[Background Information]
Book: Shao Lansun's Medical Cases; Author: Shao Lansun; Dynasty: Qing; Chapter: Wind; Category: Medical Theory
Patient Shen from Yuzhuang recently contracted wind-pathogen, with cough being particularly severe. The pulse is floating and wiry (Fu Xian), the tongue coating is slippery and greasy, there is aversion to cold, and stabbing pain in the left axilla. Treatment should focus on clearing the Lung and harmonizing the collaterals (He Luo).
Prescription: *Bai Bu* (Radix Stemonae, 7 fen, steamed), *Jie Geng* (Radix Platycodonis, 1.5 qian), *Ju Hong* (Pericarpium Citri Reticulatae, 1 qian), *Jin Fei Cao* (Herba Agrimoniae, 3 qian, wrapped for decoction), Zhi Gan Cao (Honey-fried Radix Glycyrrhizae, 8 fen), *Zi Wan* (Radix Asteris, 1.5 qian), *Jing Jie* (Herba Schizonepetae, 1.5 qian), *Si Gua Luo* (Retinervia Luffae Fructus Retinervia, 3 qian), *Xiang Bei Mu* (Bulbus Fritillariae Thunbergii, 3 qian), Guang Yu Jin (Radix Curcumae, 3 qian), *Qian Hu* (Radix Peucedani, 1.5 qian, as a guide drug), *Pi Pa Ye* (Folium Eriobotryae, 5 leaves, de-hairy).
Take two doses.
Commentary by Jie: In treating cough due to wind-cold, one must first use acrid-dispersing and light-opening agents to ventilate the Lung and resolve phlegm, so that pathogens entering from the exterior may exit via the exterior, thereby restoring the clear function of the Lung Qi.
In this case, the cough is particularly severe. The treatment focuses on clearing the Lung and dispersing wind, while using *Xuan Fu Hua* (Flower Inulae) and *Si Gua Luo* to harmonize the collaterals. This is an appropriate prescription for cough accompanied by axillary pain.
Patient Yu from Yifeng has wind-pathogen latent in the Lung, with chronic cough exacerbated by fatigue, and a history of hemoptysis. The pulse at the Cun position is floating and slippery, and the voice is hoarse. Treatment should clear the Lung, disperse wind, and resolve phlegm, preventing progression to consumptive disease. (April 5th, Gui Mao 18th day)
Prescription: *Zi Wan* (Radix Asteris, 1.5 qian), Guang Xing Ren (Semen Armeniacae Amarum, 3 qian), *Gua Lou Pi* (Pericarpium Trichosanthis, 1.5 qian), *Su Geng* (Caulis Perillae, 1.5 qian), *Ju Luo* (Medulla Citri Reticulatae, 1.5 qian), *Bai Qian* (Rhizodon Cynanchi Stauntonii, 1.5 qian), Chuan Bei Mu (Bulbus Fritillariae Cirrhosae, 2 qian), Fo Er Cao (Herba Petasitae, 3 qian), *Jing Jie* (Herba Schizonepetae, 1.5 qian), Du Dou Ling (Fructus Datura, 1 qian), Sheng He Zi (Semen Terminaliae Chebulae, 1.5 qian), Fresh *Pi Pa Ye* (Folium Eriobotryae, 3 leaves, de-hairy).
Take three doses.
Commentary by Jie: Cough arises from wind-cold invading the Lung. The Lung is a delicate organ; any misapplication of medication can cause harm. If one uses cloying and astringent herbs such as Shu Di Huang (Prepared Radix Rehmanniae), Mai Dong (Radix Ophiopogonis), Yu Rou (Fructus Corni), and Wu Wei Zi (Fructus Schisandrae) to tonify, it will trap the external pathogen, inevitably leading to hemoptysis and aphonia, as stated by Xu Huixi. In this case, due to wind-pathogen latent in the Lung and chronic cough leading to hemoptysis and hoarseness, it is likely that previous physicians mistakenly used cloying tonics, or the patient’s family erroneously administered nourishing foods, gradually transforming a common cold into a consumptive disorder through mistaken tonification. Treating with agents that clear the Lung and disperse wind addresses the root cause.
Patient Pang from Yifeng has had chronic cough. There is dryness of the throat, hoarseness, a wiry pulse, slightly white tongue coating, and headache. Temporarily, treatment should clear the Lung and disperse wind.
Prescription: *Sang Ye* (Folium Mori, 3 qian), *Jie Geng* (Radix Platycodonis, 1.5 qian), Du Dou Ling (Fructus Datura, 1 qian), *Si Gua Luo* (Retinervia Luffae Fructus Retinervia, 3 qian), Xing Ren (Semen Armeniacae Amarum, 3 qian), Sheng Gan Cao (Raw Radix Glycyrrhizae, 5 fen), Guang Ju Hong (Pericarpium Citri Reticulatae, 1 qian), Pang Da Hai (Semen Sterculiae Lychnophorae, 3 qian), Gan Ju (Flos Chrysanthemi, 2 qian), Chuan Bei Mu (Bulbus Fritillariae Cirrhosae, 1.5 qian), *Bai Qian* (Rhizodon Cynanchi Stauntonii, 1.5 qian), *Pi Pa Ye* (Folium Eriobotryae, 3 leaves, de-hairy).
Commentary by Jie: When wind-pathogen resides in the Lung, it inevitably causes rebellious Qi and cough. If diet is regulated and daily routine is regular, and if one takes formulas that disperse wind and clear the Lung, the cough will resolve naturally. It is speculated that the patient’s family recklessly consumed tonifying foods, which aided phlegm production and suppressed the wind-pathogen. The pathogen remained in the Lung collaterals, generating heat over time, obstructing the airways with phlegm, leading to dry throat and hoarseness. This formula is based on Gan Jie Tang (Licorice and Platycodon Decoction), adding herbs that clear the Lung and calm the collaterals, truly embodying the principle of seeking the root in treatment.
Patient Shi from Yaqian has unresolved wind-pathogen. The left pulse is floating and slippery, with cough and rebellious Qi, thin white tongue coating, and slightly improved headache. Treatment should stop cough, resolve phlegm, and disperse wind.
Prescription: *Bai Bu* (Radix Stemonae, 8 fen), *Zi Wan* (Radix Asteris, 1.5 qian), Ma Dou Ling (Radix Aristolochiae, 1 qian), Dong Sang Ye (Folium Mori, 3 qian), Sheng Gan Cao (Raw Radix Glycyrrhizae, 7 fen), *Bai Qian* (Rhizodon Cynanchi Stauntonii, 1.5 qian), Ju Hong (Pericarpium Citri Reticulatae, 1 qian), Guang Xing Ren (Semen Armeniacae Amarum, 3 qian), *Jie Geng* (Radix Platycodonis, 1.5 qian), *Jing Jie* (Herba Schizonepetae, 1.5 qian), Chuan Bei Mu (Bulbus Fritillariae Cirrhosae, 1.5 qian, as a guide drug), *Pi Pa Ye* (Folium Eriobotryae, 3 leaves, de-hairy).
Take four doses.
Commentary by Jie: Wind-pathogen enters through the skin and hair, affecting the Lung internally, causing headache and cough. Therefore, treatment involves clearing the Lung, descending Qi, releasing the muscle layer, and dispersing wind.
Patient Shen from Anchang has incomplete resolution of wind-pathogen. The pulse is wiry, the tongue coating is thin and white, there is headache triggered by wind, and incessant hacking cough. Treatment should primarily clear the Lung and disperse wind.
Prescription: *Zi Wan* (Radix Asteris, 1.5 qian), *Sang Ye* (Folium Mori, 3 qian), Shi Jue Ming (Concha Haliotidis, 6 qian), Dai Zhe Shi (Haematitum, 3 qian), Chuan Xiong (Rhizoma Chuanxiong, 1 qian), Chuan Bei Mu (Bulbus Fritillariae Cirrhosae, 1.5 qian), Guang Ju Hong (Pericarpium Citri Reticulatae, 1 qian), Xing Ren (Semen Armeniacae Amarum, 3 qian), Gan Ju (Flos Chrysanthemi, 2 qian), Ci Ji Li (Spina Tribuli, 3 qian), *Bai Qian* (Rhizodon Cynanchi Stauntonii, 1.5 qian), Fresh *Pi Pa Ye* (Folium Eriobotryae, 3 leaves, de-hairy).
Commentary by Jie: External invasion of wind-pathogen, unresolved for a long time, combined with internal blazing of liver-gallbladder stagnant heat, leads to failure of Lung Qi to diffuse, resulting in incessant cough and headache triggered by wind. Thus, the treatment adds herbs to pacify the Liver and descend Qi among those that clear the Lung and disperse wind.
Patient Ye from Anchang suffers from wind-heat causing head distension, rapid pulse, red sclera (Qi Lun), exterior cold with interior heat, and palpitations. Clearing and dispersing is the safest approach. (September 12th)
Prescription: Dong Sang Ye (Folium Mori, 3 qian), Mu Zei Cao (Herba Equiseti Hiemalis, 1.5 qian), Ren Zhong Huang (Calcinatus Faecis Humani, 8 fen), Dan Zhu Ye (Folium Lophatheri, 1.5 qian), Jiao Shan Zhi (Fructus Gardeniae Preparatus, 3 qian), Xia Ku Cao (Herba Prunellae, 2 qian), Gan Ju (Flos Chrysanthemi, 2 qian), Mi Yin Hua (Flos Lonicerae Japonicae Honeyed, 3 qian), Rui Ren (Semen Pruni Armeniacae, 1.5 qian), Sheng Shi Jue Ming (Concha Haliotidis Raw, 6 qian), Guang Xing Ren (Semen Armeniacae Amarum, 3 qian).
Decoct in water, take two doses.
Re-evaluation: Wind-heat remains unresolved, pulse is slightly rapid and small, with intermittent sweating. Rising of Jue-Yang causes heat. Adhering to the previous method with modifications is appropriate. (September 17th)
Prescription: Dong Sang Ye (Folium Mori, 3 qian), Sheng Mu Li (Concha Ostreae Raw, 4 qian), Qing Xiang Zi (Semen Celosiae, 3 qian), Dou Pi (Exocarpium Citri Grandis, 3 qian), Gan Ju (Flos Chrysanthemi, 2 qian), Dan Pi (Cortex Moutan, 3 qian), Ci Ji Li (Spina Tribuli, 3 qian), Nu Zhen Zi (Fructus Ligustri Lucidi, 1.5 qian), Fu Shen (Poria Cum Radix Pini, 4 qian), Jiao Zhi Zi (Fructus Gardeniae Preparatus, 3 qian), Rui Ren (Semen Pruni Armeniacae, 1 qian).
Decoct in water, take three doses.
Re-evaluation: Tongue coating is slightly yellow, pulse is wiry, fine, and rapid, with unclear alternating chills and fever in the afternoon. Temporarily, treatment should primarily clear the Shaoyang. (September 22nd)
Prescription: Qing Hao (Herba Artemisiae Annuae, 1.5 qian), Bian Chai Hu (Radix Bupleuri, 3 qian - Note: 'Bian Chai Hu' likely refers to a specific preparation or variant, often interpreted as Chai Hu or similar), Dan Zhu Ye (Folium Lophatheri, 1.5 qian), Nu Zhen Zi (Fructus Ligustri Lucidi, 1.5 qian), Zhi Bie Jia (Carapax et Plastrum Testae Preparatum, 3 qian), Rui Ren (Semen Pruni Armeniacae, 1 qian), Ci Ji Li (Spina Tribuli, 3 qian), Tong Cao (Medulla Tetrapanacis, 1.5 qian), Dan Pi (Cortex Moutan, 2 qian), Sheng Shi Jue Ming (Concha Haliotidis Raw, 5 qian), Dong Gua Zi (Semen Benincasae, 3 qian).
Decoct in water, take four doses.
Commentary by Jie: This condition is caused externally by wind-heat contraction and internally by liver-gallbladder stagnant heat. The first formula clears heat and disperses wind. The second formula addresses rising Jue-Yang and intermittent sweating, appropriately modifying with Nu Zhen Zi and Dou Pi. The third formula recognizes that external pathogens have departed but Shaoyang stagnant heat remains, causing afternoon chills and fever; thus, it clears Shaoyang and pacifies liver heat. The methods are excellent.
Certain Patient: Leukorrhea and lower back pain persist without reduction. The pulse is floating and deficient, complicated by wind-pathogen, hacking cough, choppy pulse, and bleeding from gum spaces. Treatment should primarily clear the Lung and disperse wind. (February 29th)
Prescription: Shuang Sang Ye (Folium Mori Frosted, 3 qian), Chuan Bei Mu (Bulbus Fritillariae Cirrhosae, 1.5 qian), Du Zhong (Cortex Eucommiae, 3 qian), Wu Yuan Shen (Radix Scrophulariae, 3 qian), Jiao Zhi Zi (Fructus Gardeniae Preparatus, 3 qian), Su Geng (Caulis Perillae, 1.5 qian).
Commentary by Jie: Failure of the Dai Mai (Girdle Vessel) to secure and insufficiency of Kidney fluids lead to leukorrhea and lower back pain. Now, having contracted wind-heat again, the pathogen invades the Lung and Stomach, causing hacking cough and gum bleeding. This formula clears heat from the Lung and Stomach while tonifying the deficiency of Kidney fluids, reflecting the principle that one must eliminate disease before regulating menstruation.
