开,俗名大头瘟、虾蟆温者是也。加减普济消毒饮主之,或用代赈普济散,一日五六服,或咽下或含漱,最效;
可晒干再研,不可火烘),以流去毒涎,接吹锡类散(象牙屑、焙珍珠粉各三分,飞青黛六分,梅花冰片三厘;
The fourth type involves the combination with dampness, named Damp-Warmth (Shi Wen), also known as Damp-Heat. The fifth type involves the combination with dryness, named Warm-Dryness (Wen Zao), also known as Dry-Heat. In essence, these are syndromes of Damp-Fire and Dry-Fire, which have been detailed in the previous chapter on "Warmth is Latent Fire," so no further elaboration is needed here. However, Dai's original treatise states: Latent pathogens often cause sweating; when combined with Summerheat, there is even more sweating, thus the exterior must be deficient. Therefore, acrid-releasing herbs should not be used recklessly. As for Damp-Heat, it is most suitable to employ methods that separate and drain dampness while drying the Spleen. Akebiae Caulis (Mutong) is the best choice, followed by Talcum (Huashi), then Polyporus (Zhu Ling), Poria cum Rubro (Chi Ling), and Alismatis Rhizoma (Zexie). This is because draining dampness allows both dampness and heat to exit through the clear pathways, providing an outlet for the pathogen. This discussion is truly enlightening for future students.
The sixth type involves the combination with toxicity, named Warm-Toxicity (Wen Du), also known as Heat-Toxicity, commonly referred to as Seasonal Toxicity (Shi Du). There are distinctions between Wind-Toxicity and Filthy-Toxicity.
Wind-Toxicity refers to Wind-Warmth Seasonal Toxicity, with a severity greater than various other Warmth syndromes. The treatment method should be divided into three categories:
1. Warm-Toxicity affecting the cheeks and parotid glands (Mumps and submandibular lymphadenitis): Initially, there is sore throat and swelling of the larynx, swelling around the ears and temples, swelling of the cheeks, and redness of the face. Alternatively, the throat may not hurt, but external swelling occurs; in severe cases, deafness, trismus (lockjaw), and difficulty opening the mouth occur. This is commonly known as Big Head Plague or Toad Warmth. The primary formula is Modified Puji Xiaodu Decoction (Jianzhi Puji Xiaodu Yin), or one may use Daizhen Puji Powder, taken five to six times a day, either swallowed or used for gargling, which is most effective. Jingfang Baidu San (荆防败毒散) added with Renfen Qing (Jin Zhi, human feces supernatant) is also excellent. For external swelling, apply Shuixian Gao (Narcissus Ointment). If small yellow blisters like millet grains appear on the skin after application, Shuixian Gao should no longer be used; excessive application causes severe pain and ulceration. Instead, apply Sanhuang Erxiang San (Three Yellows and Two Fragrances Powder). If heat-toxicity is blazing, causing coma and delirium, cooling detoxification and aromatic opening of the orifices are required, such as Wu's Liangxue Jiedu Tang (Cooling Blood and Detoxifying Decoction) or Fei's Qinghuo Jiedu Tang (Clearing Fire and Detoxifying Decoction), added with Guashuang Zixue Dan (Melon Frost and Purple Snow Pill). If heat accumulation causes constipation, coma, convulsions, and syncope, a large dose of cooling purgatives is necessary, using Bacui Xijiao Dihuang Tang (Extracted Rhinoceros Horn and Rehmannia Decoction) added with Jin Zhi and Yuanming Fen (refined Mirabilite). After purgation, Zhuye Dihuang Tang (Bamboo Leaf and Rehmannia Decoction) can be used to cool blood and save fluids. In summary, when using dispersing methods for this syndrome, one must guard against drying out fluids; bitter-cold and sweet-cool herbs must be added to clear fire and save fluids. When using cooling methods, one must guard against freezing the pathogen; blood-invigorating and unblocking herbs must be added to prevent stagnation of Qi and blood.
2. Warm-Toxicity causing macules: Not due to missed sweating or purgation. Initially, pulse is vigorous at both superficial and deep levels, with high fever, irritability, restlessness. Externally, there may be swelling and redness of the head and face, sore throat, vomiting of pus and blood, facial redness like brocade patterns, body pain as if beaten internally. Internally, there is oppression, nausea, abdominal pain, mania, restlessness, thirst, or delirious speech with diarrhea. Macules appearing in such cases are bean-sized, round, distinctly purple-black, dense on the chest, back, waist, and abdomen. Pathogenic toxins permeate Ying and Wei, blocking the Triple Burner, scorching Qi and blood. At this time, allowing Baihu Tang to transform macules or Xijiao Daqing Tang to detoxify will cause the pathogen to become more depressed due to cold, leading to untreatable conditions. Only purgation opens the internal blockage, providing an outlet for the pathogen, allowing macule toxins to resolve externally. The treatment method is combining Zicao Chengqi Tang (Indigo Naturalis and Da Huang Decoction) and Bacui Xijiao Dihuang Tang, added with Jin Zhi and Gaojiaoci (Gleditsia Sinensis Spine), which is most effective. In extremely severe cases, symptoms include a foul odor from the whole body, unconsciousness, open mouth with breath emission, black coating with black fissures on the tongue. One must use Shiquan Kuhuan Jiubu Tang (Ten Ingredients Bitter-Cold Rescue and Tonify Decoction), increasing Sheng Shigao (Raw Gypsum) fourfold, administering rapidly in circulation over one day and night with multiple doses. The patient will pass extremely foul red-black stools successively. On the next day, the black fissures on the tongue gradually recede, and symptoms begin to alleviate. If macules do not emerge after purgation, use Xijiao Daqing Tang; if they have emerged but fever persists, remove Shengma and Huangqin from the original decoction, add Xiyangshen (American Ginseng), Xian Shengdi (Fresh Rehmannia), Yin Hu, and Digupi (Phellodendri Cortex), and treat with clearing and moistening. Once macules are fully erupted and external heat subsides, but internal excess causes constipation and occasional delirium, only Xue Geng mixed with Ye's Shenxi Dan (Leaf's Divine Calyx Pill) is needed to clear and drain residual pathogens. Regarding differentiation: red macules indicate Stomach heat; purple indicates Stomach injury; black indicates Stomach rot. Generally, fresh red eruptions are auspicious, even if large. Dense patches of purple indicate half-dead, half-alive status; mixed green-purple colors indicate death in ten cases without survival. Although purple macules are critical, continuous administration of Huanglian Jiedu Tang combined with Xijiao Dihuang Tang can save two or three out of ten. If macules are black and sinking, death is certain.
3. Warm-Toxicity throat scarlet fever (Lanhou Sha): Commonly known as Rotting Throat Scarlet Fever. It mostly occurs in spring and winter, infecting all ages indiscriminately. Onset begins with aversion to cold, followed by high fever, irritability, and thirst. Macules are dense, skin is red like brocade patterns, throat is painful, swollen, and ulcerated, or merely painful without swelling/redness, or even white putrefaction and throat rot. Mild cases eat normally; severe cases have chest oppression, throat obstruction, and inability to eat. Pulse may be wiry-rapid, soggy-rapid, deep-rapid, deep-wiry non-rapid, right Cun exclusively large, both Cun simultaneously deep, or left side additionally tight. Some rashes resolve upon first appearance; others resolve only after eruption. Although the syndrome features intense internal fire, the exterior is often bound by wind pathogens. Physicians seeing intense heat usually prescribe rhino horn, antelope horn, Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Gardeniae Fructus (Zhizi), Phellodendri Cortex (Huangbai), Gypsum Fibrosum (Shigao), and Anemarrhenae Rhizoma (Zhimu). Strong cold-natured suppression often leads to hidden pathogen closure, coma, throat rot preventing eating, delayed untreatable conditions, or downward collapse with sudden danger.
Treatment: Initially, urgently promote muscle-relaxing exterior release to allow warm toxins to reach the exterior, such as Liu's Jiegeng Tang removing Huangqin, adding Zicao, Mudanpi, Gualoupi, and Chuanbeimu; or Modified Puji Xiaodu Decoction removing Banlangen, adding Zihua Dideng, Ye Juye, Daqingye, and Weijing. If Chan Yi (Cicada Slough), Gegen (Puerariae Radix), and Gaojiaoci (Gleditsia Sinensis Spine) are used when rashes are faint and not emerging, they can temporarily aid eruption, but must not be used once rashes appear. In winter, if severe cold binds the rash toxin preventing eruption, temporarily use Honey-fried Mahuang (Ephedrae Herba), 3 fen to 5 fen, utilizing its light ascending nature to reach hair follicles; often effective after one dose, never reuse after appearance. Furthermore, throat scarlet fever always involves phlegm-saliva; formulas must add 4 liang of raw Luobo (Raphani Fructus) and 4 fresh Qingguo (Canarium Fruit), decocted as water substitute. Next is purgation. A prairie fire cannot be extinguished with a cup of water; thus, mere clearing and moistening is ineffective. Primary purgatives are Fenghua Xiao (Weathered Niter) and Sheng Jinwen (Raw Rheum Palmatum Rhizome/Dahuang); secondary are Qing Xiye (Senna Leaf) and Yulijingren (Prunus Semen Purified); tertiary are Dan Haizhe (Aequor Sea Jellyfish) and raw Luobo. Formulas like Chen's Sihuyin (Four Tigers Drink) or Bacui Xijiao Dihuang Tang added with Yuanming Fen and Jin Zhi are most effective. The purgative method follows Wu Keyi's epidemic treatment principles: purgation continues until bowel movements occur several times, pulse becomes calm, body cools, coating turns thin-white, and appetite returns, ensuring no residual pathogens remain and fire does not flare again. This applies to the most severe cases. If after exterior release, exterior pathogens are resolved, fire is blazing, rashes erupted, pulse wiry, throat rotten, tongue crimson, thirst and mental agitation present, but bowels/bladder open, heavy clearing and transforming agents like Chen's Tuoming Yin (Life-Taking Drink) or Xiling Erxian Tang (Rhino-Antelope Two Fresh Decoction) suffice. To clear residual fire, modify Yu's Qingzao Jiufei Tang (Clear Dryness Rescue Lung Decoction), Chen's Qingfei Yin, or Cao's Sangdan Xiebai San (Mulberry-Salvia-Bran White Powder). For recovery, tonify Stomach Yin with Ye's Yangwei Tang (Nourish Stomach Decoction); harmonize Stomach Qi with Jingui Maimendong Tang (Golden Cabinet Lily Bulb Decoction); clear and nourish Lung fluid with Naixiu Zi's Yangyin Qingfei Tang (Nourish Yin Clear Lung Decoction); or nourish Kidney and cool Liver with Sangma Liuwei Tang (Mulberry-Cannabis Six Ingredient Decoction), chosen according to symptoms.
External treatments also supplement internal medicine. Ten essential external methods are selected below:
1. Prepare mouth spreaders: When trismus occurs, forcing metal tools damages teeth. If Wumei (Mume Fructus) and Bingpian (Borneolum Syntheticum) fail to open the mouth, use mouth spreaders gently to widen the jaw ring without damaging teeth.
2. Prepare tongue depressors: To examine the throat, pressing the tongue reveals the condition of the throat gate clearly.
3. Prepare almond-shaped curved knives: For swollen parotid glands blocking the throat gate, making swallowing difficult, use curved knives to drain pus and blood from the gland, widening the throat gate for medication/food intake, avoiding damage to the tonsils ("Diding"), more convenient than Chinese throat guns/knives.
4. Prepare throat mirrors: To inspect the throat gate, revealing hidden details beyond naked-eye vision.
5. Prepare skin needles: To inject serum for urgent relief of throat scarlet fever toxicity, achieving rapid results. This is called Serum Therapy. Shanghai Municipal Council reports confirm its efficacy for early-stage throat scarlet fever.
6. Apply blistering agents to drain toxins: Use Yigong San (Banmao 4 qian, wings/legs removed, fried with rice; Xuejie, Moyao, Ruxiang, Quanxie, Xuanshen each 6 fen; Shexiang, Bingpian each 3 fen, ground fine). Place a bean-sized amount on a plaster, apply to both sides of the external throat lesion. Blisters form in one week; in summer, 2-3 hours suffice. Do not leave too long. Once blisters form, prick them quickly and squeeze out yellow fluid. If purple or dark yellow, apply medicine around the blister, prick again, aiming for pale yellow fluid. Then, mash garlic into a bean-sized paste, apply to Jingqu Acupoint (PC8, radial artery depression at wrist crease), left for males, right for females, cover with clam shell and tie. After several hours, blisters form; prick, wipe dry, to remove toxic gases.
7. Gargle throat to remove toxic saliva: Crush fresh Tu Niuxi (Achyranthes Bidentata Root/Leaf) to extract one bowl of juice, warm in double boiler, gargle frequently. After gargling, bend head to drain toxic saliva. Repeat patiently over ten times until clean. This herb is a holy medicine for throat diseases, reducing swelling, dispersing blood, stopping pain, resolving phlegm. Effective for all throat conditions by removing wind-phlegm and toxic saliva. For red/swollen/white-putrefied throats, dissolve 3 qian Zijin Ding (Purple Gold Tablet) in hot water, gargle when cool, spit out, repeat. This removes throat rot and induces vomiting of wind-phlegm.
8. Blow powder into nose to ventilate and expel phlegm: Throat scarlet fever always obstructs Lung Qi. First use Chuibi Yizi San (Zhuya Zao 7 qian, Xionghuang 2 qian raw, Lilü 1 qian powder, Xiewei 7 tails, ground fine). Blow a little into nostrils to induce sneezing and expel toxic phlegm. For nasal congestion/throat closure, use Hou Bi Se Bi Zao (Toad Venom 7 fen, Xixin 4 fen, Zhensha 3 fen, Shexiang 2.5 li, Bingpian 2.5 li, Zhuya Zao 4 fen, Banxia 3 fen, Xinyi 4 fen, Badou 4 fen de-oiled, Niuhuang 2 fen, Xionghuang 4 fen. Ground ultra-fine. Cut date, remove pit, insert medicine, seal with thread). Insert left nostril for right pain, right nostril for left pain. For children, wrap medicine in cotton. Avoid direct contact with flesh to prevent ulceration. For severe throat closure, plug both nostrils. This has life-saving power for critical throat scarlet fever/closure with breathing failure, superior to Wolong Dan/Zijin Dan/opening methods that fail to induce sneezing.
9. Blow powder into throat to detoxify, remove rot, reduce inflammation, stop pain: First use Lanhou Qufu Yao (Du Niuxi root/leaf juice dried powder 1 liang, Bohe 5 fen, Qingdai 5 fen, Bingpian 3 fen. Mix, store in porcelain bottle tightly sealed against moisture/humidity. If damp, dry and grind again, never bake). Flow out toxic saliva, then blow Xilei San (Xiangya Xie, roasted Zhenzhu Fen each 3 fen, Fei Qingdai 6 fen, Bingpian 3 li; Bi Ke (Swallow nests) 20 pieces, wall ones preferred; Xiniuhuang, roasted human nail, male case uses female nail, female case uses male nail, each 5 li. Roast drugs, place on ground to remove fire qi, grind ultra-fine, store tightly in porcelain bottle. Treats rotting throat seasonal diseases, tonsillitis, gum disease, oral ulcers. For external pathogens unresponsive to other drugs, blow into lesion, saves near-death cases). Remove rot and stop ulceration. Finally use Zhu Huang San (Zhenzhu Fen 6 fen, Xiniuhuang 3 fen, Jing Chuanbei, Longgu each 4 fen, Qingguo He 3 pits. Grind fine, store tightly). Clear residual toxins and generate flesh.
10. Scrape posterior neck to disperse toxins: Apply real Bohe You (Mentha Oil) to the nape. Use a coin to scrape downwards like Gua Sha, over a thousand strokes, until spots appear. Prick spots with porcelain shard edge, suck out bad blood with leeches (Qi Kou). Without leeches, use a small suction cup. Most effective for dispersing toxins. This is the premier method for throat scarlet fever, throat bi, and wind-fire throat diseases.
As for Filthy-Toxicity, it is Damp-Warmth Seasonal Toxicity, also known as Damp-Warmth with Filth or Damp-Filth. Cases commonly called Sha Qi or Sha Hui in summer/autumn mostly belong to this syndrome. Onset with aversion to cold, progressing to pure heat, heavy/distending headache, chest/epigastric fullness, nausea/vomiting, abdominal pain/discomfort, skin heat/sweating, muscle soreness, limb fatigue, right pulse soggy/stagnant, white or yellow tongue coating. Treatment generally favors aromatic transformation of turbidity, such as Modified Huoxiang Zhengqi San. Differentiate based on heat vs. damp predominance:
If heat-dominant: accompanied by yellow-greasy coating, heart vexation, thirst. Use Zhi, Jie, Zhizi, Chi combined with Xiao Xianxiong Tang, added with Qinghao and Huashi.
If damp-dominant: accompanied by white-greasy coating, sticky mouth/no thirst. Use Huopo Erchen Tang added with Peilan Ye, Cangzhu, Baitanxiang, Baiko Mo.
If skin develops soybean-like bumps or nodules, itchy and numb: damp-toxin excreting via skin. Add Duchixiaodou, Tubaling, Lianqiao, Gaojiaoci to previous formula to vent.
If mild white blisters like crystal color appear: combine with Qianjin Weijing Tang to lightly ventilate.
If damp-toxin blocks muscles/body-wide pain: add Qianghuo, Fangfeng, Guizhi, Qinjiao to previous formula to unblock collaterals and disperse.
If damp-toxin blocks chest/diaphragm, Qi stagnation with hiccups: add Guangpi, Danzhuru, Gongdingxiang, Shidi, Chenxiang Zhi to previous formula to open and descend.
