of fullness, and risks of itching/carbuncles. It is advisable to add high-quality Lu Rong (Cervi Cornu Pantotrichum) to the decoction, swallow Zi He Che Wan (Placenta Pills), and continue Ba Wei Er Hua San (Eight Ingredients Two Flowers Powder). I have seen cases where smallpox erupted distinctly and penetrated well, but failed to form pus at the scabbing stage, often turning into disastrous conditions. This occurs due to inability to eat, spleen-stomach deficiency with self-induced diarrhea and lack of regulation, or uneven eruption, incomplete penetration, and trapped toxin mistakenly deemed sparse. These are failures of care, not caused by seasonal toxins.
**Liu Yi Tang (Six-One Decoction)** Specifically promotes smallpox pus.
* Huang Qi (Astragali Radix, 6 qian)
* Gan Cao (Licorice Root, honey-fried, 1 qian)
Crush. Take 2 qian per dose, decoct with 6 parts water and 2 parts wine until half a cup remains. Take warm. Adding Gan Lan (Canarium Album Fruit) is better. Shan Yao (Dioscoreae Rhizoma) is also acceptable.
**Zhu Jiang Wan (Pus-Assisting Pill)** For smallpox on day 7-8 with thin/non-coming pus. Urgent use.
* Huang Qi (Astragali Radix, honey-fried, 3 liang)
* Bai Shao Yao (Paeoniae Radix Alba, wine-fried)
* Dang Gui (Angelicae Sinensis Radix, wine-washed. Each 1.5 liang)
* Lu Rong (Cervi Cornu Pantotrichum, fresh/moist/amber-colored, processed like deer horn glue with milk)

* Gan Cao (Licorice Root, 3 fen)
* Yin Yang Huo (Epimedii Folium), Chuan Shan Jia (Manis Squama, earth-fried yellow), Mu Xiang (Aucklandiae Radix). Each 2 fen)
* Rou Gui (Cinnamomi Cortex, 3 li. This is a guiding herb; excessive amount causes itching)
Add 1 slice of Sheng Jiang (Zingiberis Rhizoma Recens), 1 Zao (Jujubae Fructus). Decoct in water and take. Or grind to powder. For vomiting, take with Sheng Jiang Tang. For diarrhea, take with Mi Yin (Rice Drink). For abdominal pain, take with Shen Qu (Massa Medicata Fermentata) decoction. For restlessness, take with Mai Men Dong Tang (Ophiopogonis Decoction). For thirst, take with Mai Men Dong (Ophiopogonis Radix) and Wu Wei Zi (Schisandrae Fructus) decoction. For vomiting/diarrhea, take with Huo Xiang (Pogostemonis Herba) and Chen Pi (Citri Reticulatae Pericarpium) decoction. For smallpox failing to form pus, take several doses more; no harm.
**Bu Jiang Tang (Nourish Pus Decoction)** For gray-white smallpox lacking vigor, or clear pus.
* Yin Yang Huo (Epimedii Folium, 3 fen. Excessive causes itching)
* Ren Shen (Ginseng Radix et Rhizoma), Dang Gui (Angelicae Sinensis Radix), Shan Zha (Crataegi Fructus). Each 8 fen)

* Sun Jian (Bambusae Caulis Acumen, 3 pieces)
Add Sheng Jiang, Zao, Nu Mi. Decoct in water and take. Another prescription includes Bai Zhi and Fang Feng.
**Formula for White Smallpox with Chills** On days 6-7 of smallpox, just as pus should coalesce, suddenly appearing white and pallid, lacking red/vibrant beauty, showing earthy iron depression, with unrelenting heat and chills. Take **Tian Dan Huo Niu Tang** (Tian Dan Fire Ox Decoction).
* Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix), Liao Zi (Polygoni Multiflori Folium - select fine leaves; if Chuan Liao, leaves are large with green spots, distinguish carefully. Stir-fry with Chuan Shan Jia until shell odor dissipates, remove shell).
* Dang Gui (Angelicae Sinensis Radix). Each 2 qian)
* Fu Zi (Aconiti Lateralis Radix Praeparata, 1 qian)
* Gan Cao (Licorice Root, 5 fen)
* Gui (Cinnamomi Cortex, 3 fen)
* Ju Hong (Citri Reticulatae Pericarpium Rubrum, 8 fen)
Decoct in water and take.
**[Spleen-Stomach]** When smallpox has grown and pus is about to form, focus primarily on Spleen-Stomach. Strong Spleen-Stomach means sufficient Qi and Blood, naturally facilitating easy pus formation, plump and thick, requiring no medicine. Weak Spleen-Stomach means deficient Qi and Blood, unable to circulate throughout the body to form pus, resulting in soft/clear pus (essentially water). Use **Shi Quan Da Bu Tang** (Comprehensive Great Tonifying Decoction) removing Di Huang (Rehmanniae Radix), adding Mu Xiang. Or **Ren Shen Bai Zhu San** (Ginseng and Atractylodes Powder) removing Ge Gen (Puerariae Radix), adding Huang Qi and Guan Gui. Take frequently for best results. Assess Spleen-Stomach strength by appetite and stool consistency. Low appetite with hard stool indicates sufficient Spleen-Stomach Qi. Diarrhea indicates further deficiency. Use **Si Jun Zi Tang** (Four Gentlemen Decoction) to deliver Dou Kou Wan (Cardamom Pill [for diarrhea]) until diarrhea stops, then resume Ren Shen Bai Zhu San minus Ge Gen, plus Huang Qi and Guan Gui. Clear stool with ability to eat follows the above. Inability to eat follows the above. If able to eat but hard stool persists for days, use Dan Dao Fa (Gallbladder Suppository Method [for severe constipation]) to unblock, allowing Qi to flow freely, Ying and Wei to harmonize, preventing rotting.
**[Diarrhea]** Smallpox eruption and development stages do not tolerate diarrhea. Fear interior Qi deficiency, toxin fails to exit, leading to depression. Thus, diarrhea is an inauspicious sign. Especially during pus formation, diarrhea is worse than before. Previously, disease was recent, Spleen-Stomach strong enough to bear it. Now, disease is prolonged, fluids depleted, Spleen-Stomach weak. Further diarrhea exhausts remaining Qi internally, while expanding toxin fails to manifest externally. May result in itching, inverted scabbing, chattering teeth, exhaustion, and death. Mild cases: Ren Shen Bai Zhu San minus Ge Gen, add Mu Xiang, Guan Gui, Huang Qi. Severe cases: Chen Shi Mu Xiang San, Yi Gong San (Heterogeneous Attack Powder [for slow eruption]), Rou Dou Kou Wan (Myristica Pill [for diarrhea]) can be used together. If ineffective, urgently take Bu Er San (Two-No Powder [eruption stage]).
**[Constipation and Cold Extremities]** Warm hands and feet are precious in smallpox. During pus nourishment, hot hands/feet with sweat indicate toxic heat stagnating inside, likely with urinary/fecal retention, deep/slippery/rapid pulse. Promote urination/stool: **San Yi Cheng Qi Tang** (Three-Eight Purging Decoction) removing Mang Xiao (Natrii Sulfas). Cold extremities indicate impending Yang collapse, likely with continuous white diarrhea or vomiting, deep/fine/weak pulse or floating/large/empty. Urgently warm: **Li Zhong Tang** (Regulate the Middle Decoction) plus Shu Fu Zi (Prepared Aconite), or Chen Shi Yi Gong San. Survival if hands/feet warm after medication; death if cold. If constipation, urinary retention, restlessness/wandering, abdominal distension/pain, panting/thirst, deep/slippery/rapid pulse, and non-rising sores, this is depression syndrome, Yang Jue (Yang Reversal): **Bai Xiang Wan** (Hundred Auspicious Pills [for black depression]) and San Yi Cheng Qi Tang.
**[Restlessness]** Calmness is prized throughout smallpox. When pus forms, toxin resolves, organs harmonize, spirit refreshes; calmness is especially suitable. Sudden restlessness/insomnia: examine sores. If pus is clear/incomplete, toxin remains inside, not fully out: **Long Nao Gao** (Borneol Ointment [for black depression]). If pus formed/full, restlessness due to fever drying pus: promote urination, **Dao Chi San** (Direct to Heart Powder [Heart Heat]). If sores too dense, post-pus formation, Heart Blood deficiency causing empty restlessness: **Si Wu Tang** (Four Substances Decoction) remove Chuan Xiong, add Ren Shen, Mai Men Dong, Zhi Zi Ren (Gardeniae Semen). Also, **Suan Zao Ren Tang** (Ziziphus Decoction [for restlessness]).
**[Throat Issues, Vomiting, Hiccups]** Dense smallpox affects throat. During pus formation, throat sores mature early, flesh thin/skin delicate, prone to damage. New scars tender, painful to touch. Scabs sticky, saliva entangled, causing blockage, difficulty eating/swallowing. Pain and saliva separation cause choking on water, vomiting on food. If speech clear, treatable: **Gan Jie Tang** (Licorice and Catalpa Decoction [throat]) add Niu Bang Zi (Arctii Fructus), Tian Hua Fen (Trichosanthis Radix) to benefit throat/chest, resolve phlegm. Drink plenty, naturally heals. If hoarse/unable to speak, throat ulcerated, untreatable.
**[Itching]** Tender/bright smallpox often itches at pus formation. Regulate first to prevent. If missed and itches: internally take Xiao Feng Hua Du Tang (Eliminate Wind Resolve Toxin Decoction [itching]), externally use Yin Chen Fumigation Method. If broken, apply Bai Long San (White Dragon Powder [itching]). Generally, itching is ominous, mostly bad. If patient eats/hard stool, scratched areas refill with pus, new sores appear elsewhere, varying sizes, even if all scratch/break, treatable: internally alternate **Shi Quan Da Bu Tang** and Ku Shen Wan (Sophora Pill [smallpox leprosy]), externally apply Mie Ban Jiu Ku San (Scar-Eradicate Save Bitterness Powder [smallpox leprosy]) mixed with Bai Hua Gao (Hundred Flowers Ointment [scabbing]). If itching causes confusion, scratched areas fail to swell/refill, form pits/dry black/shed skin, plus choking/vomiting/no intake, diarrhea, chattering, voice loss, cold extremities, screaming: all death signs.
**[Head/Face]** Examine smallpox frontally, as Five Organs essence gathers in face. Body sores itching/scratching less harmful. Frontal sores must not be touched. Scratching eyebrow/nose/face indicates Lung Heat: urgently use **Gan Jie Tang** add Niu Bang Zi to resolve, stopping itch (auspicious). If itch persists/spreads, Qi leaks, itch intensifies, fatal facial scratching. During eruption/pus nourishment, forehead sores look like scalded/boiled, ulcerated, no intact skin. Cheeks similarly, tearing without scratching, sand-collapse momentum unstoppable, shell scorched/water gone, resembling scab but not, Yang collapse death. Early pus formation, face sores not fully suppurated. Nose tip dry orange, glabella dry black, lips charred black, ears retracting, cheeks flat/depressed center: named "Inverted Depression," death sign, do not mistake for normal scabbing.
**[Hands/Feet]** Hand/foot smallpox requires full pus, indicating strong Spleen-Stomach/sufficient Qi-Blood. Gray-white/clear water/depressed: weak Spleen-Stomach. **Kuai Ban Yue Bi Tang** (Quick Rash Yuebi Decoction [eruption stage]). Even if scabbing occurs, later abscesses will develop at wrists/knees/joints.
**[Shoulders/Back/Buttocks]** Initial eruption: pathogenic Qi strong, Righteous Qi strong, capable of bearing. Pus formation: Qi/Blood deplete, spirit weak,不胜 (unable to bear), difficult movement, preferring supine position. Shoulder/back/buttock sores suffer most from friction. Good sores naturally thick/durable. Secondary: delayed scabbing, spontaneous rupture when ripe. Worst: scald/fire-like injury, water peeling skin. Spontaneous rupture, clear water not pus, black/dry/scorched: untreatable.
**[Leaking Pus]** Verse: Just testing pus, sac not full, sore head has hole leaking pus. Gather to seal hole, leaking true fluid hides toxin. When forming pus follicles, thick skin wrapping firmly is crucial. If pus not formed, sudden hole in sore head leaks water, or clusters outside, or water gone/hole empty/dries black: vulgarly "Leaking Sore," fatal. If pus ripe, follicle skin ripe, pus boils out, scabbing forms: common on forehead/front face, vulgarly "Pile Poop Scab." Do not equate with Leaking Sore. Leaking Sore: pus unformed. Pile Poop Scab: pus over-ripe.
**[Throat Discomfort, Thick Saliva]** At pus follicle stage, expectoration of thick sticky phlegm/pus, sometimes blood-tinged, throat discomfort, poor appetite. Lung affected by Fire Evil, insufficient fluids, sticky phlegm. Ulcerated bleeding between throat/tongue/teeth. Use **Gan Jie Tang** add Niu Bang Zi, Tian Hua Fen to clear Lung/resolve phlegm/benefit throat. Wait until scabbing for natural harmony. Do not recklessly use major cooling agents.
**[Sleep Talking/Mumbling]** Pus in smallpox formed by blood steaming. Dense sores, widespread pus/blood, fluid consumption. Heart governs Blood; Blood deficiency leaves residence empty. Heart Heat causes empty restlessness/insomnia: **Suan Zao Ren Tang**. Heart Deficiency causes sleep talking/mumbling, strange stories, unrousable: **An Shen Wan** (Calming Spirit Pill [delirium]). Severe drowsiness: first open heart orifices with **Long Nao Gao**, then An Shen Wan with Ginseng/Mai Men Dong Decoction.
**[Abdominal Pain]** Initial eruption pain: toxin inside. Incomplete eruption pain: depression. Pus formation: toxin out, no depression. Sudden pain with constipation: dry feces. Use Dang Gui Wan, Dan Dao Fa to unblock. Clear stool: cold invasion. Urgently give Li Zhong Tang plus Gui (Cinnamon Twig) or Huang Qi Jian Zhong Tang (Astragalus Establish Middle Decoction) plus Mu Xiang. Smallpox fully erupted/penetrated/suppurated, no exterior evil. Eating/clear urine/moist stool, no interior evil. Peaceful then sudden distension/pain/restlessness/panting, ash-gray sores: food stagnation. First Ding Xiang Pi Ji Wan (Clove Spleen Stagnation Pill [retained food]) with vomit residue decoction to expel stagnation. Then Ren Shen Bai Zhu San minus Ge Gen, add Qing/Chen Ju Pi (Green/Aged Tangerine Peel), alternate with Yang Pi Wan (Nourish Spleen Pill [retained food]).
