Whenever during pregnancy there is edema of the legs and knees, with fluid oozing from the instep and toes, accompanied by shortness of breath and chest fullness/stuffiness, this is caused by Spleen deficiency failing to control water metabolism, leading to blood transforming into water-dampness.
This condition is named Zi Man (Submersion of the Fetus / Hydrops gravidarum), also known as Zi Qi (Fetal Gas). It is suitable to use Jia Wei Tian Xian Teng San (Modified天仙藤 Decoction). If the Spleen Qi is extremely weak, it is suitable to take Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) concurrently, or use Zi Man Shu Tai Yin (Decoction for Submersion of the Fetus to Bind the Fetus) from Volume Nine.
Jia Wei Tian Xian Teng San (Modified天仙藤 Decoction): Tian Xian Teng (Dangshen's Vine Stem, washed, slightly stir-fried) 6 fen, Mu Gua (Chaenomeles Fruit) 1 qian, Xiang Fu (Cyperus Rhizome) 6 fen, Zi Su (Perilla Leaf) and Chen Pi (Aged Tangerine Peel) each 4 fen.
Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction): Ren Shen (Ginseng Root) and Zhi [Missing Herb] (Prepared [Herb]) each 1.5 qian, Zhi Gan Cao (Honey-fried Licorice Root) 5 fen, Bai Zhu (Atractylodes Macrocephala Rhizome) and Dang Gui Shen (Angelica Sinensis Root Body) each 1 qian, Chen Pi (Aged Tangerine Peel) 7 fen, Chai Hu (Bupleurum Root) and Sheng Ma (Cimicifuga Rhizome) each 3 fen; decoct in water and take orally.
Note: During the seventh or eighth month of pregnancy, edema of both feet is due to the downward flow of amniotic fluid dampness. Mild swelling is called Zhou Jiao (Wrinkled Feet), which facilitates easy delivery. Only when swelling is severe or yellow fluid oozes out should the aforementioned formula be taken.
Whenever during pregnancy the fetal Qi is disharmonious, rushing upward to the heart and abdomen, causing unbearable distension and pain, it is called Zi Xuan (Suspended Fetus), treated with Zi Su San (Perilla Powder).
Zi Su San (Perilla Powder): Zi Su (Perilla Leaf), Chen Pi (Aged Tangerine Peel), Bai Zhu (Atractylodes Macrocephala Rhizome), Fu Pi (Areca Peel), Dang Gui (Angelica Sinensis Whole Root), and Chuan Xiong (Ligusticum Rhizome) in equal parts; Ren Shen (Ginseng Root) and Gan Cao (Licorice Root) at half the amount; add five slices of Sheng Jiang (Fresh Ginger) and five cun of Cong Bai (Scallion White); decoct in water and take on an empty stomach. Another method is to burn Lian Fang (Lotus Sepal) to ash and swallow with wine, which leads to recovery.
An Tai Shun Qi Yin (Fetus-Anchoring and Qi-Smoothening Decoction): This is the aforementioned formula without Fu Pi (Areca Peel), adding three grains of Sha Ren (Amomum Fruit) and 3 fen of Mu Xiang (Saussurea Root) ground into juice, added with Sheng Jiang (Fresh Ginger) for decoction.
Note: From conception to the fourth or fifth month, at this time, Minister Fire nourishes the fetus. Excessive fire causes fetal heat, making Qi rush to the heart, often resulting in this symptom.
Furthermore, there is Zi Fan (Fetal Restlessness). Whenever during pregnancy there is a startled heart, timidity, restlessness, and unease, it is named Zi Fan, suitable for taking Zhu Ye An Tai Yin (Bamboo Leaf Fetus-Anchoring Decoction).
Zhu Ye An Tai Yin (Bamboo Leaf Fetus-Anchoring Decoction): Ren Shen (Ginseng Root) 3 fen, Bai Zhu (Atractylodes Macrocephala Rhizome), Huang Qin (Scutellaria Root), and Dang Gui (Angelica Sinensis Whole Root) each 2 qian, Suan Zao Ren (Ziziphus Seed) and Yuan Zhi (Polygala Root) each 8 fen, Chen Pi (Aged Tangerine Peel) 3 fen, Chuan Xiong (Ligusticum Rhizome) 7 fen, Sheng Gan Cao (Raw Licorice Root) 4 fen, Sheng Di Huang (Rehmannia Root) 1.5 qian, Mai Dong (Ophiopogon Tuber) 1 qian, Zhu Ye (Bamboo Leaf) 10 pieces; use Sheng Jiang (Fresh Ginger) and Da Zao (Jujube) as guides. If there is Phlegm, add Zhu Li (Bamboo Juice) and Jiang Zhi (Ginger Juice); if deficiency is severe, double the dose of Ren Shen (Ginseng Root); if diarrhea occurs, add Qian Shi (Euryale Seed) and reduce the dose of Sheng Di Huang (Rehmannia Root).
Note: This symptom also mostly occurs during the fourth and fifth months, when Minister Fire is in charge. Or because Sovereign Fire prevails in summer, both can invade the Lung Metal leading to irritability. Tang Jianzhong stated: Add Zhi Mu (Anemarrhena Rhizome) if caused by Minister Fire; add Chuan Lian (Coptis Rhizome) if caused by Sovereign Fire.
