The symptoms of heatstroke (Zhongshu, 中暑) include a dirty complexion, chest stuffiness and fainting, unconsciousness with loss of awareness, spontaneous cold sweating, slightly cold hands and feet, or vomiting or diarrhea, or wheezing or abdominal fullness. One may administer the powder of Laifu Dan (Revival Pill), combined with Suhexiang Wan (Styrax Pill), mixed with water and forced down the throat. Alternatively, grind Laifu Dan into powder and administer it mixed with water. Administering Queshu San (Heat-Relieving Powder) mixed with water is also acceptable. Once the patient regains slight consciousness, use Xiangru Yin (Elsholtzia Decoction) or Xiangru Tang (Elsholtzia Decoction), boiled until ready, strained to remove dregs, and taken with a small amount of Moschus (Shexiang, 麝香). Alternatively, insert garlic flesh into the nostrils, or grind garlic juice and administer it. This is because heatstroke injures the body with summer-toxin, causing Yang Qi to float outward while Yin Cold resides inward. Most heat-relieving medicines contain warm agents, such as the use of Ginger Rhizome (Shengjiang, 生姜) and Cinnamon Twig (Guizhi, 桂枝) in Dashun San (Great Harmony Powder), Clove (Dingxiang, 丁香) in Pipoye San (Loquat Leaf Powder), and Elsholtzia (Xiangru, 香薷) in Xiangru Yin. Elsholtzia is pungent and warm; garlic is also pungent and warm. Moreover, garlic has a strong, foul odor that can unblock various orifices. Generally, substances with extreme odors or fragrances can unblock orifices. For those dead from heat, one must not immediately give cold water or let them lie on cold ground, just as for those frozen to death, one must first give cold water; if suddenly brought near fire, they will die. Another method: for those who died on the road due to heatstroke, placing them in the sun or near fire, and pouring hot water over them can revive them. Upon initially feeling heatstroke, immediately use roof tiles heated by the sun, or cloths dipped in hot water, alternating to moxa/iron their chest, abdomen, and below the navel. Quickly administer the powder of Erqi Dan (Two Qi Pill) mixed with water and forced down. Another prescription uses uncursed Gleditsia Sinensis Fruit (Zaoguo, 皂角) in any quantity, scrape off the black skin, burn until smoke is nearly gone, cover with a basin placed on the ground, ensuring no smoke escapes around the edges. Take one qian of Gleditsia ash and six qian of Licorice Root (Gancao, 甘草) powder, mix evenly. Take one qian per dose, mixed with fresh drawn cool water. For those with Qi deficiency, mix with warm rice water (Wen Jiangshui, 温浆水). For coma and unconsciousness, two doses are sufficient. This is because phlegm obstructs the orifices in heatstroke patients, and Gleditsia can ventilate and clear phlegm. There is also a condition called "summer journey syndrome," which resembles heatstroke but is milder, characterized by desire to sleep and lazy speech. For excess patterns, add Coptis Rhizome (Huanglian, 黄连) one qian to Xiangru Yin; for deficiency patterns, add Elsholtzia one qian to Xingxiang Yin. After recovery, if cold sweat continues, hands and feet remain inverted/cold, and there is irritability, stuffiness, and excessive thirst, Xiangru Yin is suitable. If after recovery, the physician over-administered cold agents, causing continuous vomiting and diarrhea, external heat and internal cold, irritability, excessive thirst, and even a desire to strip naked, resembling Taiyang disease (Taiyangbing, 太阳病), this is Yin excess repelling Yang outward. Warm medicines like Xiangru Yin added with Aconite Lateral Root (Fuzi, 附子), soaked and cooled before administration, are suitable. For thirst, Suopi Yin (Splese-Restraining Decoction) added with Fuzi, also soaked and cooled, is suitable. Li Dongyuan stated: Obtained while resting is called Zhongshu (heatstroke). Zhongshu is a Yin pattern and should be dispersed. When avoiding summer heat, seeking coolness in deep halls and large buildings leads to what is named Zhongshu. The disease necessarily involves headache, aversion to cold, bodily stiffness, joint pain, vexation, great heat in the skin and muscles, and anhidrosis. This is caused by the Yin cold of rooms suppressing the body's Yang Qi, preventing its extension and emergence. It is correct for the world to mainly use Dashun San. Obtained while moving is called Zhongre (mid-heat). Zhongre is a Yang pattern, caused by heat injuring Yuan Qi, not a disease of the physical form. If walkers or farmers obtain it through labor under the sun, it is named Zhongre. The disease necessarily involves severe headache, restlessness and fever, aversion to heat, great heat upon touching the skin and muscles, excessive thirst with desire to drink, profuse sweating, and lack of energy to move. This is heavenly heat externally injuring Lung Qi, treated mainly with Cangzhu Baitu Tang (Atractylodes Macrocephala and White Tiger Decoction). Xue Ji stated: If a person's Yuan Qi is insufficient and previous medicines are ineffective, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) is suitable. Generally, in summer, Yang Qi floats outward, and Yin Qi hides inward. If people suffer internal injury to Middle Qi from diet and fatigue, or external injury to Yang Qi from summer heat and labor, they often suffer from this. The method should focus on regulating and tonifying Yuan Qi, assisted by resolving summer heat. If it is Zhongshu, it is a Yin-cold pattern; the method should focus on tonifying Yang Qi, with minor assistance from resolving summer heat. Therefore, ancient sages mostly used Ginger Rhizome, Cinnamon Twig, Aconite, etc. This is a good method deduced from the *Neijing* ("abandoning the season to follow the pattern"). Now, those who die from summer-heat symptoms have blue-black fingernails or limbs. This is all due to not investigating the cause, not warming and tonifying the interior, but indiscriminately using Xiangru Yin, etc., which causes error. Xiangru Yin is an agent that disperses Yang Qi and guides True Yin. One must examine whether the pattern matches before taking it; then what is there to worry about? If a person's Yuan Qi is inherently deficient, or engages in excessive sexual activity and drinks it, it precisely invites summer-heat.
