Renshen Buwei Tang (Ginseng Decoction for Tonifying the Stomach) [from *Shixiao Fang*]: Treats internal cataracts and dim vision caused by overexertion and irregular diet.
Ingredients: Huangqi (Astragali Radix), Renshen (Ginseng Radix) (each 1 liang), Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma) (8 qian), Manjingzi (Viticis Fructus) (2.5 qian), Huangbai (Phellodendri Cortex) (mixed with wine and stir-fried four times), Baishao (Paeoniae Radix Alba) (each 3 qian).
Preparation and Administration: Cut the herbs into small pieces. Take 3–5 qian per dose, add two zhan of water, decoct until one zhan remains, strain, and take warm after meals and before bedtime. After taking three to five doses, the visual field of both eyes becomes wide, and vision is as clear as in childhood; one may feel as if stepping on the ground without knowing height or depth. This is because yang-ascending herbs are taken frequently in winter. Stop taking the medicine when symptoms improve, and resume after five to seven days. This medicine is suitable for spring use and is considered a seasonal remedy.
Yuanyuan Neizhang Shengma Tang (Evodia Decoction for Clearing Internal Cataracts) (i.e., Chonghe Yangwei Tang):
Huangqin Huanglian Tang (Scutellaria and Coptis Decoction)
Ingredients: Huangqin (Scutellariae Radix) (washed with wine and stir-fried), Shengdihuang (Rehmanniae Radix Recens) (washed with wine), Caolongdan (Gentianae Macrophytae Radix) (washed with wine and stir-fried four times, each 1 liang above), Huanglian (Coptidis Rhizoma) (mustards removed, washed with wine and stir-fried, 7 qian).
Preparation and Administration: Cut the herbs into small pieces. Take 2 qian per dose, add two zhan of water, boil several times, strain, decoct again until one zhan remains, and take hot. Do not take in the afternoon or evening; it is effective only when taken at noon.
Fuming San (Restoring Vision Powder): Treats internal cataracts.
Ingredients: Huangqi (Astragali Radix) (1.5 qian), Shengdihuang (Rehmanniae Radix Recens), Chaihu (Bupleuri Radix), Lianqiao (Forsythiae Fructus), Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma) (each 1 qian), Danggui (Angelicae Sinensis Radix) (2 qian), Cangzhu (Atractylodis Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Chenpi (Citri Reticulatae Pericarpium) (each 0.5 qian), Huangbai (Phellodendri Cortex) (0.3 qian).
Preparation and Administration: Add two large zhan of water, decoct until one zhan remains, strain, and take warm after meals. Avoid alcohol, damp noodles, and pungent/spicy/hot spices.
Qianghuo Tuiyi Wan (Notopterygium Pill for Removing Opacities) (also known as Dihuang Wan): Treats internal cataracts, with small blue-white opacities in the right eye, large slightly white opacities, headache, dilated pupils, upper body heat and aversion to heat, dry stool or occasional difficulty in defecation, normal urination, headache and distending pain in the eyes when encountering hot weather or warm places, normal appetite, and dim vision after sunset or on cloudy days. This condition can also be treated with Ziyin Dihuang Wan (Yin-Nourishing Rehmannia Pill).
Ingredients: Shudihuang (Rehmanniae Radix Preparata) (8 qian), Shengdihuang (Rehmanniae Radix Recens) (wine-processed), Huangbai (Phellodendri Cortex) (wine-processed), Dangguishen (Body of Angelicae Sinensis Radix) (wine-processed), Chongweizi (Leonuri Fructus), Danshen (Salviae Miltiorrhizae Radix et Rhizoma) (each half liang), Heifuzi (Prepared Aconiti Lateralis Radix) (prepared), Hanshuishi (Halloysitum Rubrum) (raw use, each 1 qian), Shaoyao (Paeoniae Radix) (1 liang 3 qian), Fangji (Stephaniae Tetrandrae Radix) (wine-processed, 2 qian), Zhimu (Anemarrhenae Rhizoma) (wine-processed), Mudanpi (Moutan Cortex), Qianghuo (Notopterygii Rhizoma et Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma) (each 3 qian), Chaihu (Bupleuri Radix) (half liang or 3 qian).
Preparation and Administration: Grind into fine powder, make pills with refined honey, size of small beans. Take 50–70 pills per dose, swallow with plain water on an empty stomach. If digestion is incomplete, take when hungry. Avoid speaking after taking; follow with food to suppress the medicinal force.
Li Dongyuan's *Lanshi Micang* states: If the opacity is in the upper part, add Gegen (Puerariae Radix) and Shengma (Lygodii Spora); if in the lower part, add Chaihu (Bupleuri Radix) and Qianghuo (Notopterygii Rhizoma et Radix). This is correct.
Danggui Tang (Angelica Decoction) (see "Dilated Pupils").
Chonghe Yangwei Tang (Harmonizing and Stomach-Nourishing Decoction): Treats early-stage internal cataracts, slight dimness of vision, black spots floating in the field of vision, aqueous humor turning pale green, followed by diplopia, aqueous humor turning pale white, and eventually blindness with pure white aqueous humor.
Ingredients: Chaihu (Bupleuri Radix) (7 qian), Renshen (Ginseng Radix), Danggui (Angelicae Sinensis Radix) (soaked in wine), Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Shengma (Lygodii Spora), Gegen (Puerariae Radix) (each 1 liang), Huangqi (Astragali Radix), Qianghuo (Notopterygii Rhizoma et Radix) (each 1.5 liang), Baishao (Paeoniae Radix Alba) (6 qian), Fangfeng (Saposhnikoviae Radix) (5 qian), Bai Fuling (Poria) (3 qian), Wuweizi (Schisandrae Chinensis Fructus) (2 qian), Gan Jiang (Zingiberis Rhizoma) (1 qian).
Preparation and Administration: Cut the herbs into small pieces. Take 6 qian per dose, add three zhan of water, decoct until two zhan remain, add Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma) each 1 qian, decoct again until one zhan remains, strain, and take warm after meals.
Formula Analysis: This formula addresses uneven Liver Wood qi and internal Heart Fire. Chaihu (Bupleuri Radix) soothes the Liver, Renshen (Ginseng Radix) opens the Heart orifices, and Huanglian (Coptidis Rhizoma) drains Heart Fire as the Sovereign drug. Wine-processed Danggui (Angelicae Sinensis Radix) nourishes the hundred vessels; Wuweizi (Schisandrae Chinensis Fructus) astringes the boiling qi of the hundred vessels; the Pericardium governs blood, and Baishao (Paeoniae Radix Alba) smooths the blood vessels and disperses evil blood as the Minister drugs. Bai Fuling (Poria) drains dampness from the Bladder; Qianghuo (Notopterygii Rhizoma et Radix) clears pathogenic factors from the Small Intestine; Gancao (Glycyrrhizae Radix et Rhizoma) tonifies the Triple Burner; Fangfeng (Saposhnikoviae Radix) raises the descending function of the Gallbladder as the Assistant drugs. Yin and Yang are governed by the Spleen and Stomach; Huangqi (Astragali Radix) tonifies the Spleen and Stomach; Baizhu (Atractylodis Macrocephalae Rhizoma) strengthens the Spleen and Stomach; Shengma (Lygodii Spora) and Gegen (Puerariae Radix) circulate through the Spleen and Stomach meridians; Huangqin (Scutellariae Radix) reduces vigorous fire; Gan Jiang (Zingiberis Rhizoma) enters vigorous fire as the Guide drug. This formula combines contrary and compliant treatments, reversal and standard approaches.
Yiqi Congming Tang (Qi-Benefiting and Brightening Decoction): Treats the same conditions as above. Also treats deafness and tinnitus.
Ingredients: Huangqi (Astragali Radix), Renshen (Ginseng Radix) (each 1.25 qian), Shengma (Lygodii Spora) (7.5 qian), Gegen (Puerariae Radix) (3 qian), Manjingzi (Viticis Fructus) (1.5 qian), Shaoyao (Paeoniae Radix), Huangbai (Phellodendri Cortex) (wine-fried, each 1 qian), Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma) (0.5 qian).
Preparation and Administration: Take 4 qian per dose, add two zhan of water, decoct until one zhan remains, strain, and take hot before sleep. Decoct again at the fifth watch (early morning) and take.
Formula Analysis: The sweet-warm nature of Huangqi (Astragali Radix) and Renshen (Ginseng Radix) treats deficiency fatigue as the Sovereign drug. The sweet-neutral nature of Gancao (Glycyrrhizae Radix et Rhizoma) connects and harmonizes; the bitter-neutral-slightly-cool nature of Shengma (Lygodii Spora) circulates through the Hand-Yangming, Foot-Yangming, and Foot-Taiyin meridians as the Minister drug. The sweet-neutral nature of Gegen (Puerariae Radix) and the pungent-warm nature of Manjingzi (Viticis Fructus) both promote ascending and emerging as the Assistant drugs. The sour-slightly-cool nature of Shaoyao (Paeoniae Radix) tonifies the Middle Jiao and smooths blood vessels; the bitter-cool nature of Huangbai (Phellodendri Cortex) treats insufficiency of Kidney Water and Bladder as the Guide drug. Wine-processing and frying utilize their heat properties. If heat signs exist, gradually increase Huangbai (Phellodendri Cortex); add in spring and summer, double in intense heat. Too much renders it ineffective; remove for Spleen-Stomach deficiency. For heat doubled this amount, Xiere Huanglian Tang (Heat-Draining Coptis Decoction) is the primary treatment.
Xiere Huanglian Tang (Heat-Draining Coptis Decoction): Treats internal cataracts with the same conditions, accompanied by excessive discharge, tearing, and fishy odor.
Ingredients: Huangqin (Scutellariae Radix) (wine-fried), Huanglian (Coptidis Rhizoma) (wine-washed), Chaihu (Bupleuri Radix) (wine-fried), Shengdihuang (Rehmanniae Radix Recens) (wine-washed, each 1 liang), Longdancao (Gentianae Radix) (3 qian), Shengma (Lygodii Spora) (5 qian).
Preparation and Administration: Take 3 qian per dose, add two zhan of water, decoct until one zhan remains, strain, and take hot before noon meal. Taking it in the afternoon causes Yang qi to reverse and stagnate; stop taking before sleep to avoid aiding Yin.
Formula Analysis: This is a formula for treating external pathogenic guests. To treat the host, Shengma (Lygodii Spora) governs the Spleen and Stomach, Chaihu (Bupleuri Radix) circulates through the Liver meridian as the Sovereign drug; Shengdihuang (Rehmanniae Radix Recens) cools blood as the Minister drug, because the Yangming, Taiyin, and Jueyin meridians have abundant blood. To treat the guest, Huanglian (Coptidis Rhizoma) and Huangqin (Scutellariae Radix) both treat damp-heat as Assistant drugs; Longdancao (Gentianae Radix) specifically removes various eye diseases as the Guide drug, because all damp-heat comes from outside as a guest.
*Qianjin Fang* Cizhu Wan (Magnetite and Cinnabar Pill): Treats widening and gradual dispersion of the aqueous humor, dim vision as if walking in fog or dew, gradually seeing black spots in the air, gradually seeing objects as double, eventually inability to focus light, and internal cataracts with pale green or pale white aqueous humor.
Ingredients: Cishi (Magnetitum) (attracts iron needles), Zhensha (Cinnabaris), Shenqu (Medicated Leaven).
Preparation and Administration: First, place Cishi (Magnetitum) in a fierce fire until red-hot, quench in vinegar seven times, dry, and grind separately into extremely fine powder (2 liang). Grind Zhensha (Cinnabaris) separately into extremely fine powder (1 liang). Mix with Shenqu (Medicated Leaven) powder (3 liang). Make a paste with 1 liang of Shenqu (Medicated Leaven) powder and water, cook until floating, knead into the previous medicines, and make pills with refined honey, size of paulownia seeds. Take 10 pills initially, increasing to 30 pills, swallow with rice soup on an empty stomach.
Formula Analysis: The pungent-salty-cool nature of Cishi (Magnetitum) anchors the Kidney meridian as the Sovereign drug, preventing the aqueous humor from moving outward. Zhensha (Cinnabaris) is slightly sweet-cool, anchors the Heart meridian as the Minister drug. The Liver is the mother of the Kidney; this follows the principle that "the son can make the mother solid"; when the Liver is solid, vision is bright. Shenqu (Medicated Leaven) is pungent-warm-sweet, resolves old food in the Spleen and Stomach as the Assistant drug. Raw use stimulates its vital energy; cooked use restrains violent energy. After taking the medicine, looking down reveals nothing, while looking up gradually reveals stars and moon; this is the effect. It also treats Heart fire attacking Metal and Water deficiency counter-restricting disease. For chronic recurrent cases, taking this prevents recurrence. Take on an empty stomach; in the forenoon, use Shihu Yeguang Wan (Dendrobium Night-Light Pill) as primary treatment.
Note: In this formula, Cishi (Magnetitum) represents Water entering the Kidney, Zhusha (Cinnabaris) represents Fire entering the Heart, and Shenqu (Medicated Leaven) specializes in entering the Spleen and Stomach, reflecting the Daoist principle of the Yellow Mother mediating between the Infant. Ni Sheng's explanation is verbose. Adding Chenxiang (Aquilariae Lignum Resinatum) half liang improves the regulation of Water and Fire.
Shihu Yeguang Wan (Dendrobium Night-Light Pill): Treats the same conditions as above.
Ingredients: Tianmendong (Asparagi Radix) (roasted), Renshen (Ginseng Radix), Fuling (Poria) (each 2 liang), Maimendong (Ophiopogonis Radix), Shudihuang (Rehmanniae Radix Preparata), Shengdihuang (Rehmanniae Radix Recens) (each 1 liang), Tusizi (Cuscutae Semen) (soaked in wine), Gan Juhua (Chrysanthemi Flos), Caojueming (Cassiae Semen), Xingren (Pruni Semen) (skin and tip removed), Gan Shanyao (Dioscoreae Rhizoma), Gouqizi (Lycii Fructus), Niuxi (Achyranthis Bidentatae Radix) (soaked in wine, each 7.5 qian), Wuweizi (Schisandrae Chinensis Fructus), Jili (Tribuli Fructus), Shihu (Dendrobii Caulis), Rongshong (Cynomorii Herba), Chuanxiong (Ligustici Chuanxiong Rhizoma), Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma), Zhike (Aurantii Fructus Immaturus) (bran-fried), Qingxiangzi (Celosiae Semen), Fangfeng (Saposhnikoviae Radix), Huanglian (Coptidis Rhizoma), Wuxijiao (Rhinocerotis Cornu) (sliced), Lingyangjiao (Saigae Tataricae Cornu) (sliced, each half liang).
Preparation and Administration: Grind into fine powder, make pills with refined honey, size of paulownia seeds. Take 30–50 pills per dose, swallow with warm wine or salted water.
Formula Analysis: This formula excels in tonification. Tonifying the upper and treating the lower, benefiting slowly and long-term, not quickly. Thus, Tianmendong (Asparagi Radix), Renshen (Ginseng Radix), and Tusizi (Cuscutae Semen) unblock the Kidney, calm the Spirit, strengthen Yin, and fill essence as the Sovereign drug. Wuweizi (Schisandrae Chinensis Fructus), Maimendong (Ophiopogonis Radix), Xingren (Pruni Semen), Fuling (Poria), Gouqizi (Lycii Fructus), Niuxi (Achyranthis Bidentatae Radix), and Sheng/Shu Dihuang (Rehmanniae Radix) astringe Qi, drain dampness, cool blood, and nourish blood as the Minister drugs. Gan Juhua (Chrysanthemi Flos), Jili (Tribuli Fructus), Shihu (Dendrobii Caulis), Rongshong (Cynomorii Herba), Chuanxiong (Ligustici Chuanxiong Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Zhike (Aurantii Fructus), Shanyao (Dioscoreae Rhizoma), and Qingxiangzi (Celosiae Semen) treat wind, alleviate deficiency, benefit Qi, and dispel toxins as the Assistant drugs. Fangfeng (Saposhnikoviae Radix), Huanglian (Coptidis Rhizoma), Caojueming (Cassiae Semen), Lingyangjiao (Saigae Tataricae Cornu), and raw Wuxijiao (Rhinocerotis Cornu) dissipate stagnation, drain heat, resolve knots, and brighten the eyes as the Guide drugs. Diseases where Yin weakness cannot coordinate with Yang should take this; this is the subsequent compliant treatment method.
Yiyin Shenqi Wan (Yin-Nourishing Kidney-Energy Pill): Treats the same conditions as above.
Ingredients: Shudihuang (Rehmanniae Radix Preparata) (wine-processed, 3 liang), Shengdihuang (Rehmanniae Radix Recens) (wine-processed and fried, 4 liang), Dangguiwei (Tail of Angelicae Sinensis Radix) (wine-processed), Mudanpi (Moutan Cortex), Wuweizi (Schisandrae Chinensis Fructus), Gan Shanyao (Dioscoreae Rhizoma), Shanzhuyu (Corni Fructus), Chaihu (Bupleuri Radix) (each half liang), Fuling (Poria), Zexie (Alismatis Rhizoma) (each 2.5 qian).
Preparation and Administration: Grind into powder, make pills with refined honey, size of paulownia seeds, coat with water-pulverized Zhensha (Cinnabaris). Take 50–70 pills per dose, swallow with light salted water on an empty stomach.
Formula Analysis: This formula strengthens the Master of Water to suppress Yang brilliance. Qi is injured by anger, scattering and failing to gather; Qi disease affects Blood. The Liver needs Blood to see. The eyes are the orifice of the Heart, and the Heart governs Blood. Thus, Shudihuang (Rehmanniae Radix Preparata) tonifies Blood deficiency, Dangguiwei (Tail of Angelicae Sinensis Radix) moves Blood, and Mudanpi (Moutan Cortex) treats accumulated Blood as the Sovereign drug. Fuling (Poria) harmonizes the Middle and benefits true Qi; Zexie (Alismatis Rhizoma) drains dampness and expels evil Qi; Shengdihuang (Rehmanniae Radix Recens) tonifies the true Yin of Kidney Water as the Minister drug. Wuweizi (Schisandrae Chinensis Fructus) tonifies the Five Viscera; Gan Shanyao (Dioscoreae Rhizoma) balances Qi and harmonizes the Stomach as the Assistant drug. Shanzhuyu (Corni Fructus) strengthens Yin and benefits essence, opens the nine orifices; Chaihu (Bupleuri Radix) guides into the Jueyin meridian as the Guide drug. Honey preparation aims to retain the medicine in the chest. Zhensha (Cinnabaris) coating aims to connect to the Heart. However, it must be taken with Qianjin Cizhu Wan (Magnetite and Cinnabar Pill) for better efficacy.
Ziyin Dihuang Wan (Yin-Nourishing Rehmannia Pill): Treats the same conditions as above, also treating excessive discharge and tearing.
Ingredients: Huangqin (Scutellariae Radix), Dangguishen (Body of Angelicae Sinensis Radix) (wine-processed), Shudihuang (Rehmanniae Radix Preparata) (each half liang), Tianmendong (Asparagi Radix) (roasted), Gancao (Glycyrrhizae Radix et Rhizoma) (fried), Zhike (Aurantii Fructus Immaturus), Chaihu (Bupleuri Radix), Wuweizi (Schisandrae Chinensis Fructus) (each 3 qian), Renshen (Ginseng Radix), Digupi (Lycii Cortex), Tianmendong (Asparagi Radix) (each 2 qian), Huanglian (Coptidis Rhizoma) (1 liang), Shengdihuang (Rehmanniae Radix Recens) (wine-processed, 1.5 liang).
Preparation and Administration: Grind into fine powder, make pills with refined honey, size of paulownia seeds. Take 100 pills per dose, swallow with tea water after meals, three times daily.
Formula Analysis: This formula treats the host slowly; slowness treats the root. The bitter-cool nature of Huanglian (Coptidis Rhizoma) and Huangqin (Scutellariae Radix) eliminates excess pathogenic factors as the Sovereign drug. The pungent-warm nature of Dangguishen (Body of Angelicae Sinensis Radix) and the bitter-sweet-cool nature of Sheng/Shu Dihuang (Rehmanniae Radix) nourish and cool blood as the Minister drug. The sour-cool nature of Wuweizi (Schisandrae Chinensis Fructus) is light and floats upward, astringing the dispersion of the aqueous humor; the bitter-sweet-cool nature of Renshen (Ginseng Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Digupi (Lycii Cortex), Tianmendong (Asparagi Radix), and Zhike (Aurantii Fructus Immaturus) drains heat and benefits Qi as the Assistant drug. Chaihu (Bupleuri Radix) guides the meridian as the Guide drug. Diseases with excessive blood loss and heat signs should also take this.
Lingyangjiao Tang (Saiga Decoction): Treats Qingfeng Neizhang (Green Wind Internal Cataract), worsening dizziness with fatigue, vertigo, headache, and painful dryness inside the eyes.
Ingredients: Lingyangjiao (Saigae Tataricae Cornu), Renshen (Ginseng Radix), Xuanshen (Scrophulariae Radix), Digupi (Lycii Cortex), Qianghuo (Notopterygii Rhizoma et Radix) (each 1 liang), Cheqianzi (Plantaginis Semen) (1.5 liang).
Preparation and Administration: Grind into powder. Take 1 qian of powder with one zhan of water, decoct until 0.5 zhan remains, strain, and take warm after meals.
Lou Quanshan said: This formula, along with the subsequent Lingyangjiao San (Saiga Powder), Bugan San (Liver-Tonifying Powder), and Lingyangjiao Yinzi (Saiga Beverage), all use Lingyangjiao (Saigae Tataricae Cornu), Xuanshen (Scrophulariae Radix), Xixin (Asari Radix et Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), and Cheqianzi (Plantaginis Semen) as Sovereign drugs. Because Lingyangjiao (Saigae Tataricae Cornu) circulates through the Jueyin meridian. Danxi said that Lingyangjiao (Saigae Tataricae Cornu) enters the Jueyin meridian very swiftly, which is correct. Xuanshen (Scrophulariae Radix) and Xixin (Asari Radix et Rhizoma) circulate through the Shaoyin meridian. Haizang said: Xuanshen (Scrophulariae Radix) treats misty qi in the air and rootless fire, being a sacred medicine. Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), and Cheqianzi (Plantaginis Semen) circulate through the Taiyang meridian. If tendons and vessels are withered and dry, add Xiakucao (Prunellae Spica) in various formulas; it can dissipate knotting qi and has the merit of nourishing Jueyin blood vessels. I have tried it with verified results. However, for these formulas, one must understand the location of the pathogen. If Qi collapses, take half with Renshen Gao (Ginseng Paste); if Qi is deficient, take half with Li Dongyuan's Buwei Renshen Tang (Stomach-Tonifying Ginseng Decoction) or Yiqi Congming Tang (Qi-Benefiting and Brightening Decoction); if Blood is deficient, take half with Shudihuang Wan (Rehmannia Pill). Furthermore, practice inner observation and quiet guarding, do not be disturbed by worldly burdens, let Yin qi settle, and only then will efficacy occur.
Xingren Fang (Apricot Kernel Formula): Treats Liver-Kidney wind-deficiency, pupils showing a bluish tint, and dark vision. Moisturizes viscera, cleans dirt, opens brightness, and expels wind to brighten the eyes.
Usage: Take five apricot kernels, remove skin and tips. At the first watch (5–7 AM), sit upright on the bed edge. Do not speak or exhale forcefully. Calm thoughts and clarify the spirit. Chew one kernel without swallowing. Chew five kernels one by one finely. When saliva fills the mouth, swallow in three portions. It goes directly to the Liver and Kidney.
