两肩骨出者,如患左手,医者以右手入患人左手按之;
The cranial bones are the gathering point of all Yang meridians, including areas such as the Taiyang region (region of the temporalis muscle and surrounding area), fontanelle, and skull cap. If these areas are injured and broken, it is a matter of life and death. The physician must part the patient's hair to locate the injury, cut away the hair near the wound, and then apply medication. For severe injuries, Taohua San (Peach Blossom Powder) and Jiedixin (Juncus Effusus, 灯心草) should be used to pack the wound; for minor injuries, less is needed. Inspect the wound daily. If it becomes rotten and foul-smelling, wash it with Xinfang San (Fragrant Powder) and apply Shengji San (Flesh-Regenerating Powder) mixed with an appropriate vehicle. During treatment, avoid exposure to wind, lest blood stasis generate wind, causing fever and swelling of the head and face. Apply Yi Yao (Warming Compress Medication). If symptoms persist, apply Ruxiang Xuntong San (Frankincense Pain-Relieving Powder) and add Chuanwu (Aconitum Carmichaelii Dehanchet Root, 川乌) externally, while internally administering Xiaofeng San (Wind-Dispersing Powder). Additionally, apply Baijin San (Platinum Powder) mixed with refined oil to the affected area. An Sui San (Marrow-Anchoring Powder) may be taken internally, mixed with refined oil.
The face has seven orifices, and the eyes are one of them; eye injuries are the most difficult to treat. If beaten such that the eyeball protrudes outside the orbit, it is extremely difficult to push it back in. Apply Shensheng San (Divine Holy Powder) mixed with honey to the area and allow nature to take its course. If the cornea (black pupil) is ruptured and the aqueous humor (gall water) has leaked out, the pupil will inevitably be damaged. However, if the eyeball remains within the orbit, it can be gently manipulated back into position. Apply Shensheng San mixed with honey, and internally administer Huoxue Zhutong San (Blood-Invigorating and Pain-Stopping Powder) mixed with clear tea for recovery.
If the gum bone is fractured by impact, first manually reduce the fracture to realign the bones. Apply Shensheng San externally, then use a cloth bag to support the chin and tie it securely to the hair. If the gum tissue has detached and is swinging loosely, use tendons to manipulate it back into place. If bleeding does not stop, use Taohua San to arrest bleeding, and hold Baijin San mixed with decoction in the mouth to achieve efficacy.
In cases of falling from a height resulting in cervical spine injury, have the patient lie supine. Use a silk cloth to support the submandibular region. Untie the patient's hair, gather it into a single bunch, and grasp it firmly. Extend the patient's legs, step on their shoulders, and apply gentle, steady traction until reduction is achieved. Over-traction may cause excessive extension and laxity. Apply Shensheng San mixed with fresh ginger juice and wine. Regularly administer Ruxiang Xuntong San for complete recovery.
For clavicle fractures, manually reduce the fracture to realign the bones. Place two cotton pads, sized like fruits, on either side of the fracture to gently support and approximate the bone ends. Apply Shensheng San mixed with ginger juice to the site. If the skin is broken and ulcerated, do not use ginger juice; instead, use honey. Cut a section of bamboo tube to appropriate length and width, crush it slightly. Insert a large piece into the bone structure, pack paper around the axilla inside the bamboo, and bind with a silk bag. Administer Ruxiang Xuntong San daily.
For scapula/clavicle fractures where displacement occurs externally due to trauma, extend the arm. The physician palpates the injury site to reduce and realign the bone. Apply Shensheng San. Bind a silk bag from the axilla across to the opposite shoulder. Administer Huoxue Zhutong San daily. If the bone is displaced superiorly, the physician inserts their left hand into the patient's right armpit and uses their knee to brace the lateral ribs, allowing the bone to spontaneously reduce.
For bilateral shoulder dislocations: if the left arm is injured, the physician uses their right hand to press the patient's left arm; if the right arm is injured, the physician uses their left hand. If the humeral head is displaced inferiorly, lift it upward to reduce. Rotate the arm until the head reaches the level of the brain/skull base. Apply Shensheng San. Bind a silk strap from the shoulder to the opposite axilla, and then to the opposite shoulder. Administer Huoxue Zhutong San.
For wrist joint dislocation, the treatment method is the same as for the shoulder. Reduce and realign, then apply Shensheng San. Use a large piece of fir bark with a hole carved in the center, divided into four sections facing each other, similar to previous methods, to allow wrist flexion and extension. Secure with a silk bag around the head. Administer Ruxiang Xuntong San daily without error.
For forearm bone fractures, manually reduce the fracture fragments. Apply Shensheng San. Then use three pieces of fir bark, scraping off the rough outer bark and scoring them densely until slightly broken, ensuring appropriate length. If the fracture is inward-facing, place three short pieces inward and one long piece outward as splints. Bind with four layers of cotton. Loosen the binding towards the body to ensure qi and blood circulation. The second layer should be tighter to promote blood flow; if adjacent pieces are too tight, blood flow is obstructed, hindering bone healing. Administer Huoxue Zhutong San daily. After three days, change the splints, shaving them thinner than before, and loosen the bindings slightly. If swelling is present, add Bieqiao (Sepia Efficax, 海螵蛸) and Poxiao (Natrii Sulfas, 朴硝) to Shensheng San. If blisters form around the periphery, stir-fry Baicaoshuang (Soot from Hundred Herbs, 百草霜) until no smoke is produced, spread on paper on the ground to preserve properties, grind into powder, and apply mixed with refined oil to the blisters.
Hand bone reduction follows the same method as forearm bone reduction.
Palmar carpal bone reduction follows the same method as wrist joint reduction.
