Postpartum diarrhea is generally caused by general weakness, food stagnation, and spleen-earth deficiency failing to transform dampness. However, since lochia has not yet been expelled and the spleen and stomach are weak, it is difficult to use drying or tonifying methods directly. The treatment method should first involve taking one dose of Shenghua Decoction (Shenghua Tang), followed by adding 1 qian of Poria (Fuling) to promote urination. Once blood transformation occurs, then decisions regarding tonification, digestion promotion, or drying can be made, thereby avoiding errors such as obstruction leading to worsening symptoms or deficiency-induced incontinence. If diarrhea occurs immediately after delivery, Jiajian Shenghua Decoction (Modified Shenghua Decoction) is suitable. If chronic diarrhea persisted during pregnancy and continues postpartum, Shenling Shenghua Decoction (Ginseng and Poria Shenghua Decoction) should be used as a temporary measure to prevent collapse from deficiency. For prolonged diarrhea or spleen-deficiency diarrhea, hundreds of doses of Shenling Lianzi Yin (Ginseng, Poria, and Lotus Seed Drink) must be taken; even for those with emaciation and blurred vision, Danxi's Emergency Rescue Formula (Danxi Jijiu Fang) may be administered.
Jiajian Shenghua Decoction: Chuanxiong Rhizome (Chuanxiong) 1 qian, Angelicae Sinensis Radix (Danggui) 1.5 qian, Black Ginger (Heijiang), Honey-fried Licorice Root (Zhigancao) each 0.5 qian, Peach Kernel (Taoren) 10 grains, Poria (Fuling) 1.5 qian, Lotus Seed Meat (Lianrou), Chebula Fruit (Hezi) each 0.8 fen, one slice of Fresh Ginger (Jiang), decocted in water. If diarrhea persists after two doses, add 3 qian of Ginseng (Renshen).
Shenling Shenghua Decoction: Chuanxiong Rhizome (Chuanxiong) 1 qian, Angelicae Sinensis Radix (Danggui) 2 qian (soil-fried), Charred Ginger (Jiangtan), Honey-fried Licorice Root (Zhigancao) each 0.5 qian, Poria (Fuling) 1 qian, Tangerine Peel (Chenpi) 1 qian. [Text indicates additions for diarrhea and qi deficiency but lists no specific herbs].
Shenzhu Lianzi Yin: Ginseng (Renshen), Charred Atractylodes Macrocephala Rhizome (Jiaozhuzhu) each 2 qian, Poria (Fuling) 1 qian, Angelicae Sinensis Radix (Danggui) 1.5 qian, Stir-fried White Peony Root (Chaoshao) 0.8 fen, Honey-fried Licorice Root (Zhigancao) 0.5 qian, Tangerine Peel (Chenpi), Cimicifugae Rhizoma (Shengma) each 0.3 qian, Chinese Yam (Shanyao) 1 qian, Lotus Seeds (Lianzi) 10 grains, one slice of Fresh Ginger (Jiang). If abdominal pain occurs, add 0.5 qian of Processed Ginger (Paojiang).
Danxi's Emergency Rescue Formula: Ginseng (Renshen) 5 qian, Atractylodes Macrocephala Rhizome (Baizhu) 3 qian, Poria (Fuling) 3 qian, Prepared Aconite Root (Zhi Fu Zi) 1 qian, decocted in water and taken orally.
Within seven days before or after delivery, suffering from dysentery with red and white discharge, tenesmus, and frequent bowel movements, is the most difficult to treat. One wishes to harmonize qi and blood and expel pathogenic turbidity, yet fears the postpartum primordial deficiency. One wishes to nourish ying-blood and boost qi, greatly tonifying the spleen and kidney, yet fears that tonification will retain stasis and pathogens. Only by using Shenghua Decoction, removing Black Ginger and substituting it with Poria (Fuling) and Aucklandiae Radix (Muxiang), can one effectively eliminate lochia while simultaneously treating dysentery, allowing both actions to proceed without conflict. Subsequently, Jiawei Xianglian Wan (Modified Xianglian Pills) should be taken. After one or two days, if the condition slightly subsides, safety can be ensured. If after seven days, brown tenesmus and deficiency-type dysentery occur, supplementation is undoubtedly required. For mothers with strong constitutions whose confinement period has exceeded twenty days, Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), White Peony Root (Shaoyao), and Aurantii Fructus (Zhike) can be added to Shenghua Decoction, along with Jiawei Xianglian Wan. Chengqi Decoction (Chengqi Tang) and other purgatives must absolutely not be used. If the mother is weak, even if more than a month has passed, harsh purgative agents acting on accumulation must strictly not be used. If postpartum yin-deficiency leads to bloody dysentery that worsens over time, Siwu Decoction (Four Substance Decoction) with added Ginseng (Renshen) should be used.
Jiajian Shenghua Decoction: Chuanxiong Rhizome (Chuanxiong) 2 qian, Angelicae Sinensis Radix (Danggui) 4 qian, Peach Kernel (Taoren) 10 grains, Poria (Fuling) 0.5 qian, Tangerine Peel (Chenpi) 0.5 qian, Aucklandiae Radix (Muxiang) 0.2 fen. For red dysentery with abdominal pain, add 0.2 fen of Amomi Fructus (Sharen). Decoct to obtain 0.8 portion of liquid, take with 30 pills of Xianglian Wan.
Jiawei Xianglian Wan: Consists of Xianglian Wan plus half the amount of Lotus Seed Powder (Lianrou Fen) mixed into pills.
Jialian Shenghua Decoction: Chuanxiong Rhizome (Chuanxiong) 1.5 qian, Angelicae Sinensis Radix (Danggui) 3 qian, Stir-fried White Peony Root (Chaobaishao) 1 qian, Coptidis Rhizoma (Chuanlian) 0.6 fen (stir-fried with ginger juice), Aurantii Fructus (Zhike) 0.5 qian, Poria (Fuling) 1 qian, Licorice Root (Gancao) 0.4 qian, Aucklandiae Radix (Muxiang) 0.3 qian, decocted in water and taken orally. For severe accumulation, add Hawthorn Fruit (Shanzha) 1 qian.
