If chills, fever, and headache occur, it indicates that cold pathogenic factors are invading the Heart. When cold qi invades the Heart, Water and Fire elements battle each other; thus, at the onset of the disease, chills and fever appear first. The conflict between Water and Fire causes blood to alter within the middle jiao, resulting in dysentery with red discharge resembling water infused with *Arnebiae Radix et Rhizoma* (Zicao) or *Amaranthi Tricoloris Herba* (Xiancai). If the discharge lacks color, it signifies severe invasion of the Heart by cold pathogenic factors. If chills and fever appear first, but the discharged dysentery is purely white, it indicates that the cold pathogenic factor has invaded the Heart but has not yet disturbed the blood. If white dysentery appears first and later transforms into red dysentery, it means the invasion of the Heart by cold pathogenic factors is gradually intensifying. If red dysentery appears first and later transforms into white dysentery, it means the invasion of the Heart by cold pathogenic factors is gradually subsiding. If red and white discharges are equal, it indicates that Water and Fire are conflicting with equal strength, and cold-damp qi is battling. Sometimes there is more red and less white, sometimes less red and more white; this depends on the magnitude of the cold pathogenic force and the severity of the toxic dysentery. White predominance indicates a mild condition, while red predominance indicates a severe condition. The treatment method is to first eliminate the cold; thus, the prescribed medicine remains the same. According to the sexagenary cycle, in Bingzi, Bingwu, Jiazi, Jiawu, Gengzi, Gengwu years, Bingyin, Bingshen years, Jiayin, Jiashen years, Gengyin, Gengshen years, and Chenxu years, when the movement encounters Bing, Jia, and Geng influences, the harm is particularly severe. In Chouwei years, this syndrome should also appear. Furthermore, regardless of the Tai Sui (Grand Duke Jupiter), because heaven and earth undergo changes, the manifestations are diverse and cannot be fully exhausted. Today, people need not rigidly adhere to the Tai Sui year; one only needs to observe if, during spring and summer of a given year, there is much cold and austere transformation with little sunlight, and if cold and heat energies alternatively contend, or if there is much cold-damp transformation in summer. For dysentery caused by cold pathogenic factors invading the Heart, presenting initially with chills and fever, followed by sudden headache, or initial frequent bowel movements followed by red dysentery, or mixed red and white discharge, or pure dysentery, all are suitable for treating with Tongshen San (Miraculous Spirit Powder). After ingestion, if intense heat occurs, it will subsequently subside. If after two or three doses the intense heat does not subside, do not continue this prescription; see the separate formula discussion below.
*Ephedrae Herba* (Mahuang, 麻黄) (remove roots and nodes) 7.5 qian, *Cinnamomi Ramulus* (Guizhi, 官桂) (remove rough bark) 7.5 qian, *Ligustici Chuanxiong Rhizoma* (Chuanxiong, 川芎) 2 liang, *Atractylodis Macrocephalae Rhizoma* (Baizhu, 白术) 2 liang, *Ligustici Wallichii Rhizoma* (Gaoben, 本) 0.5 liang, *Angelicae Pubescentis Radix* (Duhuo, 独活) 0.5 liang, *Platycodonis Radix* (Jiegeng, 桔梗) 0.5 liang, *Saposhnikoviae Radix* (Fangfeng, 防风) 0.5 liang, *Paeoniae Radix Alba* (Shaoyao, 芍药) 0.5 liang, *Angelicae Dahuricae Radix* (Baizhi, 白芷) 0.5 liang, *Moutan Cortex* (Mudanpi, 牡丹皮) (remove heart) 2.5 qian, *Glycyrrhizae Radix et Rhizoma* (Gancao, 甘草) 2.5 qian, *Asari Radix et Rhizoma* (Xixin, 细辛) 3.3 qian plus, *Semen Kochiae* (Qianniu, 牵牛) 1.7 qian. Grind the above into fine powder. Take 2 qian per dose, at any time, mixed with boiled water, and drink warm including the dregs. If after two or three doses the chills and fever do not subside, do not request this prescription again; see the separate formula discussion below.
If after taking this medicine the chills and fever have subsided and the red dysentery has decreased, prepare and take the second and third formulas to achieve efficacy. If the chills and fever have subsided but the red dysentery has not decreased, take two or three more doses, but do not consume too much; limit to two doses per day. Later, when the red dysentery decreases and suddenly turns into white dysentery, sequentially prepare and take the second formula. When the frequency of defecation decreases, then prepare and take the third formula to ensure safety. However, in Liujia years and Liugeng years, during spring and summer, the seasonal qi is often cold. When people suffer from dysentery, this "Tongshen" (Miraculous Spirit) medicine is effective. If it is a Liujia year or Chouwei year, where dampness predominates, and people suffer from dysentery starting with chills and fever, add 1 liang of *Cardamomi Fructus Rotundus* (Roudoukou, 草豆蔻) to the formula and prepare together. Regardless of the Tai Sui, as long as during the year, especially in spring and summer, the climate is cold, and people suffer from dysentery starting with chills and fever, this formula is suitable.
When treating toxic dysentery at the onset, presenting initially with chills and fever, and after taking the previous formula the chills and fever have subsided and the red dysentery has decreased, it is appropriate to advance to Zhenhai San (Restore True Powder). If after taking the previous formula the chills and fever have not subsided and the red dysentery has not decreased, do not administer this medicine. Heaven and earth change, and symptoms are unpredictable; dysentery syndromes are manifold, so detailed explanation is necessary here.
*Terminalia chebula* (Hezi, 诃子) 5 fruits, wrapped in flour and roasted in fire until cooked, not raw, not burnt. Remove flour. Bite open while hot, remove seeds, use only the peel, and dry by baking. Grind and sieve into fine powder. Take 2 qian-bi (spoonfuls) per dose. Mix with 1.5 zhan of rice soup, decoct together to obtain 1 zhan. Consume warm with dregs on an empty stomach. Vomiting one or two mouthfuls of saliva is even better. After consuming several doses in this manner, the large intestine will gradually stabilize, and defecation frequency will decrease. Then prepare and take the third formula to consolidate the large intestine. If after taking the previous formula the intense heat has not subsided and bloody dysentery has not decreased, do not administer this medicine.
To treat epidemic toxic dysentery: after taking the previous two formulas, if the disease trend has subsided, and the discharged dysentery is minimal, or resembles clear feces, duck feces, tea soup, candle oil, or leaves only a small amount of red residue, it is appropriate to take this formula to consolidate the large intestine and restore true qi: Bo Shang Liu Huang Wan (Shipborne Sulfur Pill).
*Flumen Sulfur* (Liuhuang, 硫黄) (Bo Shang variety) 2 liang, remove sand and stones, grind finely into powder. *Coicis Semen* (Yiyiren, 薏苡仁) 2 liang, stir-fry, pound into powder. Mix the above two ingredients evenly, add boiled water to form pills the size of *Koelreuteriae Fructus* (Wutongzi, 桐子). Take 50 pills per dose, with rice soup on an empty stomach.

Grind into fine powder. Take 2 qian per dose, at any time, mixed with gruel drink.
