Detailed examination of the entry "Tian Zhu Dao" (Collapse of the Heavenly Pillar) in the Kidney section.
"Han Dong Wang Shi Jia Bao" treats pediatric Gan syndrome (chronic malnutrition syndrome) where Qi invades the yin region, presenting with a bright yellow luster.
Wu Jin Gao (Black Gold Ointment)
Tongcao (Tetrapanacis Medulla), Huangpi (Cortex Phellodendri), Dahuang (Rhei Radix et Rhizoma) (each 2.5 qian, calcined).
Calcine each herb to preserve its properties, grind into fine powder. Use 1 qian each time, mix with pig bile to form an ointment, and apply to the yin region. If symptoms do not subside, wash with Shechuangzi Tang (Cnidium Decoction) before reapplying the medicated paste.
Insufficient maternal Qi and Blood lead to extreme emaciation. Chao Yuanfang discussed pediatric emaciation, noting that failure to develop flesh and muscle is due to disharmony of the Spleen and Stomach, inability to digest food, resulting in weakened Qi and Blood that fail to nourish the skin and muscles. Cases complicated by Heat present as Wen Zhuang (low-grade fever), body heat, and slightly yellowish muscles. Cases complicated by Cold present immediately with diarrhea, bluish-white lips and mouth. After suffering from major illnesses, or convulsions (Jing Xian), or Shang Han (Cold Damage), or Wen Zhuang, and after medication induces vomiting, diarrhea, or sweating, the Qi and Blood remain deficient post-recovery. The Spleen and Stomach remain weak, unable to transform grain Qi to nourish the body, leading to Xu Lei (deficiency emaciation). Qian Yi stated that for Xu Lei with Cold, Muxiang Wan (Aucklandia Pill) is the primary treatment; it should not be taken in summer, but if indicated, take a small dose. For Heat, Huhuanglian Wan (Coptis Pill) is the primary treatment; it should not be taken in winter, but if indicated, take a small dose (Xu Lei and Gan are treated similarly. See Gan entries for Muxiang Wan and Huhuanglian Wan.) Xue Ji said:
One must further examine the complexion and morphology, and observe the manifestations. Red face with frequent crying indicates Heart Xu Lei. Blue face with squinting eyes indicates Liver Xu Lei. Tubercles behind or in front of the ears or below the ears indicate deficiency Fire of the Liver Meridian. Tubercles within the neck muscles indicate deficiency Heat from food stagnation. Yellow face with fullness indicates Spleen Xu Lei. Pale face with asthma indicates Lung Xu Lei. Excessive whiteness of the eyeballs indicates Kidney Xu Lei. One must still assess the mutual overcoming relationships (Five Elements) to prescribe medicine. Furthermore, one must distinguish between Cold and Heat syndromes. Abdominal pain with clear/white diarrhea, no thirst, preference for warmth indicates a Cold syndrome; even in summer, Muxiang Wan is appropriate. Body heat with irritability, dark yellow diarrhea, thirst with preference for cold drinks indicates a Heat syndrome; even in winter, Huhuanglian Wan is appropriate. These are treatments that set aside the season to follow the syndrome. Qian Yi, discussing medication and syndrome differentiation, said: A gentleman named Qi from Zheng, who liked to collect herbal powders to distribute to others, had a son whose internal organs suddenly became hot. Qi took Qingjin Gao (Blue Gold Ointment) himself, administering three doses at once. After finishing, by the third watch, the son had five bowel movements. Because the son was sleeping, Qi said that excessive sleep can also cause fright, and gave another dose of Qingjin Gao, resulting in two more bowel movements. Dry mouth and body heat followed. Qi said there was still residual mild heat and gave more Qingjin Gao. His wife said, "After more than ten doses, he is not at ease; has he fallen ill?" They summoned Qian Yi, who said, "He has developed Xu Lei. First, decoct Baizhu San (Atractylodes Macrocephala Powder) frequently. Later, use Xigua Wan (Melon Pill). He will recover in thirteen days." (See Thirst entry for Baizhu San, see Sweating entry for Mugua Wan.) Xue Ji treated a thirteen-year-old boy with a red face, fright palpitations, and fever, emaciated body, occasional pale face, belching and flatulence, frequent food stagnation. He had taken Baohe Wan (Harmonize the Stomach Pill) and heat-clearing medicines. I said: Red face and fright palpitations indicate timidity of the Heart Spirit. Pale face and belching indicate deficiency of Heart Fire. Flatulence indicates deficiency of Spleen Qi. This is a critical syndrome of congenital Heart Fire deficiency failing to generate Spleen Earth; the previous medicines were contraindicated. Disbelieving this, he took Zhizhu Wan (Immature Bitter Orange and Atractylodes Pill) and calming fright medicines, causing all symptoms to worsen: frequent stools, heavy sensation in the lower abdomen with rectal prolapse, decreasing phlegm-saliva and food intake. I first used Liujunzi Tang (Six Gentlemen Decoction) as the main formula, assisted by Buxin Wan (Tonify the Heart Pill). After over a month, food intake increased slightly, phlegm-saliva decreased slightly. Then I used Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) administered with Sishen Wan (Four Spirits Pill), and he recovered. After marriage, the illness relapsed with prolapse. It was mid-winter; his face was pale or sallow-dark, hands and feet were cold, he preferred eating pepper and ginger, felt no heat in the abdomen, pulse was floating but faint and thin upon pressure, and both Chi pulses were very faint. I then used Bawei Wan (Eight-Ingredient Pill). His Yuan Qi recovered and his physical Qi gradually filled out. By age twenty, he suffered from aversion to wind and cold, red face and eyes, fever with expectoration, red and cracked lips and tongue. Eating pepper or ginger caused his lips and mouth to break, and phlegm-heat worsened, yet his abdomen felt no heat. His pulse was either imperceptible or nearly absent. This is Cold forcing Yang outward, true Cold inside and false Heat outside. Using Bawei Wan again, all symptoms instantly resolved. An eight-year-old boy had a constantly blue face, sometimes changing color, squinting eyes during the day, and grinding teeth at night, lasting over two years. Taking Liver-clearing and Fire-descending medicines made it worse, and his physical Qi continued to emaciate. Seeking treatment from me, I said: The Liver governs the five colors; entering the Heart makes it red, entering itself makes it blue. The Liver belongs to Wood and generates Wind, so Liver Qi is Yang/Fire, and Liver Blood is Yin/Water. This is a deficiency of congenital Liver-Kidney Essence and Blood, causing deficiency Fire to move internally. Yin Blood becomes increasingly deficient, deficiency generates Wind, Wind arises from Fire, thus manifesting as red face and squinting eyes, etc., which are not external Wind. I subsequently used Dihuang Wan (Rehmannia Pill) to nourish Kidney Water and generate Liver Wood. In two months, squinting and teeth-grinding ceased. After about three months, all symptoms gradually resolved and Yuan Qi was replenished. For all Liver Meridian syndromes, if Liver Wood excess Heat generates Wind causing self-disease, or Lung Metal excess Heat overcomes Wood, one should use Liver-clearing and Fire-descending agents to drain the pathogenic Qi. For Liver Meridian Wind-Heat causing straight eyes, etc., use Chaihu Zhizi San (Bupleurum and Gardenia Powder) to clear Liver Fire, and Jiawei Siwu Tang (Modified Four Substances Decoction) to nourish Liver Blood. For Kidney deficiency causing teeth-grinding, etc., use Liujunzi Tang to strengthen Spleen Earth, and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to nourish Kidney Water, and recovery will occur. A boy with weak Spleen Qi and food stagnation presented with fever and thirst. Taking Xiehuang San (Drain the Yellow Powder) caused frequent diarrhea. I first used Baohe Wan for two doses, and he recovered, but he still had anorexia and nausea, blue face, and cold hands. I then used six Junzi, Chaihu, and Shengma for four doses. His complexion became sallow-yellow, he ate better and his hands warmed, but his body remained extremely emaciated, fatigued, febrile, with heavy lower abdomen and anal prolapse. I used Buzhong Yiqi Tang plus Banxia (Pinellia) and Roudoukou (Nutmeg) for two doses, and he stabilized. For all Spleen-Stomach syndromes, if fever and thirst, preference for cold food/drink, or yellow diarrhea with eyes closed during sleep occur, it belongs to Excess of Form and Disease; use Xiehuang San to guide and drain. If fever with dry mouth, aversion to cold, or white diarrhea with eyes open during sleep occur, it belongs to Deficiency of Form and Disease; use Yigong San (Extraordinary Efficacy Powder) to regulate and tonify. For Spleen Qi sinking, use Buzhong Yiqi Tang. For Cold Water insulting Earth, use Yihuang San. For Liver Wood insulting Spleen, use Liujunzi Tang plus Chaihu. For slight eye movement, tidal fever, convulsions, vomiting/diarrhea with anorexia, use Mizhi Baopi Tang (Secret Instruction Spleen-Preserving Decoction). For all pediatric diseases, one must first regulate and tonify the Spleen and Stomach to solidify the root; then all diseases will recede naturally. Medicine alone cannot eliminate them all. A five-year-old boy with deficient Qi and emaciation ground his teeth in sleep, had nocturnal enuresis, and frequent daytime urination. I considered this insufficient congenital Kidney Qi, and used Buzhong Yiqi Tang plus Buguzhi (Psoralea), Dihuang Wan plus Lurong (Deer Antler) to tonify Spleen and Kidney, resulting in recovery. After marriage, he had frequent urination, thirst, and fever, worsening in the afternoon. He strictly took Huangbai (Phellodendron) and Zhimu (Anemarrhena) to nourish Yin and descend Fire, later developing Kidney Wei (impotence/atrophy), bedridden for over a year. As I went north for an evaluation, I continued using the previous medicines. Fortunately, he was cautious with his health, and he recovered in half a year. An eleven-year-old boy had a pale or red face and weak legs unable to walk for long periods. I used Dihuang Wan plus Lurong. After about a year, he improved. After marriage, he had photophobia and weak legs. I used the previous pill and Buzhong Yiqi Tang, and he recovered. Later, the illness relapsed with increased thirst and foot heat, sensation of opening fontanelle, and macropsia (objects appearing larger). This was due to Kidney deficiency causing pupil dilation. Taking the previous medicines and avoiding sexual activity would have led to recovery. Due to lack of self-restraint, he ultimately died of Kidney Wei. Mr. Zhongyang (Qian Yi) said this syndrome belongs to insufficiency of brain marrow, failing to nourish. Dihuang Wan should be used to tonify. Some heal spontaneously by age seven or eight, or fourteen or fifteen, when Qi and Blood become abundant. If one indulges in lust and damages True Yin, one cannot live out one's natural lifespan. A thin boy with occasional cough took a dose of Rensu Yin (Ginseng and Perilla Drink) on his own, leading to increased wheezing, urgency, and convulsions. His face was pale or yellow. I said this is insufficient congenital Spleen-Lung Qi and deficient physical Qi. Because the previous prescription was harsh and draining, although the external pathogen was removed, Lung Qi became more deficient. When Lung is deficient, one should tonify the Spleen. I first used Yigong San plus Jiegeng (Platycodon) and Deng Teng Gou (Uncaria) for one dose, and phlegm-wheezing settled instantly. Then I removed Jiegeng, added Banxia and Danggui. After two doses, convulsions ceased. Adding Suanzaoren (Ziziphus Seed) brought peace. At age fifteen, he had fever, profuse phlegm, thirst, red face, and emaciation. I used Dihuang Wan plus Wuweizi (Schisandra) and Buzhong Yiqi Tang, each for over a hundred doses, and his physical Qi gradually strengthened. If one mistakes this for Yin Fire and uses Huangbai and Zhimu, damaging the source of transformation and generation, it becomes incurable. A five-year-old boy still drank breast milk. Pushing along the sides of the ears, neck, to the supraclavicular fossa revealed tendon knots and small nodules like beads hidden in the muscles. Urination was irregular, complexion greenish-yellow, and physical Qi emaciated. This was caused by inheriting the mother's Liver Fire. I used Jiuwei Lujian Wan (Nine-Ingredient Aloe Pill) and Wuyi Yigong San (Five-Ingredient Extraordinary Efficacy Powder) plus Zhizi (Gardenia) and Chaihu (Bupleurum) for the child, and Jiawei Xiaoyao San (Modified Free Wanderer Powder) for the mother. He soon recovered. A boy with Xu Lei had foul-smelling discharge from the ears. The Chi and Guan positions on the left hand were surging and rapid but powerless. I cleared the Liver and tonified the Kidney. Although the ear condition healed, the pulse did not fully收敛 (gather). After marriage, he developed Zhai Syndrome (consumption/TB-like disease). Mistakenly taking Huangbai and Zhimu damaged Yuan Qi further. Unable to withstand cold, heat, fatigue, and labor, he was ill every day, nearly dying. I greatly tonified the Spleen and Kidney and nourished Yuan Qi, and he recovered. A boy had symptoms as above: emaciated limbs, sallow-yellow complexion, sour-smelling stool. This was Spleen deficiency with food stagnation. I used Simei Fei'er Wan (Four-Ingredient Fattening Child Pill) and Wuyi Yigong San, and he recovered. A nine-year-old boy had acid regurgitation, aversion to food, emaciated body, and abdominal pain. I said this is food stagnation causing Xu Lei. I used Baohe Wan, and he recovered. Later, frequent abdominal pain occurred, all treated with Baohe Wan. I said: This is because the Spleen and Stomach are deficient and injured by diet. One should regulate and tonify Spleen Earth to prevent future issues. Disbelieving this, later abdominal pain relieved by pressure occurred. I used Wuyi Yigong San for two doses. As there was no response, he used Pingwei San (Calm the Stomach Powder) on his own, causing abdominal distension and pain. I still used Yigong San plus Muxiang (Aucklandia) for four doses, and he recovered. If one repeatedly uses attacking/draining agents, Yin is damaged and Yuan Qi is harmed, often leading to Xu Lei. Be careful.
