On the first day of warm disease, the Taiyang (Greater Yang) channel is affected. The Taiyang channel governs the exterior, and the exterior refers to the skin. Since the pathogen is between the skin, pain in the head, neck, waist, and spine occurs.
On the second day, the Yangming (Bright Yang) channel is affected. The pathogen resides in the muscles, resulting in feverish sensation in the flesh, dryness of the nose, and insomnia. At this stage, massage with ointments or moxibustion can be applied to induce sweating and achieve recovery.
On the third day, the Shaoyang (Lesser Yang) channel is affected, leading to heat in the chest and hypochondrium and deafness. The transmission of pathogenic factors through the three Yang channels is now complete, but the pathogen has not yet entered the Zang organs; therefore, sweating therapy can be used to achieve recovery.
On the fourth day, the Taiyin (Greater Yin) channel is affected. The Taiyin channel is the head of the three Yin channels. After the three Yang channels have been affected, the pathogen transmits into the Yin channels. Consequently, toxic qi enters the interior of the chest and diaphragm, causing dryness of the throat and abdominal fullness. In this case, emesis therapy can be used to achieve recovery.
On the fifth day, the Shaoyin (Lesser Yin) channel is affected. Toxic qi enters the abdomen, manifesting as heat in the mouth, dryness of the tongue, and a desire to drink. Purgative therapy can be employed to achieve recovery.
On the sixth day, the Jueyin (Terminal Yin) channel is affected. Toxic qi enters the intestines and stomach, presenting with irritability, fullness, and retraction of the genitals. Purgative therapy can be used to achieve recovery.
By the seventh day, the disease should theoretically resolve, as the transmission of pathogenic factors through the three Yin and three Yang channels is complete. If the disease persists beyond seven days, it indicates a "re-entry into the meridians" (Zai Jing Bing). This refers to the meridians being affected again.
If the disease persists for eight days or more without resolution, it may be due to repeated infection of various meridians, or because toxic qi remains after sweating, emesis, or purgation therapies, thus preventing the cessation of symptoms.
If the disease persists for nine days or more without resolution, it may be because the initial pathogen in one channel failed to transmit further, or because transmission through the three Yang channels was completed but failed to enter the three Yin channels. This leads to stagnation over several days and persistent symptoms. All these scenarios result from incomplete elimination of toxic qi, invasion of both the exterior and interior, damage to the meridians, and simultaneous involvement of the Fu and Zang organs.
Generally, if an individual encounters cold toxicity in winter, the disease may manifest in spring. Initially located at the exterior, if sweating, emesis, or purgation therapies are applied but the exterior syndrome persists, the toxic qi fails to disperse, leading to the eruption of macules and sores. Alternatively, if the weather is unusually warm in winter and the person contracts erratic Qi, the disease may not manifest immediately. In spring, accumulated cold may suppress the body, preventing the venting of toxic qi. When summer heat arrives, warm toxicity erupts from the skin and muscles, presenting as mottled lesions and hidden rashes resembling brocade patterns.
