[Chi Pulse Weak and Thready] If the chi pulse at the cun position is weak, this indicates a decline of Mingmen fire leading to deficiency of the Spleen and Lung. Ren Shen Li Zhong Wan (Ginseng Regulate the Middle Pill) should be used; if ineffective, Ba Wei Di Huang Wan (Eight-Ingredient Rehmannia Pill) should be substituted.
[Cun Pulse on the Right Surging and Rapid] If the cun pulse on the right hand is surging and rapid, this signifies Heart Fire overcoming Lung Metal. Ren Shen Ping Fei San (Ginseng Calming the Lung Powder) should be used; if ineffective, Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be substituted.
[Cun and Guan Pulses on the Left Surging, Wiry, and Rapid] If the cun and guan pulses on the left hand are surging, wiry, and rapid, this indicates heat in the Heart and Liver meridians. Chai Hu Qing Gan San (Bupleurum Clearing the Liver Powder) should be used; if ineffective, Niu Huang Qing Xin Wan (Calculus Bovis Clearing the Heart Pill) should be added to clear wind-heat. Subsequently, Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should still be used to anchor floating Yang.
[Chi Pulse on the Left Rapid but Forceless] If the chi pulse on the left hand is rapid but forceless, this indicates Kidney deficiency with water qi flooding upward. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) with Wu Wei Zi (Schisandra Fruit) should be used to nourish Yin. If the pulse is faint and thready, or if there is coldness in the hands and feet, or accompanied by panting and shortness of breath, Ba Wei Di Huang Wan (Eight-Ingredient Rehmannia Pill) should be urgently used to tonify Yang Qi.
[Red Cracked Tongue] A red tongue with cracks, or prickles, accompanied by thirst with desire to drink, frequent urination, intermittent fever, fever without fixed location, or heat rising from the soles of the feet, indicates dryness of Kidney Water and hyperactivity of Heart Fire. Jia Jian Ba Wei Wan (Modified Eight-Ingredient Pill) should be the main treatment, supplemented by Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). If cold-natured drugs are mistakenly used, it will inevitably transform into a deficiency-cold syndrome leading to death.
[Mouth and Tongue Ulcers with Unremitting Thirst and Intermittent Fever] Fever may cease at night while present during the day, or vice versa, with frequent urination. Heat may originate from the soles of the feet, the hypochondrium, or the lower abdomen, appearing on the exterior without a fixed location. This is a syndrome of deficiency in the three Yin meridians of the foot. Jia Jian Ba Wei Wan (Modified Eight-Ingredient Pill) should be the main treatment, supplemented by Shi Quan Da Bu Tang (Ten Complete Tonifying Decoction). If cold-natured drugs for clearing fire are mistakenly used, damaging the Spleen and Stomach again, symptoms such as empty fullness in the chest and abdomen, loss of appetite, loose stools, difficult urination, abdominal distension, limb edema, or cold hands and feet may occur. This is a deficiency-cold syndrome due to deficiency of the three Yin meridians of the foot. Jia Jian Jin Gui Shen Qi Wan (Modified Golden Cabinet Kidney Qi Pill) should be urgently used; recovery is possible.
[Abdominal Swelling] Morning relief and evening urgency indicate Yin deficiency; evening relief and morning urgency indicate Yang deficiency; urgency both morning and evening indicates dual deficiency of Yin and Yang. For Yang deficiency, Liu Jun Zi Tang (Six-Gentlemen Decoction) is taken in the morning, and Jia Jian Shen Qi Wan (Modified Kidney Qi Pill) in the evening. For Yin deficiency, Si Wu Tang (Four-Substance Decoction) with Ren Shen (Ginseng) and Bai Zhu (Atractylodes Macrocephala Rhizome) is taken in the morning, and Jia Jian Shen Qi Wan in the evening. For true Yang deficiency, Ba Wei Di Huang Wan (Eight-Ingredient Rehmannia Pill) is taken in the morning, and Bu Zhong Yi Qi Tang in the evening. If there is abdominal fullness, limb swelling, cold hands and feet, difficulty digesting food, diarrhea, or cold breath exhaled from the mouth, this indicates collapse of true Yang, deficiency-cold of the Spleen, Lung, and Kidney, failing to control and causing water flooding. Jia Jian Shen Qi Wan (Modified Kidney Qi Pill) should be urgently used, otherwise rescue is impossible.
[Fever with Aversion to Cold] If the cun pulse is faint, it is named insufficiency of Yang Qi; when Yin Qi enters the Yang aspect, aversion to cold occurs. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. If the chi pulse is weak, it is named insufficiency of Yin Qi; when Yang Qi sinks into the Yin aspect, fever occurs. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used. If there is limb fatigue during the hot summer season, this is caused by damp-heat invasion, belonging to deficiency of form/constitution and excess of pathogenic qi. One should specifically tonify Yang Qi using Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). If there is fever with great thirst and desire to drink, red eyes, and red face, this is fever due to Blood deficiency, belonging to deficiency of both form and disease. One should specifically tonify Yin Blood using Dang Gui Bu Xue Tang (Angelica Tonifying Blood Decoction).
[Fever with Thirst] If the cun and guan pulses on the right are floating, large, and forceless, this is deficiency of Spleen and Lung Qi. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. If rapid and forceful, this is heat in Spleen and Lung Qi. Zhu Ye Shi Gao Tang (Bamboo Leaf and Gypsum Decoction) should be used. If the cun pulse is faintly rapid and forceless, this is deficiency-heat of Lung Qi. Zhu Ye Huang Tang (Bamboo Leaf Yellow Decoction - *Note: Text likely refers to Zhu Ye Shi Gao Tang*) should be used. If the chi pulse is faint and thready or faintly rapid and forceless, this is decline of Mingmen fire. Ba Wei Di Huang Wan (Eight-Ingredient Rehmannia Pill) should be used. If the cun and guan pulses on the left are rapid and forceful, this is heat in Heart and Liver Qi. Chai Hu Zhi Zi San (Bupleurum and Gardenia Powder) should be used. If rapid but forceless, this is deficiency of Heart and Liver Qi. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used. If the chi pulse is rapid and forceless, this is deficiency fire of the Kidney meridian. Jia Jian Ba Wei Wan (Modified Eight-Ingredient Pill) should be used. Generally, thirst after healing from sores, sores appearing after thirst, mouth and tongue ulcers, sore throat and swelling, cracked lips, yellow tongue, red eyes, and surging phlegm-saliva are all signs of a deteriorating condition. Only these pills can save the patient.
[Eyes] Blurred and weak vision, or internal obstruction with black spots, belongs to Blood deficiency and divine spirit exhaustion. Zi Yin Shen Qi Wan (Nourishing Yin Kidney Qi Pill) should be used. If vision is weak or abnormal shapes are seen, this belongs to weakness of Yin essence. Zi Yin Di Huang Wan (Nourishing Yin Rehmannia Pill) should be used. If vision is weak or objects appear enlarged, this belongs to Yang excess and Yin deficiency. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used. If eyes feel tight and body is fatigued, or skin and muscles are numb, this belongs to deficiency of Spleen and Lung Qi. Shen Xiao Huang Qi Tang (Miraculous Efficacy Astragalus Decoction) should be used. If vision darkens at night, even under lamplight, this belongs to sinking of Yang deficiency. Jie Ming Ye Ling San (Clarifying Brightness Night Spirit Powder) should be used. If eyes are dim, body is fatigued, with internal obstruction and tinnitus, this belongs to deficiency of Spleen and Stomach Qi. Yi Zhi Cong Ming Tang (Benefiting Wisdom and Brightness Decoction) should be used. The essence and qi of the five Zang and six Fu organs all receive their source from Spleen Earth, ascending to penetrate the eyes. The Spleen is the head of all Yin meridians, and the eyes are the gathering place of blood vessels. Tonifying Spleen Earth is the best approach.
[Nosebleed and Hematemesis] If the cun and guan pulses on the left are rapid and forceless, this is Blood deficiency. Si Wu Tang (Four-Substance Decoction) with Ren Shen (Ginseng) and Bai Zhu (Atractylodes Macrocephala Rhizome) should be used. If floating and forceless, this is Qi deficiency. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. If the chi pulse is rapid or forceless, this is Kidney deficiency. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used. If the cun and guan pulses on the right are rapid and forceful, this is heat in the Lung and Stomach. Xi Jiao Di Huang Tang (Rhinoceros Horn and Rehmannia Decoction) should be used. If rapid but forceless, this is deficiency-heat in the Lung and Stomach. First use Xi Jiao Di Huang Tang from "Ji Sheng", then Si Wu Tang (Four-Substance Decoction) with Ren Shen (Ginseng), Fu Ling (Poria), and Bai Zhu (Atractylodes Macrocephala Rhizome). If the chi pulse is rapid and forceless, this is Yin deficiency. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used. If the face is yellow, eyes are dry with excessive discharge, and hands are numb, this is deficiency of Spleen and Lung. Huang Qi Shao Yao Tang (Astragalus and Peony Decoction) should be used.
[Little Appetite] If inability to eat is due to Stomach Qi deficiency, Si Jun Zi Tang (Four-Gentlemen Decoction) should be used. If inability to transform food is due to Spleen Qi deficiency, Liu Jun Zi Tang (Six-Gentlemen Decoction) should be used. If stools are unformed or vomiting occurs, this is deficiency-cold of Spleen Qi. Liu Jun Zi Tang (Six-Gentlemen Decoction) with Gan Jiang (Dried Ginger) and Mu Xiang (Aucklandia Root) should be used. If vomiting and thirst occur, or aversion to cold drinks/food exists, this is deficiency-heat of Stomach Qi. Wu Wei Yi Gong San (Five-Flavor Enhancing Function Powder) should be used. Preference for cold drinks/food indicates excess heat of Stomach Qi. Xie Huang San (Draining Yellow Powder) should be used.
[Vaginal Discharge] Caused by external pathogens entering the uterus during menstruation or postpartum, transmitting to the five Zang organs. Green discharge belongs to the Liver; Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) with Fang Feng (Saposhnikovia Root) should be used. For damp-heat stagnation with red, scanty, painful urination, add Chao Hei Long Dan Cao (Stir-fried Black Gentian Root) to the previous formula. For Liver Blood deficiency, or dryness-heat/wind-heat, Liu Wei Wan (Six-Ingredient Pill) and Xiao Yao San (Free and Easy Wanderer Powder) should be used. Red discharge belongs to the Heart; Xiao Chai Hu Tang (Minor Bupleurum Decoction) with Huang Lian (Coptis Rhizome), Shan Zhi (Gardenia Fruit), and Dang Gui (Angelica Sinensis Root) should be used. For injury from excessive thinking, Miao Xiang San (Wonderful Fragrance Powder) and Liu Wei Wan (Six-Ingredient Pill) should be used. White discharge belongs to the Lung; Liu Wei Wan (Six-Ingredient Pill) and Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with Shan Zhi (Gardenia Fruit) should be used. Yellow discharge belongs to the Spleen; Liu Wei Wan (Six-Ingredient Pill) and Liu Jun Zi Tang (Six-Gentlemen Decoction) with Shan Zhi (Gardenia Fruit) and Chai Hu (Bupleurum Root) should be used; Gui Pi Tang (Restore the Spleen Decoction) should not be applied. Black discharge belongs to the Kidney; Liu Wei Wan (Six-Ingredient Pill) should be used. For dual deficiency of Qi and Blood, Ba Zhen Tang (Eight-Treasure Decoction) should be used. For sinking of Yang Qi, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. For downward pouring of damp-phlegm, add Fu Ling (Poria), Ban Xia (Pinellia Tuber), Cang Zhu (Atractylodes Rhizome), and Huang Bo (Phellodendron Bark) to the previous decoction. For Qi deficiency with phlegm-fluids, Si Qi Tang (Four-Seven Decoction) should be used to deliver Liu Wei Wan (Six-Ingredient Pill). If the disease has been long-standing with sinking of Yuan Qi, or damage from purgatives, strengthening the Spleen and Stomach and raising Yang Qi is best. Adhering to a person's fatness or thinness and using drying-dampness or draining-fire drugs will instead damage the Spleen and Stomach, causing significant harm.
[Blood in Urine and Stool] If the right guan pulse is floating and rapid, this is Qi deficiency with heat. Si Jun Zi Tang (Four-Gentlemen Decoction) with Sheng Ma (Cimicifuga Rhizome) and Dang Gui (Angelica Sinensis Root) should be used. If the chi pulse is floating and large or weak/faint, this is sinking of Yuan Qi. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. If the left guan pulse is surging and rapid, this is Blood deficiency. Si Wu Tang (Four-Substance Decoction) with Chao Shan Zhi (Stir-fried Gardenia Fruit), Sheng Ma (Cimicifuga Rhizome), and Qin Jiao (Gentiana Macrophylla Root) should be used. If the pulse is slow-thready or floating-large, this is Qi deficiency. Si Jun Zi Tang (Four-Gentlemen Decoction) with Sheng Ma (Cimicifuga Rhizome) and Pao Jiang (Prepared Ginger) should be used. If the chi pulse is surging-rapid or forceless, this is Kidney deficiency. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used. If blood in urine is caused by sexual exhaustion damaging essence and Qi, with deficiency-Yin moving fire, for various bleeding diseases, regardless of complex pulse and symptom patterns, using the previous pill ingredients decocted and taken orally is best.
[Diarrhea] Occurring in the fifth watch or early morning indicates Spleen and Kidney deficiency. Si Shen Wan (Four Spirits Pill) should be taken in the fifth watch, and Bai Zhu San (Atractylodes Powder) during the day. If ineffective, or if it recurs after stopping, Ba Wei Wan (Eight-Ingredient Pill) should be urgently used to tonify Mingmen fire to generate Spleen Earth, which will stop the diarrhea.
[Constipation] Belongs to deficiency of Spleen and Lung, dryness of Large Intestine fluids, or Blood deficiency with fire scorching. Do not count the days. If diet is heavy, one must wait until abdominal fullness occurs, with the urge to defecate but inability to do so; this indicates heat in the rectum. Pig bile should be used to moisten. If bitter-cold and acrid-dispersing drugs are recklessly used, Yuan Qi will be further injured, leading to uncontrolled evacuation or middle-jiao痞 (fullness/stagnation). For Qi and Blood deficiency, Ba Zhen Tang (Eight-Treasure Decoction) with Ma Zi Ren (Hemp Seed) should be used. For deficiency of intestines and stomach, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with Ma Zi Ren (Hemp Seed) should be used. If Kidney fluids fail to moisten, Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) with Ma Zi Ren (Hemp Seed) should be used. If rich flavors accumulate and obstruct, with dribbling urinary retention, this is Liver and Kidney deficiency. Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used to nourish Kidney Water, and Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) to tonify Spleen and Stomach. For fever or afternoon fever, Liu Jun Zi Tang (Six-Gentlemen Decoction) and Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) should be used to nourish Yin Blood and clear wind-heat. If accompanied by bone and tendon pain, Tou Jing Jie Luan Tang (Penetrating Meridians Relieving Spasm Decoction) and Qin Jiao Di Huang Tang (Gentiana and Rehmannia Decoction) should be used first, followed by Ba Zhen San (Eight-Treasure Powder) with Mu Dan Pi (Moutan Cortex) and Chai Hu (Bupleurum Root). If wind-expelling drugs were mistakenly used injuring Yin Blood, Yi Lao Qu Feng Wan (Yi Lao Wind-Expelling Pill) should be used. For conditions affecting thighs, scrotum, or feet, Si Sheng San (Four Generations Powder) and Di Huang Wan (Rehmannia Pill) are best. If Caowu (Aconitum Carmichaelii Lateral Root) or Chuanyou (Aconitum Kusnezoffii Root) were mistakenly ingested, or Badou (Croton Seed) or Pishi (Arsenic) applied, causing muscle ulceration, it may become a leprosy-like condition; practitioners must examine carefully.
[Red Face with Itching or Eyebrow Loss] Belongs to wind-heat in the Lung meridian. Ren Shen Xiao Feng San (Ginseng Expelling Wind Powder) and Hua Pi San (Birch Bark Powder) should be used.
