For "Duikou" (Opposite Neck Carbuncle) caused by seven emotions (emotional factors), tonification is appropriate; for that caused by six excesses (wind, cold, summerheat, dampness, dryness, fire), dispersing and releasing the exterior are appropriate. If dampness and heat have been clashing within the body since long ago, leading to a parotid carbuncle on one side, which has ulcerated for a long time with excessive pus and induration that does not resolve, resolution and transformation should be employed.
*Adenophorae Radix* (Nanshashen), *Moutan Cortex* (Danpi), *Coicis Semen* (Yimi), *Forsythiae Fructus* (Lianqiao), *Fritillariae Bulbus* (Dabei), *Glycyrrhizae Radix et Rhizoma* (Gancao), *Lonicerae Japonicae Flos* (Yinhua), *Paeoniae Radix Rubra* (Chishao), Lotus Root (*Ou*), *Ardisiae Folium* (Gonglaoye).
The pus of Duikou has gradually cleared, and the swelling has subsided; it seems suitable to begin consolidation, yet the previous method should still be used with modifications.
Add *Angelicae Sinensis Radix* (Danggui) to the previous prescription, remove *Ardisiae Folium* (Gonglaoye).
Wind-damp-heat interact and fumigate the upper part; the parotid malar carbuncle is swollen, hard, and has a head, while the root of the Duikou sore is diffuse and ill-defined. Neither is a minor ailment, and the fu-qi (visceral qi) is unsmooth. It is appropriate to use *Neishu Huanglian Tang* (Internal Draining Coptis Decoction) with modifications.
*Menthae Haplocalycis Herba* (Bohe), *Coptidis Rhizoma* (Huanglian), *Paeoniae Radix Rubra* (Chishao), *Angelicae Sinensis Radix* (Danggui), *Forsythiae Fructus* (Lianqiao), *Citri Reticulatae Pericarpium* (Chenpi), *Lonicerae Japonicae Flos* (Yinhua), *Glycyrrhizae Radix et Rhizoma* (Sheng Gancao), *Platycodonis Radix* (Jiegeng), *Fritillariae Bulbus* (Dabei), *Gardeniae Fructus* (Hei Zhi), *Phragmitis Rhizoma* (Dan Zhuye).
Liver qi combined with phlegm stagnates; scrofula (Luoli) grows densely on both sides of the neck, and the Zhongwan area feels obstructed. Nourishing yin, clearing liver fire, and resolving phlegm should be employed.
*Chuanxiong Rhizome* (Chuanxiong), *Angelicae Sinensis Radix* (Danggui), *Cyperi Rhizoma* (Xiangfu), *Prunellae Spica* (Xiakucao), *Citri Reticulatae Pericarpium* (Chenpi), *Sargassum* (Haizao), *Poria* (Fuling), *Curcumae Radix* (Guang Yujin), *Bombyx Batryticatus* (Jiangcan), *Fritillariae Bulbus* (Dabei), *Eupatorii Herba* (Peilan), *Citrus Unshiu Leaf* (Juye).
Liver fire causes scrofula in the neck and nape; from the shoulder blades to the groin there is fever, the pulse is rapid, and the meridians throughout the body feel pulling and painful. *Xiaoyao San* (Free and Easy Wanderer Powder) with modifications is indicated as the main treatment.
*Angelicae Sinensis Radix* (Danggui), *Menthae Haplocalycis Herba* (Bohe), *Codonopsis Radix* (Huang Shashen - note: likely typo for Dangshen or similar, translated as Codonopsis based on context of tonification, though literal is Yellow Sand Ginseng), *Forsythiae Fructus* (Lianqiao), *Glycyrrhizae Radix et Rhizoma* (Fen Gancao), *Paeoniae Radix Rubra* (Chishao), *Bombyx Batryticatus* (Jiangcan), *Moutan Cortex* (Danpi), *Bupleuri Radix* (Chaihu), *Fritillariae Bulbus* (Dabei), *Prunellae Spica* (Xiakucao).
Liver depression fails to disperse; qi-fire combines with phlegm and coagulates on the left side of the neck. "Shirong" (Loss of Glory) presents with hard swelling and tendon/meridian pulling pain. Clearing the liver and resolving depression is appropriate.
*Chuanxiong Rhizome* (Chuanxiong), *Angelicae Sinensis Radix* (Danggui), *Paeoniae Radix Alba* (Baishao), *Rehmanniae Radix* (Shengdi), *Polygoni Multiflori Caulis* (Ye Jiaoteng), *Bombyx Batryticatus* (Jiangcan), *Concha Meretricis et Cyclinae* (Gefen), *Fritillariae Bulbus* (Dabei), *Uncariae Ramulus cum Uncis* (Gougou), *Prunellae Spica* (Xiakucao), *Moutan Cortex* (Danpi), *Fortunella Leaf* (Jinjiye).
For Shirong with hard swelling and pain radiating to the shoulders and back, add three qian of *Gardeniae Fructus* (Hei Shanzhi) to the original formula, and remove *Polygoni Multiflori Caulis* (Ye Jiaoteng) and *Uncariae Ramulus cum Uncis* (Gougou).
Excessive labor and overthinking damage the Heart and Spleen; Liver Wood loses its smooth flourishing nature, and qi transformation turns into fire. Turbid phlegm from the Yangming channel ascends, causing hard swelling on the left side of the neck, resulting in Shirong. There is hot stabbing pain, with phlegm-fire intermingling in the collaterals. Since administering medication, the swelling and heat have slightly decreased, but movement causes qi to rise, and drinking/swallowing meets obstruction. Lying down brings gradual relief. The Lung is the master of qi, and the Kidney is the root of qi. When kidney water fails to nourish the liver, the storage function fails, causing liver qi to rush upward directly. This is due to the dysfunction of the Lung and Stomach. It is appropriate to nourish kidney water, soften the liver, and guide qi back to the Kidney. However, the tongue coating is white and slippery with red tips on both sides, indicating injury to the yin aspect. Upper jiao phlegm-qi is obstructed and depressed; it seems best to first clear the upper jiao and simultaneously calm the liver wood, so that depression resolves and phlegm dissipates, allowing diet to proceed smoothly, after which kidney tonification can be discussed.
Administer medicines for clearing the lung and resolving phlegm.
Liver depression combined with phlegm affects the right side of the neck with Shirong, presenting as hard swelling. The condition has persisted for five months, with pulling pain in the chest, back, neck, and nape. Both Liver and Spleen are injured, and qi and blood are simultaneously damaged. Tentatively propose replenishing qi and nourishing prosperity (blood/yin).
*Angelicae Sinensis Radix* (Dangguishen), *Codonopsis Radix* (Dangshen), *Atractylodis Rhizoma* (Dongzhu), *Paeoniae Radix Alba* (Baishao), *Chuanxiong Rhizome* (Chuanxiong), *Pinelliae Tuber* (Qing Banxia), *Citri Reticulatae Pericarpium* (Chenpi), *Glycyrrhizae Radix et Rhizoma* (Zhi Gancao), *Rehmanniae Radix* (Chao Shengdi), *Eupatorii Herba* (Peilan), *Jujubae Fructus* (Hongzao), *Zingiberis Rhizoma Recens* (Wei Jiang).
