Huangdi asked: The "Jie" (Joint) chapter of acupuncture mentions "resolving confusion," which refers to fully understanding how to harmonize Yin and Yang, balancing excess and deficiency through tonification and sedation. How exactly is this confusion resolved? Qibo replied: When severe wind pathogens invade the body, the blood vessels become unevenly deficient. The deficient side lacks Zheng Qi (True Vital Energy), while the excess side harbors pathogenic factors. This leads to an imbalance in bodily weight and stability, causing the patient to tilt and sway as if intoxicated, unable to distinguish directions, experiencing sudden upward and downward sensations, and reversing constantly. These symptoms are more severe than ordinary confusion. Treatment involves tonifying the deficiency and sedating the excess to restore Yin-Yang balance. Acupuncture achieving this effect works faster than resolving confusion. If evil qi lodges unilaterally in half the body and penetrates deeply into Ying (Nutritive) and Wei (Defensive) levels, the gradual decline of Ying and Wei causes Zheng Qi to depart, leaving only the evil qi, resulting in Pian Ku (hemiplegia). If the evil qi is shallow, it manifests as unilateral pulse pain. In cases of Wind Ni (Wind Reversal), there is sudden limb pain, a sensation of damp cold over the body, frequent sighing with chills, irritability when hungry, and symptom fluctuation after eating. One should select points from the Hand Taiyin Lung Meridian and its interior-exterior paired meridian (Hand Yangming Large Intestine Meridian), as well as the Foot Shaoyin Kidney Meridian and Foot Yangming Stomach Meridian. For muscle tremors due to cold, select Ying-level points; for bone tremors due to cold, select Jing-Well and Jing-River points.

Pian Ku is characterized by unilateral immobility and pain, while speech remains normal and intelligence is intact, though limbs cannot be controlled. If mental confusion is mild and speech is intelligible, the condition is treatable; if severe enough to cause aphasia, it is untreatable. For diseases originating in the Yang aspect and later entering the Yin aspect, one should first treat the Yang aspect and then the Yin aspect, carefully assessing whether the Qi is superficial or deep to determine point selection. Severe wind disease causing heavy joints and loss of eyebrows and beard is called Da Feng (Great Wind). Treatment involves needling the muscle layer until sweating occurs for 100 days, followed by needling the bone marrow level until sweating occurs for another 100 days, totaling 200 days, until hair regrows, at which point needling stops.

Someone asked: There is a condition where the body feels hot, limbs are flaccid and weak, sweating profusely like bathing, aversion to wind, and shortness of breath. What is this disease? It is called Jiu Feng (Wine Wind), caused by excessive alcohol consumption leading to damp-heat. Treat with Zexie (Alisma Rhizome), Baizhu (Atractylodes Macrocephala Rhizome) each in ten parts, and Mixian (Dendrobium nobile stem) in five parts. Mix these and take three finger-pinches per dose after decoction. If the body has suffered trauma with significant blood loss, contracted wind-cold, or sustained falls, resulting in flaccid limbs that cannot be held, it is called Ti Jie (Body Unbinding). Select the San Jie Jiao (Three Intersections) point below the umbilicus. San Jie Jiao refers to Guanyuan (CV4), located three cun below the umbilicus, where the Foot Yangming Stomach Meridian, Foot Taiyin Spleen Meridian (another version says Foot Taiyang Bladder Meridian), and Ren Mai intersect.
