Baihe disease refers to a condition where there is no specific meridian involvement, yet all the vessels share a common origin and are consequently affected by pathogenic factors. It often arises from shanghan (cold damage) or xu lao (deficiency consumption), evolving into this illness after a major disease fails to fully resolve. Its manifestations include: desiring to eat but being unable to do so; frequently remaining silent; desiring to lie down but being unable to sleep; desiring to walk but being unable to move; food may sometimes taste good, while at other times there is no desire to eat. The patient appears healthy but cannot walk while lying down; they feel cold yet are not actually cold; they feel hot yet are not actually hot; bitter taste in the mouth, and dark yellow urine. Baihe disease is difficult to cure with various medicines; administering drugs often causes severe vomiting and diarrhea, as if possessed by spirits. The body shape appears normal, but the pulse is slightly rapid. If headache occurs upon urination, recovery takes sixty days. If there is no headache upon urination but only a slight sensation of chilliness (xixiran), recovery takes forty days. If urination feels smooth but dizziness persists, recovery takes twenty days. Signs may appear before the onset of illness, four to five days after onset, or mildly manifest after twenty days or a month. If chills and vomiting occur, the disease is in the upper jiao (upper burner), and recovery should take twenty-three days. If abdominal fullness, slight wheezing, dry stool, defecation once every three to four days, and occasional loose stools occur, the disease is in the middle jiao (middle burner), and recovery should take sixty-three days. If dysuria (difficult and dribbling urination) occurs, the disease is in the lower jiao (lower burner), and recovery should take forty-three days. Treatment should follow the specific symptoms.
Huhu comprises two diseases affecting the throat and genitalia. Initially resembling shanghan, it may develop from shanghan. Symptoms include silence and somnolence, inability to sleep with eyes closed, and restlessness when lying or sitting. Parasites eating the throat are called "huo" (confusion), while those eating the genitalia and anus are called "hu" (fox). The patient dislikes food, has no desire to smell food odors, and their face alternates between red, black, and white. Parasites in the upper part cause hoarseness; parasites in the lower part cause dry throat. These are caused by damp-toxic qi.
In cases of shanghan, shiqi (seasonal qi pathogens), or rebing (heat disease), internal heat combined with poor diet leads to empty stomach and intestines, allowing san chong (three parasites) to become active and feed on the five zang organs and lower parts. Signs include colorless gums, a completely white tongue, ulcers inside the lips in severe cases, heavy limbs, and a vague, sleepy state. All these indicate parasites eating the anus. If the anus rots exposing the viscera, death ensues. One should frequently examine the inner upper lip; if ulcers and blood-spitting saliva are present, with millet-like eruptions inside the lip, it indicates mental restlessness (ao nu), meaning parasites are in the upper part eating the five zang organs. If ulcers form on the inner lower lip, the patient is unconscious, indicating parasites are eating the lower part; both can be fatal.
