Although tongue coating should be distinguished by color, yellow indicates stomach fire (Wei Huo), and white indicates lung heat (Fei Re). However, the appearance of a black coating is extremely ominous; in measles cases, a black coating signifies the exhaustion of Heart Qi.
The tongue is the sprout of the Heart. The Heart governs Ministerial Fire (Jun Huo), and measles pathology is inherently fire-related. Therefore, coatings appear on the tongue, categorized into three types: yellow, white, and black. A white coating indicates lung heat, while a yellow coating indicates spleen and stomach heat. Both require clearing heat, resolving the exterior, and promoting free flow. Treatment must also be guided by a comprehensive analysis of external symptoms. For instance: Xuan Du Fa Biao Tang (See Clause 5) with Shengma (Cimicifugae Rhizoma), Jiegeng (Platycodonis Radix), and Gancao (Glycyrrhizae Radix et Rhizoma) removed; Gegen Jie Ji Tang (See Clause 5) with Chishao (Paeoniae Radix Rubra) and Gancao removed; Gegen Shu Xie Tang (See Clause 33), Fangfeng Bai Du San (See Clause 5) with Jiegeng and Gancao removed; and Suge Tang (See Clause 6) with Chaihu (Bupleuri Radix), Chishao, and Gancao removed. These formulas may all be modified according to specific symptoms. A purely black coating indicates the exhaustion of Heart Qi. A black coating that is moist suggests heat invading the Blood level (Xue Fen); a black coating that is dry suggests heat invading the Qi level (Qi Fen). Both are critical conditions. Regardless of whether the disease is in its early or late stage, treatment should uniformly focus on clearing heat and promoting free flow. Baibu Jiedu Tang (See Clause 26) is suitable. For moist black coatings, use more Shengdihuang (Rehmanniae Radix) and Yuanshen (Scrophulariae Radix); for dry coatings, double the dosage of Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), Maidong (Ophiopogonis Radix), and Danzhuye (Lophatheri Herba), adding Digupi (Cortex Lycii Radicis) and Mutong (Akebiae Caulis). If the black color gradually recedes, it is auspicious; if it does not recede, the condition is incurable.
Dry lips indicate fire-heat in the Stomach and Spleen meridians. Redness and chapping indicate severe heat, while pale whiteness indicates mild heat. Only when the lips are purple-black and dry are bitter-cold herbs appropriate.
Lip dryness results from heat in the Stomach and Spleen meridians. There are three presentations. First, pale-white and dry lips indicate mild heat; treat with Baibu Tang (See Clause 26) removing Gancao and Jingmi (Glumi Semen), adding Maidong, Mutong, and Niubangzi (Arctii Fructus). Second, red and dry lips indicate extreme heat depth; treat with Baibu Jiedu Tang (See Clause 26) adding Yuanshen, Niubangzi, Lianqiao (Forsythiae Fructus), and Mutong. Third, purple-black and dry lips indicate severe and intense heat; treat with Baibu Jiedu Tang (See Clause 26) adding Xijiao Zhi (Rhinocerotis Cornu Juice), Yuanshen, Niubangzi, Mutong, and Cheqianzi (Plantaginis Semen). One must only adjust the clearing of heat and promotion of fluid distribution according to the severity of symptoms to be considered a skilled physician.
