Pediatric patients presenting with high fever, vomiting, restlessness of the heart and mind (Shen), convulsions of the hands and feet, rigidity of the body, and retroversion of the eyes suffer from Fetal Convulsion Wind Syndrome (Tai Jing Feng). "Fetal Convulsion" arises because the pregnant woman failed to maintain proper regimen during gestation, such as consuming alcohol, indulging in sexual desires, experiencing anger, or suffering from fright and physical impact. When the mother is stimulated, the fetus inevitably responds; alternatively, external pathogenic wind may injure the fetus. Consequently, the child inherits the maternal qi and becomes ill immediately after birth. The clinical manifestations include: within the first month of life, high fever, upward rolling of the eyes, clenched fists, trismus (lockjaw), rigidity of the back and waist, excessive salivation with vomiting, convulsive jerking and frightened crying, retraction of the cheeks, and open fontanelles. Alternatively, the cheeks may be red, or the face blue with closed eyes. (Note: Cases of fetal wind causing eye closure must not be mistaken for Slow Spleen Wind [Man Pi], and warm tonifying medicines should not be used recklessly.) Conditions such as upward deviation of the eyes and trismus also stem from this etiology. Observing the color of the skin between the eyebrows: a bright red mixed with greenish hue indicates a treatable condition, whereas a dark cyan-black hue indicates an untreatable one. A fingerprint on the tiger's mouth area curving inward suggests treatability, while one extending outward suggests untreatability. Initial treatment should focus on dispersing wind pathogens, relieving convulsions, resolving phlegm, and regulating qi, along with applying external paste to the fontanelle. For severe cases, Zhu Yin Wan (Cinnabar-Silver Pill) may be used to purge toxins. If the face is blue and fists are clenched with convulsions, use Bao Ming Dan (Life-Preserving Pill) (indicated for both acute and chronic convulsions), Gouteng San (Uncaria Powder) (see Chronic Convulsion section), or Quanxie San (Scorpion Powder) (see Hemiplegia and Trismus section). Generally, pediatric zang-fu organs are fragile and delicate; preparations containing mercury powder (Yin Fen) that heavily settle and suppress should not be used routinely. In such emergencies, take pig milk, finely grind small amounts of Niuhuang (Calculus Bovis) and Shexiang (Moschus), mix them, and apply to the mouth. Simultaneously administer Daochi San (Guide Out the Red Powder) to drain the son of the Liver (i.e., Heart fire), which will lead to recovery.
The *Shibi Jing* song states: Within the first month of gestation, maternal fright causes illness manifesting at birth. Rigidity of the waist precedes lip puckering during crying; a blue face, clenched fists, and retracted cheeks indicate severity. Eye closure and trismus suggest tense meridians. (At conception, if Yin qi is weak, the fetus is prone to fright and miscarriage, leading to severe muscle wasting; if Yang qi is weak, the fetus resists fright but remains small with thin muscles, often failing to reach full lifespan. If term pregnancy ends in fright-induced lip puckering, swollen cheeks, nasal congestion, and trismus, do not treat as convulsion initially; induce slight sweating first, then treat convulsion and regulate qi. The nursing mother should take qi-regulating herbs, and the infant should have fontanelle paste applied.) Even if called a thousand times, the eyes remain unopened. Treatment focuses on retreating wind-phlegm; once phlegm is gone, wind-pathogen and fright naturally recede. (If neglected, the patient falls into coma. Treating fright first may cause vomiting and diarrhea; in autumn and summer, this leads to Spleen Wind. At initial onset, do not diagnose as Spleen Wind, lest failure to sweat produce other complications. Use Fengsui Wuxi Wan, followed by Yin Wan; these belong to the Acute and Chronic Convulsion categories.)
Qian Yi stated: Convulsions occurring during the day are true cases; if they occur more than two or three times, death is inevitable. False cases involve frequent attacks but are less severe. True fetal convulsion stems from internal generation of fright-epilepsy, while false cases result from external injury by wind-cold. Because blood and qi are not yet solid, the body cannot withstand stimulation, leading to convulsions. To identify false cases: if the breath exhaled from the mouth is hot, it can be treated by dispersing; Daqing Gao (Great Blue Ointment) is the primary formula, and fontanelle application is also applicable.
Tian stated: For convulsions occurring within the first month of life, urgently take pig milk, finely grind small amounts of Chen Sha (Cinnabar) and Niuhuang (Calculus Bovis), mix, and apply to the mouth for miraculous effect. The nursing mother should take three doses of Fangfeng Tongsheng San (Saposhnikovia Decoction for Unblocking the Heroic), and the convulsions will subside naturally.
Dou stated: For infants under one month old presenting with convulsions resembling stroke and near-death states, grind Chen Sha (Cinnabar) thickly in fresh well water, and apply the juice to the five centers (palms and soles); this is most effective.
Tang stated: For treating fetal epilepsy and convulsive wind, fully intact Scorpions (Quanxie) including head and tail can be used. Wrap them in fresh Mentha leaves, bind with hemp thread, roast over fire until dry, and grind into powder. Separately grind Sheng Zhusha (Raw Cinnabar) and Shexiang (Moschus) in small amounts. Take with Ophiopogonis Decoction (Maimendong Tang).
Xue Ji stated: A pediatric patient suffered from fetal convulsion with no response to various medications. Zihewche (Placenta Hominis) was ground into a mud-like paste. Each dose was approximately one qian, taken with emulsified milk. Concurrently, the mother took Shiquan Dabu Tang (Ten Significant Tonifying Decoction) added with Gouteng (Uncaria Ramulus cum Uncis) and Lulu (Euphorbiae Humifusae Herba). After more than two months, attacks gradually lessened. After over a year of medication, attacks became sparse. After another year, there were no further attacks. Following the eruption of smallpox, symptoms recurred. Zihewche was ground into paste, mixed with glutinous rice flour to form pills the size of soybeans. Each dose consisted of one hundred pills, taken with milk. After consuming two placentas, the patient recovered completely. Upon marriage, symptoms recurred. The previous pills were used again along with Shiquan Dabu Tang, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) added with Danggui (Angelicae Sinensis Radix), Huangqi (Astragali Radix), Rougui (Cinnamomi Cortex), and Wuweizi (Schisandrae Fructus). Over a year, benefiting from abstinence from sexual activity, the patient recovered. Later, due to labor and exertion, symptoms returned. The previous pills and Shiquan Dabu Tang were ineffective. A large dose of Dushen Tang (Ginseng Decoction) was administered, consuming several jin. Only then did attacks subside slightly. Subsequently, Ren Shen (Ginseng Radix) 2 liang and Fuzi (Aconiti Lateralis Radix Praeparata) 1 qian were given for several doses, stopping the attacks immediately. The previous medicines were continued, interspersed with Dushen Tang, leading to recovery. Another pediatric patient with fetal convulsion recovered using Zihewche Wan (Placenta Pill), Shiquan Dabu Tang, and Gouteng Gao (Uncaria Ointment). Symptoms recurred after marriage. Large doses of Dushen Tang, Liuwei Dihuang Wan added with Wuweizi, Huangqi, and Danggui were decocted and taken for half a year. Attacks lightened slightly, and after a year, there were no recurrences. Whenever fatigue or anger triggered recurrence, the previous medicines were used, resulting in immediate recovery. Furthermore, after recovering from Shanghan (Cold Damage), symptoms recurred with complete deficiency signs that were indescribable. Two placentas were decocted in Dushen Tang, and after consuming over ten jin, the patient recovered. Later, suffering from wind-cold cough, dry throat, and internal heat, the patient recovered using Liuwei Dihuang Wan ingredients added with Wuweizi, decocted and taken, along with Shiquan Dabu Tang.
