The brain is the gathering point of all Yang meridians. For injuries to the Taiyang acupoint, fontanelle (anterior fontanelle), or cranial vault, if there is any breakage, one must part the hair to locate the injury, cut away the hair near the wound, and then apply medication. If blood gushes out, chew a piece of Juncus effusus (Jiedixin) into a ball, dip it in Taohua San (Peach Blossom Powder) to pack the wound, which will stop bleeding immediately. For minor injuries, this is unnecessary. If rotting and foul odor occur, first decoct Xiaofeng San (Wind-Expelling Powder) for oral administration, then decoct Xinxiang San (Fragrant Powder) to wash the area. During washing, avoid wind exposure, as cold and heat symptoms may worsen and become difficult to treat. If the head and face are swollen, this indicates wind pathogen invading the interior; Xiaofeng San should be taken. For swelling at the affected site, mix Shengshen San (Holy Spirit Powder) with honey, or use ginger juice and pure wine for topical application. If bone marrow exudes, use Ansui San (Marrow-Anchoring Powder) mixed with clear tea for topical application; using both medicines together is particularly effective. If the skull is depressed, apply Baijin San (White Gold Powder) combined with Huaiwu San (Huaiwu Powder) topically to lift the depression immediately, and take medicine internally for recovery.
The face has seven orifices, with the eyes being the most important for life. Treatment requires meticulous care. If the eyeball protrudes outside the eyelid, push it back in while warm, apply Shengshen San topically to reduce blood stasis and swelling, and continue internal medication. If the cornea breaks and fluid leaks, the eye will likely be lost. If the eyelid turns inward, gently flip it back to its original position and apply Shengshen San. If blood invades the eye, decoct tender branches of peach and willow, Rehmanniae Radix (Shengdihuang), and Pheretima (Dilong) in water, soak lean pork leg meat in it, and apply to the eye. Secretly prescribe Huoxue Zutong San (Blood-Invigorating and Pain-Stopping Powder) and formulas for clearing the head and face; other treatments follow standard protocols for traumatic bleeding.
For mandibular dislocation, have the patient sit still and massage the area over a hundred times until the mouth opens. The practitioner inserts both thumbs into the mouth, holds firmly, pulls down slightly, then pushes up to reduce the joint. Keep a handkerchief supporting the jaw for an hour before removing.
For injury to the alveolar bone, palpate and manipulate to realign, apply Shengshen San topically, and externally support the lower jaw with a silk handkerchief tied to the top of the head. Extract loose teeth; straighten mobile ones with chopsticks. For persistent bleeding, rinse with a decoction of Galae Fructus (Wubeizi) and Alumen (Baifan). Dissolve Baijin San in rice water and hold in the mouth, or pack with Taohua San.
For neck stiffness after falling from height, administer Xiaofeng San or Zutong San plus Biyao Hunhun San (Numbing Powder). Have the patient lie supine, support the chin with a silk ribbon tied to the crown, gather the hair with the ribbon, and ensure the head lies flat. The practitioner sits on the ground, extends legs, places feet on the patient's shoulders, and slowly applies traction to realign. Apply Shengshen San mixed with fresh ginger juice, leek juice, wine, and vinegar. Bind firmly and take Xuntong Zutong San daily.
For clavicle fracture, administer Xiaofeng San, Zutong San, plus Biyao Hunhun San. Manipulate to realign. Apply Shengshen San mixed with honey. Use a section of bamboo, sized to the injury, split into two. Shape one piece to fit the bone gap, place cotton under the arm, and bind with a silk ribbon from the armpit across the opposite shoulder. Take medicine for effect.
For fractured and dislocated clavicle, administer Xiaofeng San, Zutong San, plus Biyao Hunhun San. Prepare medicated towels soaked in Xinxiang San decoction to wash the shoulder and relax the joint. Have the patient lie on their side. An assistant stands in front, pulling the patient's arm level with the shoulder, while pressing against the patient's ribcage with a foot to open the space. The practitioner stands behind, manipulates the bones to align, tests mobility by raising the arm above the head and across the chest to touch the opposite sternum without shaking. Apply Shengshen San mixed with ginger and leek juice. Cover with paper-wrapped fir bark, and bind with a silk ribbon from the injured armpit to the opposite shoulder. Pierce holes in the bark for moisture. Take Jiajian Huoxue Xuntong San daily.
For scapular dislocation, where the lower segment acts as a pestle and upper body as a mortar, first take Zutong San plus Biyao Hunhun San. Wash the area with Xinxiang San decoction to relax tendons and bones. For left-sided dislocation, have the patient lie down. An assistant sits by the left knee, bends the left leg, steps on the patient's left ribcage, and ties a belt around the patient's elbow to the assistant's waist. The assistant supports the elbow, leans forward, arches back, and slowly applies traction to reduce the joint into the axilla. If the bone retracts inward, have the patient lie on their side, use a footstool to rotate the leg, have an assistant sit on the stool, suspend the patient's elbow via a ribbon over their shoulder, step on the ribcage, and lift the shoulder/elbow with traction. Reduce the joint, test mobility (arm overhead and across chest), then apply Shengshen San. Bind with two silk ribbons: one from injury to opposite armpit, one to the current shoulder. Stuff cotton in the armpit for stability. Take Xiaofeng San and Zutong San daily.
For bilateral arm fractures, administer Xiaofeng San, Zutong San, plus Biyao Hunhun San. Prepare thin splints from fir bark, wrap in cotton paper, oil them, and weave binding ropes into a lattice structure to allow ventilation. Soften tendons with hot medicinal water. Have the patient lie down, tie elbows to the practitioner's waist. The practitioner sits by the knees, grasps the elbows, steps on the armpits, arches back, and applies slow traction to realign. Apply Shengshen San mixed with ginger, leek juice, and vinegar on oiled cloth. Splint securely, sling the arm under the neck, and encourage frequent flexion/extension to prevent tendon stiffening. Take Jiajian Huoxue Zutong San daily. If blisters form under splints, do not pop; apply Heishen San mixed with sesame oil.
For elbow/wrist dislocations (colloquially "pang KT"), administer Zutong San plus Biyao Hunhun San. Support and reduce the joint. Test mobility. Weave short splints with binding rope, leaving 2 fen gaps between joints for flexibility.
For wrist fractures, treatment is difficult. Avoid overly cooling medications. Do not keep the arm constantly suspended; encourage movement. When sitting, rest on a table; when lying, move limbs frequently. After three days, gradually test range of motion by reaching behind the head and back. Swelling should be treated with non-cooling topical agents to prevent tendon stiffness.
For dorsal hand fractures, administer previous medicines. Have patient lie supine. Practitioner steps on armpit, lifts three middle fingers with one hand for traction, and reduces with the other. Apply splints made of wide fir bark spanning palm to knuckles, with a hole for wrist flexion. Add smaller splints along the forearm sides. Bind with five ropes, securing the thumb base. Sling under neck. Test mobility after three days. Take medicine.
For crushed palm flesh, realign bones. Apply Baijin Shengji San mixed with sesame oil to the wound, surround with Shengshen San mixed with honey. Cover with paper-wrapped fir bark and soft bamboo leaf. Bind securely. Internal medicine may be omitted.
For finger fractures, realign bones and skin. Stop bleeding with Taohua San. Wrap the finger in a large soft bamboo leaf, secure with paper, apply Baijin Shengji San mixed with sesame oil, and bind with silk. Re-moisten with sesame oil next day. On the third day, reapply Baijin Shengji San and take Huoxue Zutong San. Alternatively, use Shengshen San mixed with honey.
For shoulder dislocation, if the left arm is dislocated, the practitioner uses the right hand to grasp it, kneeling on the patient's side, and pulls with a belt. If the bone points upward, support it manually until relaxed, then align the hand with the shoulder level before applying medication. Pack paper in the armpit. Bind with two ribbons crossing from one shoulder to the opposite armpit. Take Zutong San daily.
For lumbar/spinal fractures, have the patient lie prone on a bench with a heavy roller under the abdomen. Tie shoulder blades and feet to the bench to curve the spine, lifting the fracture site. Press a curved pole along the spine to slowly reduce the fracture. Apply Shengshen San, cover with paper-wrapped fir bark, and bind tightly with a warming abdominal binder. Take Jiajian Huoxue Zutong San daily.
For intercostal muscle/bone fractures, no splinting is needed; take Jiajian Zutong San daily.
For hip (Huantiao) dislocation, this is difficult to treat. Shortened limb cases are easier than lengthened ones. Take Jiajian Huoxue Zutong San daily.
For leg fractures, administer previous medicines. Have patient lie supine, tie chest/armpits to bench. For right leg injury, extend left leg, bend right leg. Practitioner stands by the bench, steps on the right buttock. An assistant ties a belt around the right tibia and pulls for traction. Practitioner reduces the bone, holds firm, extends the leg to check alignment, applies medication, and splints. Take Jiajian Huoxue Zutong San daily.
For knee fractures/dislocations, administer previous medicines. Use a woven bamboo hoop to encircle the kneecap, padded with silk, with two side straps. Have patient lie supine, extend leg. Practitioner reduces the bone, secures the hoop, applies Shengshen San inside and out, wraps in grass paper to keep clean. Take Huoxue Zutong San daily.
For tibial fractures, treat similarly to femoral fractures.
For ankle dislocations causing limping, administer previous medicines. Use two large fir bark splints extending from calf to sole, with a hole for the ankle. Use another large splint for the sole, extending to the heel, leaving sides open. Weave smaller splints into a lattice structure... [text incomplete].
For toe fractures, follow finger fracture protocol. Formulas follow.
