Case 5: This is a case of measles complicated with noma (gangrenous stomatitis). Both the internal and external treatment prescriptions are worthy of reference. The contemporary expert Ye Juquan advocated using Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) or Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) with modifications for this condition, which also yielded good results.
(V.) Pediatric Wind-Warmth
A young child suffered from fever for eight days, with inadequate sweating, coughing with excessive phlegm, restlessness and oppression, nausea, and convulsions resembling wind-stroke. The stool was loose, extremities were slightly cold, tongue coating was thin, greasy, and yellow, and pulse was slippery and rapid but not vigorous. The previous physician treated it as slow wind-stroke, using Ginseng, Atractylodes, Poria, Pinellia, Fritillaria, [Tooth/possible error], Hyriate Shell, and Uncaria. However, the restlessness and nausea worsened. This is due to latent wind-warmth pathogens invading and accumulating in the Lung and Stomach, stored in the meridians, unable to vent outward, showing signs of internal collapse. If Lung pathogen is not resolved, it transfers to the Large Intestine causing loose stools; if Yangming pathogen does not reach out, Yang qi fails to circulate causing cold limbs. This cannot be equated with slow wind-stroke. Moreover, slow wind-stroke is a deficiency syndrome; how could there be restlessness? Even if present, Shaoyin pulse and symptoms should appear. Given the various pathological mechanisms, there may be an internal threat of measles. Urgent measures should be taken to disperse and vent, hoping for recovery. Prescription: Schizonepeta Spike 4.5g, Kudzu Root 6g, Cicada Slough 2.4g, Mint 2.4g, Platycodon Root 2.4g, Fermented Soybean 9g, Charred Honeysuckle Flower 9g, Forsythia Fruit 4.5g, Red Poria 9g, Immature Bitter Orange Char 4.5g, Stir-fried Bamboo Shavings 4.5g, Agastache Stem 4.5g.
Second visit: After taking the dispersing and venting decoction, profuse sweating occurred, and restlessness and nausea significantly decreased. Faint red spots appeared on the cheeks, forehead, neck, and chin, indicating the emergence of measles. Coughing with excessive phlegm persisted, body heat did not subside, tongue body was red, coating was thin, greasy, and yellow, and pulse was slippery and rapid. The latent warmth pathogen had a tendency to emerge outward, but Lung and Stomach Qi was obstructed and unventilated. Continue with acrid-cool clearing and resolving, diffusing Lung and transforming phlegm, hoping that once measles erupts and heat subsides, it will be auspicious. Remove Fermented Soybean from the original prescription and add Purple Floating Duckweed.
(From *Ding Ganren's Medical Cases*)
[Commentary] This case was misdiagnosed as slow wind-stroke; in reality, it was latent wind-warmth pathogens invading the Spleen and Stomach, with a tendency toward internal collapse. Ding considered that the child might have measles that failed to erupt, so he adopted dispersion and ventilation as the major principle. Throughout this case, the treatment principle remained acrid-cool clearing and resolving, diffusing Lung and transforming phlegm. The formula used was Yin Qiao San (Forsythia and Honeysuckle Decoction) with modifications, featuring precise compatibility and clear indications.
(VI.) Pediatric Damp-Warmth
Case 1: Son of Zhang XX, 3 years old. First visit: Damp-warmth for twelve days. Pulse at both hands was wiry and rapid. Tongue coating was dry and yellow. Thirst with frequent drinking. Epistaxis. High fever like burning. Head shaking and eyes staring. Unfixed traction and convulsion of limbs. Restless and uneasy spirit. Inadequate coughing. White miliary rash faintly visible but indistinct. This condition initially involved pathogen invasion of Lung and Stomach, then transmitted to Shaoyin and Jueyin meridians. Currently, Liver Yang is rampant. The illness is severe and dangerous, placing treatment in a dilemma: releasing the exterior would aid fire and cause spasms/convulsions, while cooling would trap heat and obscure the orifices. Prescribe calming Liver and extinguishing wind, acrid-cool opening orifices and venting, hoping for heat reduction and pathogen resolution. Prescription: Zixue Dan (Purple Snow Pill) 0.6g (delivered with Centella asiatica soup), Chuan Bei Mu and Xiang Bei Mu 6g each, Mint Leaf 2.4g (added later), Abalone Shell 15g (decocted first), Hyriate Shell 2.4g, Forsythia Fruit coated with Cinnabar 9g, Uncaria Hook 9g (added later), Apricot Kernel 9g, Poria coated with Cinnabar 9g, Cicada Slough 2.4g, Trichosanthes Peel 9g, Honeysuckle Flower 9g, Curcuma Aromatic Rhizome 4.5g, Imperata Rhizome 9g (text says "kou ke", likely typo for amount, interpreted as appropriate dose), Fresh Lotus Leaf one piece, Gold utensil one piece decocted as water substitute.
Second visit: Previous methods of calming Liver, extinguishing wind, acrid-cool opening orifices and venting resulted in the white miliary rash emerging smoothly, which is a favorable sign. However, crying without sound, absence of tears and nasal discharge, occasional restlessness, coughing without phlegm, head shaking, eyes staring, and unfixed limb convulsions persist. Liver Yang remains rampant, caused by intense heat in Lung, Stomach, Heart, and Liver meridians. Follow the original plan and advance further. Prescription: Abalone Shell 24g (decocted first), Acorus Tatarinowii Rhizome 2.4g, White Akebia Stem 2.4g, Poria coated with Cinnabar 9g, Forsythia Heart 9g, Xiang Bei Mu 9g, Zixue Dan 9g, Hyriate Shell 2.4g, Curcuma Aromatic Rhizome 4.5g, Yi Yuan San 15g (wrapped), Uncaria Hook 9g (added later), Scutellaria Baicalensis Root 4.5g, Honeysuckle Flower 9g, Trichosanthes Peel 9g, Jing Chuan Bei 6g (core removed), Apricot Kernel 9g, Imperata Rhizome 60g, Antelope Horn Slice 0.9g, Gold utensil one piece decocted together as tea.
Third visit: Yesterday, spiritual orifice-opening and clearing Metal purifying Lung methods reduced all symptoms. Modify the previous prescription (details omitted). Fourth visit: Use sweet and harmonizing methods to harmonize Stomach, assisted by clearing Metal. Prescription: Dendrobium 6g, Tangerine Reticulated Vein 2.4g (text says 24g, likely typo), White Poria 4.5g, Eupatorium Fortunei Leaf 4.5g, Yi Yuan San 9g, Coix Seed Husk 6g, Xiang Bei Mu 6g, Raw Coix Seed 9g, Stir-fried Millet Sprout 9g, Uncaria Hook 9g (added later), Stir-fried Barley Sprout 9g, Fresh Lotus Stem 2 chi (remove thorns), White Akebia Stem 2.4g.
(Organized by Zhang Zanchen: Zhang Boxi's Medical Cases, *Shanghai Journal of Traditional Chinese Medicine* 3:31, 1962)
Case 2: Damp-warmth for half a month. Body heat fluctuates with sweating. White miliary rash emerges continuously. Spirit is fatigued and irritable. Limbs are cold. Nausea. Loose stools. Thirst but little desire to drink. Tongue coating is thin. Pulse is soft and fine. This indicates insufficiency of Qi and Yang, with residual pathogens lingering. Prescription: Silver Twig 9.5g, Artemisia Annua 9g, Prepared Aconite Root 9g (decocted first), Live Magnetite 30g (decocted first), Cinnamon Twig 2.4g, White Peony Root 4.5g, Poria coated with Cinnabar 9g, Pinellia Ternata 9g, Tangerine Peel 4.5g.
(Organized by Xu Zhongcai: Introduction to Pediatric Experience of Dr. Xu Xiaopu, *Shanghai Journal of Traditional Chinese Medicine* 4:5, 1962)
Case 3: Child Chen, damp-warmth for seventeen days. White miliary rash emerges continuously. Sweating does not resolve the condition. Phlegm rattle and rapid breathing. Gingival rot and foul breath. Coma and deviated eyes. Limb tremors. Urine is clear and abundant. Tongue coating is yellow and greasy. Pulse is slippery and rapid. Pathogen has internally collapsed; difficult to save. Prescription: Ephedra Herb Honey-fried 2.4g, Raw Gypsum 12g (decocted first), Prepared Aconite Root 4.5g (decocted first), Coptis Chinensis Rhizome 3g, Apricot Kernel 12g, Hyriate Shell 9g, Live Magnetite 30g (decocted first), Dragon Tooth 30g (decocted first), Tangerine Peel 4.5g, Poria 12g, Gastrodia Elata 6g, Scorpion Tail 2 pieces, Dry Acorus Tatarinowii Rhizome 9g.
(Organized by Deng Jiacheng: Pediatric Cases of Dr. Xu Xiaopu, *Chinese Journal of Traditional Medical Science and Technology* 4:26, 1965)
[Commentary] Pediatric damp-warmth easily damages fluids and consumes essence, producing complications more readily than in adults. Therefore, treatment must be timely and accurate. Case 1 involves damp-warmth transforming into dryness, blazing in Yangming, consuming Yin and stirring blood, leading to epistaxis and spasms; this is a adverse progression. Treatment must provide an exit for the warm pathogen for the child to survive. Thus, the first visit focused on Lung, Stomach, Heart, and Liver meridians, primarily using methods to calm Liver, extinguish wind, acrid-cool open orifices, and vent, to dissipate heat. The key point is that after medication, the pathogenic heat must vent and the clear orifices must open. Dr. Zhang excels in warm diseases; his prescriptions grasp the main contradictions in pathology, with complete theory, method, formula, and herbs, and profound meaning in compatibility and usage. In Case 2, the appearance of white miliary rash was due to excessive sweating damaging Yang Qi, making it difficult for external pathogens to vent. Thus, Aconite was used promptly to support Yang. According to Dr. Xu's experience, in late-stage cases with deficiency of healthy Qi and lingering pathogens, low-grade fever persistence, combining Aconite with Silver Twig and Artemisia Annua yields significant effects.
Case 3 is a severe damp-warmth case where pathogen collapses into Jueyin, stirring liver wind, with signs of internal closure and external collapse, yet urine is clear and abundant. Dr. Xu seized upon "clear urine" as an indicator of declining Qi and Yang, using Aconite to support healthy Qi and expel pathogens, suppress Yin and transform Yin, combined with drugs to diffuse Lung, drain heat, settle fright, and extinguish wind. Cold and hot properties are used together; Aconite and Gypsum are used simultaneously, one to support Yang and the other to restrain hyperactivity, matching the pathology of internal closure and external collapse.
(VII.) Epidemic Encephalitis B
Case 1: Wang Mou, Male, 9 years old, admitted to a certain hospital on August 23, 1956. Diagnosed with Epidemic Encephalitis B. Hospital examination: (Omitted). Course and treatment: Onset on August 19 with high fever, headache, and somnolence. Next day, consciousness became unclear. Admitted on the 23rd, already comatose. Temperature 39.6°C. No sweat, red eyes, no bowel movement, yellow urine. Pulse floating, flooding, and forceful. Tongue coating yellow and greasy. Clearly a case of equal severity of Summer-heat and Dampness. Appetite recovered, remaining symptoms cured. Discharged next day.
(From *Pu Fuzhou's Medical Cases*)
Case 2: Huang Mou, Male, 3 years old, admitted on August 20, 1958. Medical Record No.: 29303. Confirmed diagnosis: Epidemic Encephalitis B. Upon admission, high fever reached 40°C. Sweating, thirst, red face, dry lips, vomiting. Tongue coating yellow and moist. Bowel movement twice daily, slightly loose. Pulse rapid. Right pulse large, Left pulse small (Text "Right Tai Zi Left" is likely a typo for "Right Large Left Small" or similar). Believed Summer-heat pathogen had entered Yangming Qi level. Prescribed heavy acrid-cool agent, Baihu Tang (White Tiger Decoction) with modifications: Raw Gypsum 45g, Anemarrhena Rhizome 6g, Dioscorea Rhizome 9g, Forsythia Fruit 9g, Rice Grain 9g, Roasted Licorice Root 3g. Morning of 21st, second visit: Fever rose to 40.5°C. Tongue yellow and greasy. Bowel movement three times daily, loose. Continued original prescription, increased Gypsum to 60g. Afternoon visit: Temperature rose to 40.9°C. Added Ginseng. Heat remained unchanged. Diarrhea persisted. Third visit on 22nd: Heavy doses of Baihu Tang used for two consecutive days. High fever not only did not subside, but diarrhea increased to four times. Startled by noise, rough breathing, nausea. Condition deteriorating. However, sweating, thirst, high fever, yellow tongue, and large, rapid pulse are indications for Baihu Tang. Why did symptoms worsen instead of improving after taking it? After deep contemplation, suddenly realized: Child's Renying pulse is rapid, red face, high fever, sweating, slight panting indicate exterior pathogen. Tongue yellow but not dry, nausea and upward rebellion, frequent loose stools indicate Summer-heat and Dampness accumulated in Spleen and Stomach. This is a case of diarrhea with accompanying heat. Previous repeated use of Baihu Tang to clear Yangming meridian heat committed errors of ignoring exterior pathogen and violating the taboo of Gypsum and Anemarrhena being cool and moistening, stirring dampness. No wonder high fever and diarrhea increased. Since the child has diarrhea with accompanying heat, it is purely a Gegen Qinlian Tang (Kudzu, Scutellaria, and Coptis Decoction) pattern. Urgently prescribe: Kudzu Root 12g, Scutellaria Baicalensis Root 9g, Coptis Chinensis Rhizome 1g (Text "1 Shi Ke" is likely a typo for 1g or 3g, interpreted as 1g based on context), Licorice Root 3g. After just 2 doses, heat reduced to 39.4°C; after another 2 doses, reduced to 38.8°C. Bowel movements improved, nausea stopped, quickly recovered and discharged.

[Commentary] The clinical manifestations of "Encephalitis B" belong to the category of Warm Diseases in TCM, commonly diagnosed as Summer-heat Warmth, Latent Summer-heat, or Wind-summer-heat. Since 1954, when Guo Keming et al. at Shijiazhuang Infectious Disease Hospital achieved good results treating this disease mainly with Baihu Tang (White Tiger Decoction), many units nationwide learned and promoted this experience, with some variations and developments. The general treatment for Encephalitis B focuses on clearing heat, detoxifying, and nourishing Yin, while also eliminating pathogens according to the specific conditions of Wind, Summer-heat, and Dampness, treating based on etiology.
Case 1 is differentiated in TCM as equal severity of Summer-heat and Dampness, pervading the Triple Burner, with symptoms of high fever, headache, coma, diagnosed by Western medicine as "Encephalitis B". The condition is severe, so treated with heavy acrid-cool agents, Xinjia Baihu Tang (New Modified White Tiger Decoction), plus Scallion and Fermented Soybean to prevent internal invasion. After medication, heat went but dampness remained latent; still suitable for diffusion and venting. Pu further modified Erxiang Wan (Two Fragrant Pills) and Zhengqi San (Upright Qi Powder). After medication, dampness drained and heat vented, guiding pathogen outward; finally cured with clearing heat and harmonizing Stomach method. The whole case features careful differentiation and matching of syndrome and formula.
Case 2 is "Encephalitis B" with "diarrhea with accompanying heat". Initially, Yue believed Summer-heat pathogen had entered Yangming Qi level and treated with modified Baihu Tang, leading to deterioration. After careful differentiation, it was determined to be exterior pathogen with Summer-heat and Dampness accumulated in Spleen and Stomach, diagnosed as diarrhea with accompanying heat, treated with Gegen Qinlian Tang (Kudzu, Scutellaria, and Coptis Decoction) to release exterior, clear interior, and stop diarrhea. From the treatments of Cases 1 and 2, the significant role of Zhongjing's formulas in treating this disease can be seen. In Case 2, Yue honestly admitted his initial differentiation was insufficient and medication inappropriate. Not hiding one's faults and seeking truth from facts is commendable.
Case 3 involves hyperactive Summer-heat, stirring internal Liver wind, Qi and Ying levels both blazing, pathogen transmitting to Pericardium. Liu used venting pathogen and washing away Summer-heat, clearing Ying and detoxifying, calming Liver and extinguishing wind, clearing Heart and opening orifices as main methods. Over several visits, formulas varied greatly. The characteristic was adding acrid-opening and diffusing agents to clearing formulas, feeding rice soup to assist Stomach and generate fluids, facilitating pathogen exit via sweat. Downward purging is generally contraindicated in Warm Diseases, but due to internal heat accumulation in this child, the purging method used contained the intent of increasing fluids. Thus, after medication, heat结 was released, Fu organ Qi circulated, and exterior Qi harmonized naturally.

[Commentary] This case presents typical symptoms of poliomyelitis. Guided by Warm Disease theory, Zhao differentiated it as latent warmth injuring meridians. Thus, the initial approach heavily used clearing heat, detoxifying, unblocking meridians, and activating collaterals, adding Zhibao Dan to awaken spirit, open orifices, remove filth, and detoxify. During treatment, agents for extinguishing wind and unblocking joints were incorporated for modification. The prescription balanced pathogen and healthy Qi, hitting the mark precisely.
