Whenever the head is struck and the patient loses consciousness, immediately apply moxibustion with a fire lamp to the Yintang (Baihui area between eyebrows), Fontan (B16), Taiyang (Ex-HN5), Ermen (TE21), Jiafengchi (Fengchi GB20), Jianjing (GB21), Quchi (LI11), Hegu (LI4), Xingjian (LR2), and Zusanli (ST36). If the patient does not regain consciousness, cut the hair at the injury site, identify the bleeding point, scrape thin bark from a locust tree (Huai Pi) to the size of a copper coin, place it on the bleeding point, and perform moxibustion for three to five moxa cones (Zhuang). Also moxibustion Baihui (GV20) for three to five cones. If locust bark is unavailable, administer San Gui end and child's urine decoction orally. If bleeding persists, crush Baochao ash with fresh ginger and apply to the bleeding site; remove the next day. Apply Shengji San (Regenerate Flesh Powder) mixed with clear sesame oil to the affected area; Jinqiang Yao (Golden Gun Medicine) may also be used. When using Shengji San or wound-closing medicines, apply twice daily: first wash with clear sesame oil, then apply the medicine.
For severe limb injuries from falls or blows where blood and qi attack the heart causing aphasia, urgently administer Heqi San (Harmonizing Qi Powder) orally to restore consciousness. Apply warming compresses (Yun Yao) to the injured area and continue administering the original formula with added Chenxiang (Aquilaria Wood) and San Gui. If there is no improvement, administer honey mixed with ginger juice after steaming.
For injuries to the upper body limbs, add Chuanxiong (Ligusticum Rhizome), Baizhi (Angelica Dahurica), and Xixin (Asarum) to Xiaofeng San (Eliminate Wind Powder). For injuries to the lower body limbs, add Mugua (Chaenomeles Fruit), Niuxi (Achyranthes Root), and Binglang (Areca Seed).
If the testicles are suspended outside the body by antler branches or tree limbs and cannot be retracted, urgently mix Shenmeng San (Divine Protection Powder) or egg white with well water (Jinghua Shui) and apply to the navel and both thighs, avoiding the wound and genitals. Within one to two shichen (2-4 hours), the testicles will naturally retract; subsequently apply Shengji San for self-healing.
For anterior injuries causing urinary retention, use 40-50 long-beaked river snails, 40-50 leek bulbs, 40-50 green onion heads, 30-40 garlic cloves, and 2 qian of Piao Xiao (Mirabilite). Grind Beijing ink thickly into a paste, apply to the navel, then crush the other ingredients and apply over the ink paste. Bind with silk cloth; urination will resume immediately. Alternatively, crush Tongguan San (Opening Pass Powder) with green onions, heat them, and apply to the lower abdomen to promote urination.
For posterior injuries causing constipation, Wutong San (Five Transport Powder) is commonly used. Also known as Qianjinzi (Croton Seed), which has strong purgative effects when desiccated, but caution is advised as it can damage Yuan Qi (Original Qi) in weak patients. Instead, add San qian of Junjiang Yunqi San (General Harmonizing Qi Powder) to Xiaofeng San and take warm to relieve constipation.
For brain concussion causing aphasia, cut the hair at severe injury sites. Place thin locust bark on Baihui (GV20) and the injury site for moxibustion to restore consciousness. Subsequently, administer Xiaofeng San with Baizhi, Chuanxiong, Xixin, and child's urine decocted.
For persistent coughing and choking after trauma, administer Shiye (Persimmon Leaf), Pipaye (Loquat Leaf, retaining hairs), and Muxiang (Vladimiria Root) together via decoction. This condition, characterized by hiccups and dysphagia, is often due to cold in the stomach and is considered a malignant symptom. Traditional formulas using Chenpi (Tangerine Peel), Banxia (Pinellia Tuber), and Xiangfu (Cyperus Rhizome) are effective. According to this method, moxibustion should be applied to Rupang acupoint (left for males, right for females, located below the nipple, marked like a red bean, three cones).
For hemoptysis caused by trauma, first administer Baicaoshuang (Soot from Hundred Herbs) mixed with child's urine. If bleeding persists, use Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction). If Rhino Horn is unavailable, substitute with Shengma (Cimicifuga), along with Mudanpi (Moutan Bark) 1 liang and Chishao (Red Peony Root) 1 qian. Decoct these four ingredients in water and take orally.
In cases of hemoptysis during summer months due to heatstroke rather than direct trauma, administer Xiangru Yin (Elsholtzia Decoction). If ineffective, combine with Xijiao Dihuang Tang. Xiangru Yin consists of Huanglian (Coptis Rhizome), Xiangru (Elsholtzia Herb), Houpo (Magnolia Bark), and Bian Dou (Lablab Bean) stir-fried with ginger juice.
For excessive bleeding from trauma leading to syncope, this is termed Blood Fainting (Xue Yun). It is essentially a wind syndrome; administer Xionggui Tang (Ligusticum and Angelica Decoction) made of equal parts Chuanxiong and Danggui, decocted in water.
For summer traumas, regardless of severity, first administer Wuling San (Five Ingredent Powder) before taking trauma-specific medicines. Wuling San consists of Zhuling (Polyporus), Baizhu (Atractylodes Macrocephala Rhizome), Chiling (Alisma Rhizome), Zexie (Alisma Fruit), and a small amount of Guigui (Cinnamon Twig), decocted in water.
For traumatic hematuria, first administer Wuling San with Qu Mai (Dianthus Herb), Cheqian (Plantago Seed), Mutong (Akebia Stem), and Huashi (Talcum), removing Rougui (Cinnamon Bark) to form Siling San (Four Ingredient Powder). Decoct in water.
For nasal fractures with bleeding from the seven orifices, first apply moxibustion with mugwort wool, then massage the fifth thoracic vertebra near Dazhui (GV14). After moxibustion, take Xiaofeng San with Maomaohua (Rice Flower) or roots. (Moxibustion is applied at Renzhong GV26).
For severed tendons in limbs, first apply Shenmeng San to the axilla and back of the hand to cut off blood supply, then use hemostatic wound-closing agents. Subsequently, take Xiaofeng San with San Gui and Zouma (Cantharis). Healing will be facilitated.
For sprains with blood and qi attacking the heart, use Liushen Muxiang Tang (Six Divine Sandalwood Decoction) with San Gui added. For severe immobility, grind Xiaofeng San in wine. Apply warming compresses to the chest for recovery.
For deep wounds in necrotic tissue, cover the wound with Jinqiang Yao and hemostatic powder to stop bleeding and promote closure without additional medications or wind avoidance. Clean the wound with breast milk after some days, apply Shengji San fully, and wash once daily. Avoid wind and rain when changing dressings. Apply black umbrella paper to the skin, then cover with black plaster. Remove the paper upon healing.
For closed limb fractures/contusions, apply Yulong Gao (Jade Dragon Ointment) to the injury, bind with cloth, and splint with cedar bark. Take Xiaofeng San with half a cup of good wine, and Ruxiang Xuntong San (Frankincense Seeking Pain Relief Powder) with Zouma, mixed with wine and ginger juice. Repeat fomentation and splinting every two to three days for natural recovery. These fomentation powders and decoctions must not be disseminated freely.
For open limb wounds with bleeding, qi has leaked; fomentation is unnecessary. Cover the wound with Jinqiang Yao. Splint as before, take Xiaofeng San with San Gui and Zouma. Wash the wound with pure sesame oil every two to three days and change dressings.
For foul-smelling wounds, apply processed Tianji Luo (Snail with Horns, dried and powdered) to eliminate odor.
When both acute injury and chronic damage are present, treat the acute injury first, or treat both simultaneously. To stop bleeding urgently, use Xiaofeng San with San Gui for pain. Add Chenxiang and Muxiang for qi stagnation. Subsequently apply Yulong Gao to the damaged area; this is a skilled technique.
For rotting flesh with maggots, use 5-5 qian of Taoren (Peach Kernel), fried and pounded into a cake. Apply to the empty wound cavity to extract maggots. If Taoren is unavailable, use tender leaves dried on tiles, powdered, and mixed with sesame oil.
For trauma-induced convulsions with opisthotonos (arched back), urgently moxibustion Fontan, Hegu, Yongquan (KI1), and Baihui. If ineffective, seek careful diagnosis. For locked jaw, blow Tongguan San into the nose; if sneezing occurs, treatable with Xiaofeng San. No sneeze implies untreatability. For opisthotonos, place thin locust bark on the wound and moxibustion until moisture appears; continue with Xiaofeng San.
For eye injuries, manually realign the eyeball. Apply Shenmeng San mixed with egg white around the orbit. If "gall water" leaks, the eye is ruined and incurable.
For falls disrupting Lung and Liver, causing qi-blood attack and aphasia, have the patient lie supine for an hour. Apply warming compresses to the chest and take Ruxiang Xuntong San for recovery.
For trauma-induced vomiting preventing medication intake, due to cold invasion, take fresh ginger juice mixed with honey, steamed and administered, to stop regurgitation. Salted ginger is beneficial for diet.
For unconsciousness from falls, blows, or cuts, first administer Muxiang Tang orally, allow nature to take its course, then assess the wound and medicate accordingly. Use hemostatics, oil-mixed medicines, or pain-relieving Xiaofeng preparations; physicians must adapt flexibly.
For severe chopping injuries with swelling, pain, abdominal distress, and bone fractures, urgently administer pain-relieving medicine. Reduce dislocated joints, apply medicine to painful areas, and splint with cedar bark. Ensure splints are neither too loose (bone movement) nor too tight (circulation blockage). Moisten dry medicine with ginger juice. Use Huoxue Zhutong San (Invigorate Blood and Stop Pain Powder) to eliminate old tissue, generate new, reduce swelling, stop pain, set bones, and invigorate blood. Take 2 qian four times daily with ginger juice and wine; recover in 1-2 days.
For bruising entering the chest/diaphragm with abdominal distension, shortness of breath, poor appetite, mental confusion, limb rigidity, pain, and coma approaching critical state, urgently administer Wutong Wan (Five Transport Pill) to promote evacuation. Follow with Huoxue Zhuntong San, several doses, decocted with ginger as a guide.
For elderly or young patients with fall-induced lumbar pain, abdominal distension, stabbing pain, and urinary/fecal retention, urgently administer Wutong Wan for unblocking, followed by Ruxiang Xuntong San.
For knife wounds infected by water dampness causing edema, alternating tidal fever, and coma approaching critical state, Xiaofeng San is most effective. Take with wine and ginger. Apply Shengji San with sesame oil to the wound; apply Shenmeng San mixed with Haipiao Xiao (Cuttlefish Bone) and Piao Xiao, sweetened with honey, to surrounding swelling. Subsequently take Huoxue Zhuntong San.
For gun/knife wounds with excessive bleeding, swollen painful wounds, coma, tidal fever, anorexia, limb weakness, nausea, worsening at night, and refractory symptoms, urgently use Huoxue Zhuntong San. Decoct in water, boil briefly, add wine, strain, and take warm. If wounds are swollen/painful with irritability, remove Houpo and Rougui, decoct in water only, and take on an empty stomach.
For broken skin, stop bleeding with Taohua San (Peach Blossom Powder), apply Shenmeng San, take Huoxue Zhuntong San, wash with Xinxiang San (Fragrant Powder), and regenerate flesh with Baijin San (White Gold Powder). Physicians using these methods rarely fail.
For pus-filled, rotting, foul-smelling wounds, wash with Xinxiang San plus a pinch of salt decocted in water.
For healed but weak limb fractures with residual pain, take Ruxiang Xuntong San with Zouma.
For purple discoloration of muscle from bruising, apply Banxia (Pinellia) powder mixed with cold water, then remove. Apply Shengxing (Raw Arisaema) powder similarly to restore normal color. Finally apply Shenmeng San.
For stripping injuries or spinal fractures with three-day constipation and chills, urgently use Wutong Wan for unblocking, then Huoxue Zhuntong San. If complications arise (fever, nausea, qi rebellion), take the attached Xiaofeng San formula, followed by Ruxiang Xuntong San with Zouma for daily recovery. Apply Shenmeng San and Wanying Gao (Universal Plaster) to the site.
For initial trauma, check for periorbital ecchymosis ("raccoon eyes"). For head/scalp/fontanelle/skull fractures, treat urgently. Dispel wind, relax locked jaw, take Xiaofeng San to withstand wind/cold/tidal fever. Add San Gui for pain; add Jiangcan (Silkworm), Tianma (Gastrodia), and Quanxie (Scorpion) for severe wind. For all open wounds, use Taohua San for hemostasis, bind with silk, avoid wind. Urgently apply Baijin San mixed with sesame oil on cloth, incubated; check if wound is cool to verify physician's presence. Change Xiaofeng San dressing daily (every three days in cold weather). Keep wound closed from wind/water to prevent difficult-to-treat tidal fever. Take Ruxiang Xuntong San with ginger/wine for pain anytime.
For skull fractures, apply fresh ginger juice mixed with wine and Baijin San, plus Huaisan San, to the injury site for immediate relief. Take Ansui San (Calming Marrow Powder) with green tea. For loss of voice, use nasal insufflation powder to induce sneezing; smoke burning Zhu Ya Zao Jiao (Tariptus Fructus) into the nose. If wind is present, add Jiangcan and Quanxie. Otherwise, locked jaw may be fatal; exercise caution.
For intestinal bleeding/separation and abdominal distension from trauma, take Wutong Wan. If diarrhea persists, eat congee to stop it. If still persisting, decoct roasted Da Fuzi (Aconite Lateral Root) with ten slices of ginger. Alternative: grind Shichangpu (Acorus Rhizome) in water, use well water to wash face and hands to stop bleeding. Treat carefully based on specific injuries; do not adhere rigidly. This condition is difficult to treat.
For cane wounds, use Baijin San. If jaw locks, insufflate nasal powder to induce sneezing for easier treatment. Do not let women see cane wounds at home, as they hinder healing. Avoid sleeping with women; abstain for recovery.
For qi-stagnation lumbar pain in men and women, take Junjiang Yunqi San.
(The above methods are general guidelines. Physicians must adapt flexibly according to circumstances, not adhere rigidly. Study carefully daily.)
