This refers to the condition where alveolar pyorrhea (alveolar suppuration) and carbuncle toxins, after ulceration, form lesions characterized by white pus exuding from the interdental spaces; it is extremely difficult to regulate and treat. For these two conditions of alveolar fistula (cavity leakage), one must abstain from alcohol, sexual indulgence, and all spicy, irritating, or toxic foods. For regulation and treatment, use Hupo San (Amber Powder) insufflated into the affected area. If tissue is necrotic, treat it similarly to alveolar fistula medications. The decoction should consist of herbs that nourish yin and descend fire. Insufflation requires patience; frequent application will naturally lead to resolution and recovery.

First, use Gansui San (Kansui Powder) with a small amount of processed Crotonis Fructus (Zhi Badou) insufflated into the lesion. Externally, use a tongue depressor to open the mouth, then insufflate the swollen area until it naturally subsides and heals, accompanied by oral decoction.

First apply Gansui San (Kansui Powder), then Liansha San (Cinnabar and Sand Powder), along with a decoction added with Gypsum Fibrosum (Shigao) and Forsythiae Fructus (Lianqiao).

The internal decoction should preferably contain herbs that support the spleen and clear heat. Externally, use Zhenzhu San (Pearl Powder): take fresh cotton wool the size of a finger, shape it into a strip, moisten with water, flatten it again, and dip it in the powder. If gingival recession persists, continue insufflating Zhenzhu San (Pearl Powder). Initially, use formulas that cool blood and clear the stomach, which applies to excess fire cases. If the gum roots are rotting due to deficiency-fire stirring from stomach deficiency, externally apply flesh-generating powders and internally take formulas that clear the spleen and descend fire.

Insufflate with Liansha San (Cinnabar and Sand Powder), combined with a decoction. If swelling is severe, add Gansui San (Kansui Powder) for insufflation. If there is necrosis, use medicines for oral ulcers, continuing until new flesh grows.

If the condition is severe, use Gansui San (Kansui Powder) for insufflation and take decoctions that cool blood, descend fire, and resolve toxicity. For mild cases, use Liansha San (Cinnabar and Sand Powder). If black necrotic tissue has broken down, combine with necrotic-tissue-removing formulas for insufflation. If there is no necrosis, or if necrosis has been completely removed, use flesh-generating medicines, adding Calculus Bovis (Niuhuang) and Concha Margaritifera (Zhenzhu), doubling the dosage of Terminalia Chebula Retzii Fructus (Ercha), and taking a decoction, before proceeding with cooling [treatments].

Insufflate Liansha San (Cinnabar and Sand Powder) onto the root of the tongue, and use a decoction added with Rhinocerotis Cornu (Xijiao), Coptidis Rhizoma (Chuanlian), and Gardeniae Fructus (Shanzhi).

For Yinyang San (Yin-Yang Powder), first wipe the area with slices of fresh ginger, then apply the powder. Use a heavy dose of Liansha San (Cinnabar and Sand Powder) and Gansui San (Kansui Powder) for insufflation. The decoction should include Coptidis Rhizoma (Chuanlian), Gardeniae Semen (Zhiren), and cooked with Flywheel Maypop Root Soil Water (Zaoxintu) prepared by wet-pulverization for oral administration.

Apply Menthae Haplocalycis Herba (Bohe) and Borneolum Syntheticum (Bingpian) as powder, and take Decoctio Junci (Dengxin Tang) along with the decoction.

Use the nasal fumigation formula; fumigating the tongue with smoke will cause symptoms to subside. Take a heart-clearing decoction and simultaneously insufflate Liansha San (Cinnabar and Sand Powder).

Grind Sinapis Semen (Jiecaizi) into powder, mix with vinegar, and apply to the neck. Internally take heart-clearing formulas.
